Speech Background History: Arielle Vasquez is a 1-year 5-month-old female who was referred by her pediatrician for a comprehensive speech and language evaluation due to limited language skills and delayed milestones. Arielle’s mother served as the informant for the evaluation. The evaluation was performed in English, as this is the primary language at home. Arielle’ mother reported that she has very intense stomach issues and has been going to a GI since she was younger. Mother reported she is on two different medications to assist with bloating and using the restroom more consistently. Mother reported she is a very picky eater and is currently pending an evaluation at the same facility. Mother reported Arielle was born full term however was a risk baby as mother had a tumor outside of her placenta. Mother reported Arielle had hiccups everyday and at the 2 month park Arielle was hospitalized due to a liver transplant. Mother reported Arielle has been delayed on her milestones including walking, crawling, and talking. Mother reported she only says approximately 4 words at this time. Mother reported she is allergic to amoxicillin. Mother stated Arielle can demonstrate some aggressive behaviors such as hitting especially when it comes to her brother and can be very anxious when people are close to her.

Speech Assessment Summary: Arielle is a 1 year, 10 month old female who was re-evaluated to determine continued eligibility of speech and language services. Per parent report, Arielle has made progress and is now using signs at home and in therapy. However, she continues to be non-verbal. Arielle was observed that she remains shy around unfamiliar communication partners. Arielle was observed to consistently put the thumb in her mouth. The Preschool Language Scales (PLS-5) was used to determine progress on receptive and expressive language development. The following were her scores: AUDITORY COMPREHENSION - SS 92, Percentile Rank 30, Age Equivalent 1;6 EXPRESSIVE COMMUNICATION - SS 98, Percentile Rank 45, Age Equivalent 1;8 TOTAL LANGUAGE SCORE - SS 95, Percentile Rank 37, Age Equivalent 1;7 Arielle has demonstrated progress in her behavior in speech, she is walking independently to the speech room without crying and requesting for mother. Without mother in the room Arielle is engaging more with therapist. Arielle is imitating sings and using more signs than verbalizations. Arielle demonstrates a shy and timid demeanor, she puts her hands in her mouth constantly and fidgets with her clothing. Arielle does not like to interact with other people in the clinic or anything that is out of routine. Arielle requires maximum cueing, modeling, and redirection to attend to the tasks presented. Based on formal assessment, parent interview, and clinical observation, Arielle no longer presents with a mixed expressive/receptive language delay. Speech therapy is no longer warranted at this time. Arielle will be discharged from speech therapy at the end of current authorization period.

Assessment of Goals/Response to Treatment: Arielle presented as alert, cooperative, and oriented throughout the session, demonstrating a positive disposition for therapeutic activities. Significant progress was noted towards the play and socialization goal, with the patient maintaining joint attention and reciprocal engagement during structured play activities, such as peek-a-boo and singing, for periods of up to two minutes in 7 out of 10 opportunities. Regarding receptive language, the patient is currently meeting the established objective of following simple one-step directions incorporating core vocabulary. She successfully followed commands such as "give me" and "open" with minimal gestural cues in 6 out of 10 trials presented during the session. Progress in expressive language is also evident, as the patient is exceeding the targeted goal. She consistently utilized a total communication approach, independently producing vocalizations and approximations for exclamatory sounds like "uh-oh" and imitating animal sounds such as "moo" with 70% accuracy across modeled opportunities.

