Based on the caregiver report and clinical observation, English was determined to be the appropriate language for intervention.

His feeding difficulties appear to be multifactorial, influenced by possible medical factors (suspected autism, oral motor skill challenges), nutritional concerns (limited dietary variety and underweight status), sensory-based feeding deficits (strong aversions to food textures), and psychosocial components (learned mealtime behaviors and food-related avoidance). No overt clinical signs of aspiration or penetration were observed during the evaluation, and there is no caregiver-reported history of aspiration pneumonia or recurrent, unexplained fevers. Ronnie’s mother reported possible gastrointestinal concerns, including constipation, with bowel movements occurring approximately every 4–5 days. A referral to a gastroenterologist is recommended to further evaluate potential GI involvement that may be contributing to his feeding difficulties. To supplement clinical findings, Mrs. Gentry completed the Pediatric Eating Assessment Tool (PediEAT), a caregiver-reported measure of feeding behaviors. Her responses indicated scores approaching the "High Concern" range in the area of selective/restrictive eating. Ronnie scored within the "No Concern" range for physiological symptoms, problematic mealtime behaviors, and oral processing skills. These results are consistent with Ronnie’s clinical presentation and observed feeding challenges. CURRENT FEEDING: Ronnie currently eats with assistance from his mother, as he is not yet able to manipulate utensils. However, he is able to self-feed using his hands. Meals are typically provided in his highchair with a consistent feeding schedule. He averages two meals per day (around 1:00 p.m. and 6:00/7:00 p.m.) and one snack, also around 3:30 p.m., with each eating session lasting approximately 15–20 minutes. Ronnie’s appetite is variable, and he shows a strong preference for crunchy, crispy, dry, and uniformly textured foods. Ronnie currently accepts approximately 16 solid foods and 2 liquids. He has food jagged (i.e., eliminated) an additional 11 previously accepted foods. According to USDA dietary guidelines, children between 2 and 2.5 years of age typically require approximately 1,000–1,200 calories per day, depending on activity level, and should consume a varied diet consisting of at least 35–40 nutrient-dense foods across all food groups. Although Ronnie’s exact caloric intake is unknown, caregiver reports and clinical observation suggest his current diet is nutritionally inadequate, particularly low in protein diversity, fruits, and vegetables, all of which are essential for supporting growth, immune function, and overall development. Discharge will be considered once he is consistently meeting established therapy goals and accepting at least 35 or more foods across multiple food groups to support adequate nutritional intake for growth and development. Caregivers will play a critical role in implementing and maintaining the Home Management Intervention Program (HMIP) to promote generalization of feeding strategies into the home environment and to support long-term carryover and success.

