Occupational Background History: Elowyn is a 3.2 year old girl who was seen on this date for a scheduled occupational therapy evaluation in order to obtain current level of function. Mother Caitlyn, was present in order to serve as the primary informant throughout the evaluation. Per mother's report child was born at 37 weeks. Child did not experience any complications at birth, does not have any visual or hearing deficits. Per parental report child struggles with sensory integration with hypersensitivity to noises, unfamiliar persons and places, textures, visuals and social interactions. During evaluation child demonstrates ability to follow simple directions and engage in tasks although does require redirection to task, and assistance to transition to new tasks.

Occupational Diagnosis/Impressions: Elowyn demonstrates delays hindering executive functioning skills, functional behavior and safety. Child also demonstrates difficulty with manual control skills, object manipulation, manual dexterity hindering fine motor precision and accuracy. Child will stop task due to difficulty processing what was asked limiting engagement. Child also demonstrates fine motor and grasp delays in BUE evident by premature grasp patterns, spillage and difficulty with manual dexterity tasks. Elowyn also demonstrates deficits in sensory processing and self-regulation, which have impaired their ability to complete activities of daily living and instrumental activities of daily living that compromise their routines and roles at home and in the community. Assessment of Goals/Response to Treatment: The patient presented as alert and cooperative, demonstrating readiness for engagement in today's skilled occupational therapy session. Intervention focused on enhancing motor, psychosocial, and cognitive skills through structured cooperative play activities. Progress was noted in visual motor integration and fine motor control, as the patient demonstrated the ability to imitate and copy 4/4 basic pre-writing shapes with approximately 50% accuracy while maintaining a functional grasp. The patient exhibited significant gains in self-regulation and coping skills, requiring only one verbal prompt to successfully transition between activities without any adverse behavioral response. Furthermore, she effectively utilized functional communication strategies to request breaks as needed, which supported her ability to sustain attention and complete all structured therapeutic activities. During cooperative play, the patient tolerated social interaction with moderate therapist assistance to facilitate reciprocity, demonstrating improved social participation without distress.

Due to a recent change in Tricare’s process, all patients are now required to be seen by their Primary Care Provider (PCP/PCM) and have an updated referral submitted to Tricare before we can request authorization for therapy. Please note: -The referral end date provided by your PCP/PCM will determine the authorization period for therapy services. -If possible, we recommend requesting a referral period of up to one year, as this will help reduce the number of required follow-up visits. -If you are unsure whether a current referral is on file, please contact your PCP/PCM or Tricare Referral Management directly. This change began earlier this month. While we are actively reviewing all existing referrals, we kindly ask for your assistance in scheduling an appointment with your PCP/PCM and ensuring an updated referral is submitted to help avoid any delays in therapy services. If you are able to view your referral end date, please be aware that your child will need to be seen again before that date to continue services.