Arielle was born full-term via vaginal delivery with no complications, though a tumor was attached to the outside of the placenta during pregnancy. She experienced frequent hospitalizations from birth to 6 months due to vomiting, choking, and breathing difficulties when lying flat. At 2 months old, she was admitted to the ICU for elevated liver levels and was considered for an emergency liver transplant, though her levels stabilized without surgical intervention. Due to ongoing gastrointestinal concerns, she has been followed by a pediatric GI since 1 month of age and was diagnosed with GERD. Feeding difficulties began early, with purees not introduced until around 8–9 months due to her medical status. Arielle continues to reject all table foods despite repeated exposure to a variety of textures and flavors. Her current diet is limited to items such as bananas, applesauce, yogurt, crackers, cereal (select types), Gerber snacks and fruit pouches, and Ensure drinks. Her feeding difficulties appear to be multifactorial, involving the four core domains of pediatric feeding disorders. Medically, concerns include ongoing gastrointestinal issues such as GERD and constipation, as well as possible enlarged tonsils or adenoids that may be impacting feeding and sleep. Nutritionally, Arielle demonstrates significantly limited food variety and volume, indicating a restricted food inventory that may place her at risk for nutritional insufficiency. Functionally, emerging oral motor challenges may be affecting her ability to safely and efficiently manage age-appropriate textures. Psychosocially, learned mealtime behaviors, food-related anxiety, and avoidance appear to be contributing to her feeding difficulties. No overt signs of aspiration or penetration were observed during the evaluation, and there is no caregiver-reported history of aspiration pneumonia or recurrent unexplained fevers. To supplement clinical findings, Mrs. Villanueva completed the Pediatric Eating Assessment Tool (PediEAT), a caregiver-reported measure of feeding behaviors. Her responses yielded scores approaching the “High Concern” range in the areas of physiological symptoms and problematic mealtime behaviors. A rating scale of no concern on selective or restrictive eating and oral processing skills. These results align with Arielle's clinical presentation and observed feeding difficulties. CURRENT FEEDING: Arielle currently eats with assistance from her mother, with meals typically occurring at the table on a consistent feeding schedule. She averages three meals per day, with eating sessions lasting approximately 10 to 20 minutes, depending on food acceptance. Her mother express ongoing concern about persistent picky eating and a highly limited food repertoire. Currently, Arielle consistently accepts approximately 11 solid foods and 7 liquids, with evidence of food jagging for at least 4 previously accepted items (e.g., bread, donuts, lemon, strawberry). Accepted liquids include juice, lemonade, lactose-free milk, tea, milkshakes, water, drinkable yogurts, and caloric supplements. Her current intake includes a wide range of carbohydrates (e.g., crackers, chips, tostadas, cookies, dry cereals, cream of wheat), limited proteins (e.g., yogurt, peanuts, cashews, breakfast shakes), and a narrow range of fruits (e.g., bananas, mango). Vegetables are not currently accepted in any form. According to USDA dietary guidelines, toddlers ages 1 to 2 typically require 900–1,000 calories per day from a balanced diet that includes a variety of 35–40 foods across all food groups, including fruits, vegetables, whole grains, and protein sources. Arielle’s restricted intake of protein and absence of vegetables places her at risk for nutritional imbalance and insufficient exposure to essential vitamins, minerals, and macronutrients necessary for growth and development. FEEDING OBSERVATION: During the feeding observation, Arielle was seated in a supportive chair and maintained a stable 90-90-90 posture (hips, knees, and ankles) throughout the session. A variety of foods were presented, including both preferred and non-preferred items. Arielle readily accepted and consumed approximately 70% of the preferred foods offered with minimal prompting. When presented with a non-preferred food (strawberry), Arielle demonstrated early-stage sensory exploration by touching the item with her fingertips six times. However, when the clinician donned gloves to support further interaction with the food, Arielle began to cry, indicating distress. Due to the severity of her food aversions and potential negative associations with gloves or increased proximity to non-preferred foods, further presentation of aversive items was withheld to avoid reinforcing fear or discomfort. Establishing rapport and a sense of safety with the treating clinician was prioritized, as this is a critical foundation for effective feeding intervention. Parent education was provided regarding the Steps to Eating hierarchy to help the family understand Arielle’s current sensory tolerance. According to the caregiver, Arielle typically touches non-preferred foods with her fingertips before throwing them on the floor. These behaviors reflect strong texture-based aversions and align with functioning at the “touch with finger pads” level of the hierarchy, indicating limited sensory engagement beyond initial tactile contact. Arielle’s behaviors during the session were reported to be consistent with her typical mealtime patterns at home. ORAL MOTOR SKILLS: During intake of preferred foods, Arielle demonstrated several oral motor deficits, including limited tongue