Feeding Assessment Summary: Ronnie is a 2-year, 8-month-old male who was initially referred for feeding therapy due to significant food aversion, limited nutritional intake, and weight concerns, including a diagnosis of failure to thrive. He began skilled feeding therapy during this authorization period and is now due for a re-evaluation to determine continued medical necessity and appropriateness of feeding therapy services. A comprehensive reassessment was completed using a bedside feeding evaluation, caregiver interview, and skilled mealtime observation to assess Ronnie’s current feeding and swallowing skills and response to intervention. Based on re-evaluation findings, Ronnie continues to present with a CHRONIC PEDIATRIC FEEDING DISORDER (PFD) with ongoing oral phase dysphagia. Although Ronnie demonstrated periods of improved regulation and feeding participation earlier in the authorization period, his feeding skills remain below developmental expectations and have been further impacted by recent medical events, gastrointestinal symptoms, and fear-based feeding regression. At this time, there are no concerns for pharyngeal or esophageal dysphagia, as Ronnie did not demonstrate overt clinical signs or symptoms of aspiration or penetration during observed intake. During this authorization period, Ronnie’s mother reported intermittent gastrointestinal symptoms, including episodes of vomiting and diarrhea, with vomiting described as clear and occurring upon waking, without signs of acute illness. Following these GI episodes, Ronnie consistently demonstrates decreased interest in food and reduced intake, often leaving meals early or refusing foods altogether. Caregiver reports that Ronnie is currently consuming Greek yogurt exclusively, which has been associated with improved GI tolerance, stable bowel movements, and fewer gastrointestinal symptoms. Earlier in the authorization period, Ronnie demonstrated improved tolerance to food exploration, increased regulation during meals, and willingness to interact with new foods. Both the clinician and caregiver observed that Ronnie appeared more regulated and open to trying foods compared to baseline. Ronnie was able to consume and/or reintroduce a variety of foods across categories, including protein sources such as pizza (multiple types and brands), taquitos, waffles, reintroduced egg and noodles, potato salad, mashed potatoes, ham, and hot dogs; starches such as French and sesame bread, bread rolls, veggie chips, pretzels, air-fried Cheez-Its, and pumpkin pie. In mid-December, Ronnie experienced a choking incident involving a lodged cookie, which resulted in increased fear during meals and a subsequent regression in solid food intake. Since this event, Ronnie has demonstrated heightened feeding aversion and avoidance of previously accepted solid foods. Additional concerns reported include sleep disturbances, inconsistent meals, recent weight decline, and reduced nutritional variety. Caregiver and clinician also noted increased curiosity and willingness to engage with foods during this period. However, following the onset of gastrointestinal symptoms and a choking event, Ronnie experienced a regression in intake and is no longer consistently consuming or attempting to try new foods, indicating that medical factors continue to significantly impact feeding progression. Despite this regression, Ronnie continues to demonstrate improved regulation during supported feeding interactions, particularly when caregivers provide appropriate wait time, cue reading, and pacing, indicating that feeding skills remain present but are masked by fear-based and medically influenced avoidance rather than loss of ability. During the re-evaluation session, Ronnie was observed consuming Greek yogurt, which was the only food tolerated at the time of assessment. He consumed approximately 60% of the meal offered. Ronnie demonstrated effective labial closure, spoon stripping, and active labial movement when yogurt contacted his lips. A munching chewing pattern was observed during oral manipulation. Ronnie was able to create an adequate seal for straw suction, indicating functional oral motor skills for liquid intake via straw. No overt clinical signs or symptoms of aspiration or penetration were observed during intake. Ronnie appeared regulated throughout the feeding session, with caregiver demonstrating strong responsiveness to his cues by providing appropriate breaks and pacing. Caregiver reported that failure to provide breaks often results in crying, further supporting the role of regulation and caregiver strategy in Ronnie’s feeding success.Oral motor skills were assessed through informal observation during yogurt intake only, as Ronnie is not currently tolerating solid foods. He demonstrated adequate labial closure, coordinated lip movement, and a vertical (munching) chewing pattern. Tongue lateralization and rotary chewing could not be fully assessed due to limited texture exposure. At Ronnie’s age, children are expected to demonstrate progression toward diagonal and rotary chewing patterns, improved bolus manipulation, and increased tolerance of textured solids. Ronnie’s current oral motor presentation suggests immature but functional skills, with performance significantly influenced by fear, sensory factors, and medical tolerance, rather than an absence of skill. Although a referral to Gastroenterology was previously requested through the primary care provider, it was not approved, and a written recommendation to discontinue feeding therapy was provided despite ongoing feeding regression, nutritional concerns, and weight instability. Given Ronnie’s continued feeding challenges, GI symptoms, and nutritional risk, continued GI and dietitian consultation remain strongly recommended to address underlying medical contributors and support nutritional adequacy. Despite prior progress, Ronnie continues to present with severe feeding regression, restricted intake, weight instability, and nutritional risk, compounded by GI symptoms and a recent choking event. Intervention will remain child-led, sensory-based, and regulation-focused, prioritizing emotional safety, medical tolerance, and gradual progression through the Steps to Eating Hierarchy. Collaboration with Gastroenterology and a registered dietitian is strongly recommended to address underlying GI contributors and nutritional adequacy. Skilled feeding therapy remains medically necessary to address Ronnie’s chronic pediatric feeding disorder and oral phase dysphagia. (BASELINE: 0/10 : CURRENT: 1/10; GOAL NOT MET) In 6 months, Ronnie will demonstrate improved self-regulation during meals by reducing overstuffing behaviors and accepting age-appropriate bite sizes with modeling and pacing cues in 3 out of 10 opportunities across 3 consecutive sessions, to promote safe intake and support oral control. (BASELINE: 0/10 : CURRENT: 4/10; GOAL MET) In 6 months, Ronnie will throw non-preferred foods into "throw away bowl" instead of on the floor in 3 out of 10 opportunities across 3 consecutive sessions, in order to decrease problematic mealtime behaviors of throwing foods. Discharge will be considered once he is consistently meeting established therapy goals and demonstrating safe, efficient, and developmentally appropriate feeding across settings. Caregivers will play a critical role in implementing and maintaining the Home Management Intervention Program (HMIP) to ensure generalization of feeding strategies into the home environment and to support longterm success.

Trunk support is good. 5) no known allergies 6) sleeping patterns are varied. Does not nap during the day (refusal) goes to bed at different times (ex. If he goes to bed at 10pm he wakes up at 1am remains awake till 3 or 4am wakes up at 1 or 2pm and eats) He sleeps maybe sometimes with an open mouth but i see no breathing issues. 7) no medications at all 8) yes pediatrician addressed concerns of failure to thrive growth. He dropped from 22.8 to 22.3 and height has been stunted due to weight. Only head circumference is growing. She told me to see her again in 3 months. 9) no gastrointestinal problems thT i know of but i believe he gets full quick and eats little bit throughout the day mostly snacking. Has bowel movements 4-5 days if full and if i give him something like applesauce or applesauce with fiber too often he will have diarrea for 1-2 days. 10)

Foods he used to like eating such as hamburger he just picks at the bread and only eats the bread. Does not seem interested in trying other foods as much anymore. Is still not holding a spoon or form by himself i have to feed him if he eats eggs or yogurt. He only eats eggs and yogurt at around 2pm everyday because he wakes up at that time and water and only wants to snack on crunchy foods like cheez itz, gold fish, banana chips

These foods will be the same foods for a one-month span. To help Ronnie's sensory system, we will stick to a color scheme (any foods that are light in color so white or yellow). Here are some ideas: protein: yogurt, ham, chicken nuggets, turkey deli, cheese sticks, cheese, eggs. starch: rice crackers, rice cakes, veggie strips, pasta, noodles, bread, english muffin, veggies: yellow squash, corn, cucumber. fruits: yellow apples, pears, pineapple, dried mango/apple/pear. Let me know if you have any questions (: However, I have noticed that during the first sessions kiddos do take some time to adjust and tend not to eat during therapy so if you think that will be the case with Ronnie feel free to pick 2 non-preferred foods and 2 preferred foods (:
