Child struggled to initiate and/or complete 3 step directions with max assistance via visual demonstrations, and verbal prompting for motivation, task initiation and redirection to task. Child exhibits limited attention evident by fleeing/escaping non-preferred activities, and need for constant redirection.

Assessment of Goals/Response to Treatment: Therapist and Hadley explored emotions. Therapist had her label them and describe how they felt and when scenarios which could provoke them. She was able to identify with min cues. Therapist then prompted to pick a color and a symbol for the emotions discussed and then therapist guessed which represented which. Therapist did the same for Hadley. This opened up discussion as to why the color and symbol were chosen. Therapist then prompted to draw family using animals to represent each member. This was also explored by probing why animals were chosen. Min prompts provided to explain.

Since the onset of therapy, she has demonstrated meaningful progress, including improving strength, less intoeing on right LE, improving flexibility. However, she continues to present with, weakness, limited balance, left lower extremity intoeing and tightness in left hip. The patient was seen on November 21, 2025, for a scheduled physical therapy evaluation to reassess her current functional status. The patient is a 10 year old female who scored at 50/56 on Pediatric Balance Scale indicating mild impairment with balance and scored 67/80 on Lower Extremity Functional Scale indicating mild to moderate functional impairment with lower limb problem. Patient has made improvements with strength balance and foot position but continues to demonstrate mild- moderate functional impairment with left foot in toeing, some difficulty with mobility and balance. Without continued skilled physical therapy, the patient is at increased risk for developing further impairments that may restrict participation in daily activities, compromise posture, limit performance of ADLs, and negatively affect overall quality of life. Ongoing skilled physical therapy services are medically necessary to support optimal developmental progression, improve strength and flexibility/AROM, enhance functional mobility, and promote gross motor skill acquisition required to keep pace with peers.