lateralization, a vertical (munching) chewing pattern, intermittent drooling, and reduced mastication time. By approximately 18 to 24 months of age, children are typically expected to exhibit controlled biting, effective rotary chewing, lateral tongue movement, and the ability to manage a variety of textures with appropriate bolus formation. Due to Arielle’s restricted food repertoire and refusal of most table textures, a comprehensive assessment of her oral motor skills across food types could not be completed. Continued monitoring is recommended as additional textures are gradually introduced during therapy. Overall, Arielle presents with decreased oral motor coordination and efficiency, which may be contributing to concerns regarding the safety and effectiveness of her oral intake. RECOMMENDATIONS: Feeding therapy is strongly recommended to address Arielle’s oral motor deficits, sensory-based food aversions, and limited dietary variety. Her current oral processing challenges, including reduced tongue lateralization, vertical chewing pattern, drooling, and decreased mastication time—impact the safety and efficiency of her oral intake. Therapy should begin with rapport-building and emphasize increasing tolerance for non-preferred foods using the Steps to Eating hierarchy, food chaining, and gradual sensory desensitization. Oral motor intervention will target improved chewing efficiency, tongue mobility, and bolus control. Parent coaching will be essential to support consistent strategies across settings, and a Home Management Intervention Program (HMIP) will be provided to facilitate carryover. Medically, concerns include ongoing gastrointestinal issues such as GERD and constipation, as well as possible enlarged tonsils or adenoids that may be impacting feeding and sleep. Nutritionally, Arielle demonstrates significantly limited food variety and volume, indicating a restricted food inventory that may place her at risk for nutritional insufficiency. Functionally, emerging oral motor challenges may be affecting her ability to safely and efficiently manage age-appropriate textures. Psychosocially, learned mealtime behaviors, food-related anxiety, and avoidance appear to be contributing to her feeding difficulties. No overt signs of aspiration or penetration were observed during the evaluation, and there is no caregiver-reported history of aspiration pneumonia or recurrent unexplained fevers. To supplement clinical findings, Mrs. Villanueva completed the Pediatric Eating Assessment Tool (PediEAT), a caregiver-reported measure of feeding behaviors. Her responses yielded scores approaching the “High Concern” range in the areas of physiological symptoms and problematic mealtime behaviors. A rating scale of no concern on selective or restrictive eating and oral processing skills. These results align with Arielle's clinical presentation and observed feeding difficulties. CURRENT FEEDING: Arielle currently eats with assistance from her mother, with meals typically occurring at the table on a consistent feeding schedule. She averages three meals per day, with eating sessions lasting approximately 10 to 20 minutes, depending on food acceptance. Her mother express ongoing concern about persistent picky eating and a highly limited food repertoire. Currently, Arielle consistently accepts approximately 11 solid foods and 7 liquids, with evidence of food jagging for at least 4 previously accepted items (e.g., bread, donuts, lemon, strawberry). Accepted liquids include juice, lemonade, lactose-free milk, tea, milkshakes, water, drinkable yogurts, and caloric supplements. Her current intake includes a wide range of carbohydrates (e.g., crackers, chips, tostadas, cookies, dry cereals, cream of wheat), limited proteins (e.g., yogurt, peanuts, cashews, breakfast shakes), and a narrow range of fruits (e.g., bananas, mango). Vegetables are not currently accepted in any form. According to USDA dietary guidelines, toddlers ages 1 to 2 typically require 900–1,000 calories per day from a balanced diet that includes a variety of 35–40 foods across all food groups, including fruits, vegetables, whole grains, and protein sources. Arielle’s restricted intake of protein and absence of vegetables places her at risk for nutritional imbalance and insufficient exposure to essential vitamins, minerals, and macronutrients necessary for growth and development. FEEDING OBSERVATION: During the feeding observation, Arielle was seated in a supportive chair and maintained a stable 90-90-90 posture (hips, knees, and ankles) throughout the session. A variety of foods were presented, including both preferred and non-preferred items. Arielle readily accepted and consumed approximately 70% of the preferred foods offered with minimal prompting. When presented with a non-preferred food (strawberry), Arielle demonstrated early-stage sensory exploration by touching the item with her fingertips six times. However, when the clinician donned gloves to support further interaction with the food, Arielle began to cry, indicating distress. Due to the severity of her food aversions and potential negative associations with gloves or increased proximity to non-preferred foods, further presentation of aversive items was withheld to avoid reinforcing fear or discomfort. Establishing rapport and a sense of safety with the treating clinician was prioritized, as this is a critical foundation for effective feeding intervention. Parent education was provided regarding the Steps to Eating hierarchy to help the family understand Arielle’s current sensory tolerance. According to the caregiver, Arielle typically touches