Assessment of Goals/Response to Treatment: Ethan presented to the session alert, oriented, and c a cooperative and eager demeanor, actively participating in all therapeutic activities. Skilled OT services were provided to address ADLs, motor planning, cognitive integration, VMI, and psychosocial skills through structured and play-based interventions. To address self-care, Ethan participated in a shoe-tying task using a 3-D model. He demo'ed the ability to initiate the sequence (crossing laces and forming the first loop) req. only min verbal cues. However, he req. max verbal cues and mod physical A for completion of the final knot-tying steps in 6/7 trials, indicating continued difficulty c higher-level motor planning and sequencing. He successfully completed the full sequence 1x req. mod verbal cues and physical A, demo'ing emerging skill acquisition. Significant progress was noted in UBD, as Ethan I donned and doffed a front-opening shirt and successfully manipulated all 5 buttons req. only min cues for sequencing in 1/1 opportunity, indicating improved B integration, FM coordination, and I c ADLs. To further address underlying skills, Ethan completed a 24-piece puzzle req. min cues for visual scanning and problem-solving, targeting VMI and sustained attention. Additionally, he participated in a therapeutic game (“Don’t Break the Ice”), demo'ing appropriate turn-taking, social reciprocity, eye-hand coordination, and problem-solving skills throughout the activity. Overall, Ethan demos measurable progress in ADL performance, particularly UBD, c continued need for intervention targeting motor planning and sequencing for shoe tying. He continues to benefit from skilled OT services to improve I c complex self-care tasks and further develop cognitive and motor skills.

Birth weight was 5 lbs 2 oz and current weight is approximately 40 lbs. Patient passed newborn audiological screen at hospital. Mother has always had concerns with child’s overall development. Mother was always advised, patient is fine, it’s because he was premature. Mother was advised at one point that child had muscular dystrophy however follow tests were negative for muscular dystrophy. Mother states he is weak, falls a lot, does not move like he should and presents with fear of heights/ went hiking and he was grabbing parents and not wanting to go further up the mountain. Mother was referred to orthopedic doctor at age 2 - X-rays taken, bones look fine, and was advised patient not a good candidate for shoe inserts for pronation due to not complaining of pain. Mother would like to seek neurologist to see if it is related to hemiplegia that the school pointed out to parent at ARD. Mother has not been able to get into physical therapy prior to this due to patient person information was hacked /identity theft and mother has been struggling to get it resolved for the past 2 years; until now where she was able to find a facility to get child services. Patient referred to physical therapy due to atypical gait/mobility. Patient presents with weakness, decreased mobility, decreased balance, lacks coordination, atypical gait, decreased activity tolerance, and decreased kinesthetic sense which impairs patient’s ability to meet developmental milestones with activities of daily living, routines and for overall quality of life. The patient is a 6 year 7 month old male who scored at<4 year age equivalence on BOT-2 in Balance, Strength, and Agility in Gross Motor area indicating a Well Below Average status with simple and complex skills; thus severely impacting development. Without skilled physical therapy, the patient is at risk for developing further impairments that may negatively impact the ability to perform both simple and complex daily tasks, sustain proper posture, fulfill ADLs , and limit overall quality of life. Skilled physical therapy services are required to promote optimal level of developmental function, minimize risk for permanent disability, improve overall strength throughout, improve flexibility/AROM, promote functional mobility to habilitate optimal and overall functional development to keep up with peers. Patient would greatly benefit from skilled PT interventions in order to address established delays and facilitate functional independence and performance. Home programming alone is not sufficient to make the needed progress, direct one to one services are required for progress to be achieved. Results of the evaluation were reported and explained to parent(s), along w/ the diagnosis, prognosis, including the frequency, time and duration of treatment and contents of the plan of care. Family will be provided w/ a verbal report of progress noted along w/ any homework for the family as part of a home program (if applicable) at the end of every session. 02-01-2026	Daniel Valenzuela	On 1/29 during a make up appointment, Ethan was participating in static balance holds on a bosu balance trainer. While performing his second to last rep and after a long rest period, patient suffered a LOB episode which resulted with him attempting to step off of bosu ball. Patient's R ankle was seen with an episode of lateral instability, which resulted in a controlled fall onto the mat. Afterwards, the patient complained of R ankle pain and demonstrated signs and symptoms consistent with ankle sprain/strain due to patient reported mild to moderate increase to pain with at end range with inversion along with weight bearing on the affected extremity and was palpated with increase swelling along lateral dorsum of R foot. Patient was returned to mother who was immediately informed of incident and reported understanding. Mother was educated on RICE technique and was asked to monitor patient for the next few days and informed to follow up with doctor if pain did not alleviate or if it worsened. Mother verbalized understanding.

However she did help me put in a report as the provider in efforts to try and remove the coverage from superiors records. She did let me know that this process can take up to 30 days to remove from their portal. Here is the reference number I got from Superior I-174628778, please let me know if you and have questions or concerns. Thank you!

Re-evaluations are completed once per month for the following month. Your current authorization expires 1/16/26. It’s important to complete your re-evaluation within the current authorization period. I know we had spoken that he would need a schedule change however currently I am unable to accommodate the previously discussed schedule. I wanted to see if this week we would be able to come in Wednesday at 3:30pm? If his previous time slots do not works, we would have to place Ethan on a wait list and he wouldn’t receive services until further after school availability opens up, which might be a long time.

At this time, I show that Ethan has missed 27 scheduled visits. I do have 8 AMs open on Wednesday if you are willing to bring him in that extra day. This is all to work on bring his attendance up above 90%, which is company policy, as he is currently at 22%. I also understand he was on hold for a while so that is also affecting his attendance but, I just wanted to make sure you were aware of where he is at right now so we can work on getting some make-ups in. The address is 1510 N Zaragoza Rd, Suite B1, El Paso, TX 79936. It is very close to the current building, and I’m happy to send a photo showing its location in relation to where we are now if that would be helpful, just let me know! I’ve already checked out the space, and it has a lot of great materials and activities that I’ll be able to use during our sessions. My first official day at the new location will be February 2nd, so please still come to the building you have been until the end of the week.