non-preferred foods with her fingertips before throwing them on the floor. These behaviors reflect strong texture-based aversions and align with functioning at the “touch with finger pads” level of the hierarchy, indicating limited sensory engagement beyond initial tactile contact. Arielle’s behaviors during the session were reported to be consistent with her typical mealtime patterns at home. ORAL MOTOR SKILLS: During intake of preferred foods, Arielle demonstrated several oral motor deficits, including limited tongue lateralization, a vertical (munching) chewing pattern, intermittent drooling, and reduced mastication time. By approximately 18 to 24 months of age, children are typically expected to exhibit controlled biting, effective rotary chewing, lateral tongue movement, and the ability to manage a variety of textures with appropriate bolus formation. Due to Arielle’s restricted food repertoire and refusal of most table textures, a comprehensive assessment of her oral motor skills across food types could not be completed. Continued monitoring is recommended as additional textures are gradually introduced during therapy. Overall, Arielle presents with decreased oral motor coordination and efficiency, which may be contributing to concerns regarding the safety and effectiveness of her oral intake. RECOMMENDATIONS: Feeding therapy is strongly recommended to address Arielle’s oral motor deficits, sensory-based food aversions, and limited dietary variety. Her current oral processing challenges, including reduced tongue lateralization, vertical chewing pattern, drooling, and decreased mastication time—impact the safety and efficiency of her oral intake. Therapy should begin with rapport-building and emphasize increasing tolerance for non-preferred foods using the Steps to Eating hierarchy, food chaining, and gradual sensory desensitization. Oral motor intervention will target improved chewing efficiency, tongue mobility, and bolus control. Parent coaching will be essential to support consistent strategies across settings, and a Home Management Intervention Program (HMIP) will be provided to facilitate carryover. Feeding Assessment Summary: Arielle is a 1-year, 11-month-old female who was initially referred for a feeding evaluation due to persistent refusal of table foods, limited food variety, and sensory-based feeding difficulties. She began skilled feeding therapy in August 2025 and is now due for a re-evaluation to determine continued medical necessity and appropriateness of feeding therapy services. A comprehensive reassessment was completed using a bedside feeding evaluation, caregiver interview, and skilled mealtime observation to evaluate Arielle’s current feeding and swallowing skills and progress since initiation of therapy. Based on the re-evaluation findings, Arielle continues to present with a CHRONIC PEDIATRIC FEEDING DISORDER (PFD) with ongoing oral phase dysphagia. Although Arielle has demonstrated meaningful and measurable progress during this authorization period, her feeding skills remain below age-expected levels and continue to be impacted by medical, feeding skill, nutritional, and psychosocial factors, consistent with the ASPEN Pediatric Feeding Disorder framework. At this time, there are no concerns regarding the pharyngeal or esophageal phases of swallowing, as Arielle does not demonstrate clinical signs or symptoms of aspiration or penetration across liquids or solids, and a prior swallow study completed on 05/06/2025 was within functional limits. During the re-evaluation, Arielle demonstrated heightened emotional responses, including crying and vocalizing for her mother, which intermittently impacted her ability to participate in structured feeding tasks. Sensory regulation strategies were implemented throughout the session. Auditory sensitivities were noted, with caregiver reporting that certain sounds are distressing for Arielle. Per parent report, white noise has been effective for regulation, and auditory exposure strategies are currently being addressed in occupational therapy. Incorporation of auditory supports during feeding therapy is planned to further support regulation and engagement. Non-preferred foods presented during this authorization period included strawberry, cinnamon apple rice cake, chicken nugget, Fruity Loops, pretzels, and dried mango. With maximum verbal and visual modeling, Arielle demonstrated engagement at higher levels of the Steps to Eating Hierarchy, including tasting and consuming small amounts of non-preferred foods such as strawberry and cinnamon apple rice cake. During reassessment, Arielle was presented with yogurt, where spoon stripping was observed, indicating emerging oral-motor control. When presented with ground beef, Arielle pointed, displayed a disgusted facial expression, and nearly cried. Play-based interventions were implemented, including container play (“open–shut”) and songs, which improved participation. Arielle tolerated spoon contact with the food following clinician modeling but demonstrated distress when the spoon initially touched the meat. With corn, Arielle displayed avoidance behaviors and requested her mother. Emerging diagonal chewing was observed during reassessment, indicating continued progress in oral motor development; however, consistent rotary chewing is not yet present. Arielle also demonstrated increased dysregulation when foods were presented in multiple small pieces, suggesting ongoing challenges with visual complexity and sensory load. During this authorization period, Arielle met all five short-term goals, demonstrating significant gains in oral motor coordination, sensory tolerance, and mealtime participation. Specifically, she achieved goals targeting tongue lateralization, diagonal chewing, appropriate food disposal behaviors, palm contact with non-preferred foods, and upper distal extremity contact, reflecting progress along the Steps to Eating Hierarchy and improved regulation during structured feeding tasks. Despite meeting goal criteria, Arielle continues to demonstrate significant sensory defensiveness, particularly with tactile input involving wet or mixed textures and contact beyond finger pads and palm. While she has shown improved tolerance with play foods and structured sensory play, she frequently attempts to immediately wipe her hands following contact with real foods, indicating persistent sensory processing challenges. Upper distal extremity contact remains highly dysregulating, often resulting in crying or withdrawal behaviors, suggesting that this skill is not yet generalized and continues to require skilled support. Per caregiver report, Arielle has demonstrated carryover at home, including adding rice cakes to her accepted food repertoire. She has also attempted to lick dried mango and has chewed and swallowed strawberry and freeze-dried strawberry during therapy; however, these skills have not yet generalized consistently at home, with the exception of rice cakes. Caregiver reports that play-based feeding strategies continue to be beneficial and that Arielle’s independence and participation have improved, particularly since initiating occupational therapy services. Although Arielle has made meaningful progress, she continues to demonstrate significant sensory-based feeding challenges, limited generalization of skills, and immature oral motor patterns that interfere with safe, efficient, and age-appropriate feeding. Without continued skilled feeding therapy, Arielle remains at risk for restricted dietary variety, continued sensory aversions, and delayed progression through the Steps to Eating Hierarchy, which may negatively impact long-term feeding development. Continued feeding therapy is medically necessary to further advance Arielle’s sensory tolerance, support generalization of feeding skills across environments, continue development of oral motor coordination, and provide caregiver education to support carryover. Intervention will remain child-led, sensory-based, and regulation-focused, with close collaboration with occupational therapy to support Arielle’s sensory and auditory regulation needs.

Occupational Progress to Date: Formal assessment was completed using the Peabody Developmental Motor Scales, Second Edition (PDMS-2), a standardized tool used to evaluate motor abilities. On the Grasping subtest, the patient, with a chronological age of 27 months, achieved a standard score of 5, placing their performance in the 5th percentile. This score is more than -1.5 standard deviations below the mean and reflects an age equivalency of 12 months, indicating a significant 15-month delay. Occupational Treatment Plan: Arielle is a 2-year, 3-month-old female who was evaluated for skilled Occupational Therapy services to address deficits impacting functional performance. Based on standardized assessment, clinical observation, and parental report, the patient demonstrates significant delays in fine motor precision, visual-motor integration, manual dexterity, and sensory processing that adversely affect her ability to participate in age-appropriate activities of daily living, including play and self-care tasks. Formal assessment was completed using the Peabody Developmental Motor Scales, Second Edition (PDMS-2), which objectively quantifies these delays. On the Grasping subtest, Arielle achieved a standard score of 5 (5th percentile), representing performance more than -1.5 standard deviations below the mean and an age equivalency of 12 months, a significant 15-month delay. On the Visual-Motor Integration subtest, she obtained a standard score of 6 (9th percentile), which is more than -1.0 standard deviations below the mean and corresponds to an age equivalency of 21 months, indicating a 6-month delay. The patient's rehabilitation potential is considered good with consistent intervention and family support. However, underlying deficits in sensory processing and self-regulation present as barriers to progress, as they can impact attention, task tolerance, and the ability to engage in challenging novel motor tasks. Skilled intervention will directly address these sensory needs to improve regulation, thereby facilitating greater participation and maximizing the therapeutic benefit of each session. Consistency in attendance and caregiver follow-through with home programming will be critical to overcoming these barriers and realizing the patient's good rehabilitative potential. Given the magnitude of the patient's quantified delays, a frequency of two 45- to 60-minute sessions per week is medically necessary to provide the intensity of intervention required to remediate deficits and facilitate new skill acquisition. This frequency allows for sufficient repetition, grading of activities, and consistent implementation of sensory strategies needed to promote motor learning and establish functional habits. A home program alone is insufficient to address delays of this significance. Therefore, based on the objective deficits identified, continued skilled Occupational Therapy services are medically necessary and recommended to close the gap between the patient's current abilities and age-appropriate expectations, prevent regression, and support the achievement of her long-term functional goals.

Assessment of Goals/Response to Treatment: Patient presented as alert and cooperative, actively participating in a skilled occupational therapy session. Intervention focused on improving sensory processing, self-care, and fine motor skills through a structured sensorimotor play activity. To address tactile defensiveness, the patient engaged with a mixture of chocolate pudding and crumbled cookies. She demonstrated progress toward her sensory goal by tolerating this novel, wet, and grainy texture with her hands for approximately 20 seconds with therapist modeling before showing signs of aversion, contributing to 58% progress toward the 30-second goal. This activity was graded to incorporate utensil management, targeting self-feeding skills. With minimal hand-over-hand assistance to facilitate a palmar grasp on the spoon and initiate scooping, the patient brought the spoon to her mouth with moderate spillage on 2 out of 5 attempts. This shows emerging progress from her baseline of finger feeding. To address fine motor and visual motor integration, the session transitioned to a block activity. The patient utilized an appropriate grasp and, with minimal physical cues for shoulder stability and controlled release, successfully stacked 2 one-inch blocks on 2 of 4 trials, an improvement from her baseline of not yet stacking. Throughout the session, the patient’s tolerance to ambient noise was monitored. When an unexpected loud sound occurred, she startled but recovered within 15 seconds with moderate assistance in the form of co-regulation and deep pressure, demonstrating initial progress toward her goal of tolerating everyday sounds.

11-19-2025	Berenice Escobedo	I was able to speak to Arielle's mom in person and explain everything that Daniella had told her regarding services and Arielle no longer qualifying. Mom had misunderstood that Daniella had submitted evaluation and that insurance had denied it because she had submitted it too early. When mother spoke to the insurance they had mentioned that it would get denied because evaluation needed to be submitted when there are two visits left. I explained to mother that evaluation was not submitted to the insurance, only to the front office. I reviewed what Daniella had already explained, she scored high and that there is no medical reason to justify that she still needs speech services. I explained that although I understand her concern (Arielle not verbally expressing herself ) that everything she is doing (gestures, pointing, vocalizations, sign language) is all a form of expressive language. I explained how it is all based on her age and how in the future 3-6 months she can try again if she feels Arielle is not progressing at home or communicating wants and needs. I will be making up her visits as her authorization ends during the holidays, as well providing mother with more activities and materials she can continue to use at home after services end. I assured mother we are here to help her if she has anymore questions or concerns. Mother seemed content with the explanation.
11-18-2025	Daniella Anchondo	Parent is adamant about patient receiving speech therapy services. Assessment results indicate that the child does not meet eligibility criteria for medically necessary speech therapy at this time. The SLP explained insurance qualification requirements, and discussed why services cannot be ethically justified based on current scores. Parent was given option to do private pay or wait at least 3 months to administer another evaluation. Refer to messages below: Daniella Anchondo 11/18/2025 2:53 PM Hello, this is Daniella. I spoke to the front desk regarding new insurance in January and whether we needed a new referral. As I stated over the phone, it depends on the insurance whether they would approve more services. Given her age and scores, it is unlikely that they would. What I strongly suggest is waiting about 3 months (around March or April) to schedule another evaluation and hope her scores go down a little bit based on her increase in age. We would need a new referral to complete that. Let me know if you have further questions. I explained that she was not going to qualify based on her scores, and the report was completed and submitted to the front office, not to the insurance. The insurance will not grant services because Arielle scored well above the mean for her age, so we cannot ethically justify continued therapy through insurance. However, if you want continued therapy we can go with the suggested option above in hopes that scores come down to a mild diagnosis where we can justify therapy, or private pay. To clarify, when we assess language, we look at both receptive and expressive skills. Receptive language refers to how well a child understands language, such as following directions, understanding vocabulary, and comprehending questions or stories. Expressive language refers to how a child communicates, which includes not only spoken words but also gestures, pointing, facial expressions, signs, pictures, combining words, and forming sentences. I’m not sure what the insurance could have meant by “anything being closed.”

I had a tumor that was attached to the outside of my placenta, she would hiccup everyday in my stomach after I would eat, and no complications at birth. 3. She does see a GI for GERD. 3. She was hospitalized at 2 months for a possible emergency liver transplant but she got better. 4. She has always been delayed in everything because of her constantly being ill. We were at doctor appointments since she was a week old for different reasons. She started seeing a GI at one month when her primary care physician couldn't find a solution to her stomach problems. Neither the PCP or GI were concerned with her being delayed they said it was normal because of everything she's been through. 5. Allergic to amoxicillin 6. She has breathing issues when she born until about 6 months. She also had chocking problems. For both of those reasons she had to sleep with a pillow underneath because if she was laying flat she would start choking and stop breathing. She no longer has breathing problems but does tend to chock on her snacks at times. She gets 9-12 hours of sleep. She wakes up briefly during the night crying or just moving around. She started snoring and sleeping with her mouth open since she got her upper endoscopy in February, 2025. 7. Her medications are polyethylene powder and famotidine

Her GI diagnosed her with GERD and she only drinks Lactose free milk 10. My concerns are that she does not eat table food what so ever. I've constantly put different types of food in small portions such as chicken, broccoli, oranges, strawberries, a bean, cheese strand, egg, pancake, French toast, she refuses to eat any. The only thing that she will eat is banana, her gerber snacks, applesauce, gerber fruit pouches, crackers, cereal (certain ones) , yogurt, malt o meal, half a bagel once in a blue moon, and her ensures.

I can tell you’ve been so proactive and supportive through everything she’s been through, and it makes total sense why you’re concerned. Based on what you've shared, it sounds like she may be experiencing both sensory and oral-motor challenges when it comes to eating, especially with her limited food range and history of choking. These are exactly the types of concerns we can assess in a feeding evaluation. During our session, I’ll look at how she responds to different textures, how she uses her mouth to explore or chew food, and what her body language tells us. I’ll also give you strategies based on what we see, and if needed, we’ll develop a plan to gently expand her diet while keeping meals as positive as possible. For the evaluation, please bring a variety of textures so we can see how she reacts (apple sauce, strawberry, chicken) along with her preferred 3 foods of your choosing.

These foods will be the same foods for a one-month span. To help Arielle's sensory system, we will stick to a color scheme (any foods that are light in color so white or yellow). Here are some ideas: protein: yogurt, ham, chicken nuggets, turkey deli, cheese sticks, cheese, eggs. starch: rice crackers, rice cakes, veggie strips, pasta, noodles, bread, english muffin, veggies: yellow squash, corn, cucumber. fruits: yellow apples, pears, pineapple, dried mango/apple/pear. Let me know if you have any questions (: However, I have noticed that during the first sessions kiddos do take some time to adjust and tend not to eat during therapy so if you think that will be the case with Arielle feel free to pick 2 non-preferred foods and 2 preferred foods (:

As I stated over the phone, it depends on the insurance whether they would approve more services. Given her age and scores, it is unlikely that they would. What I strongly suggest is waiting about 3 months (around March or April) to schedule another evaluation and hope her scores go down a little bit based on her increase in age. We would need a new referral to complete that. Let me know if you have further questions. Thank you

I explained that she was not going to qualify based on her scores, and the report was completed and submitted to the front office, not to the insurance. The insurance will not grant services because Arielle scored well above the mean for her age, so we cannot ethically justify continued therapy through insurance. However, if you want continued therapy we can go with the suggested option above in hopes that scores come down to a mild diagnosis where we can justify therapy, or private pay. To clarify, when we assess language, we look at both receptive and expressive skills. Receptive language refers to how well a child understands language, such as following directions, understanding vocabulary, and comprehending questions or stories. Expressive language refers to how a child communicates, which includes not only spoken words but also gestures, pointing, facial expressions, signs, pictures, combining words, and forming sentences. I’m not sure what the insurance could have meant by “anything being closed.”
