Speech Assessment Summary: Amaya is a 5 year, 11 month-old female who was seen to assess her speech and language for continued need of services. Amaya was accompanied by her mother who was present in the clinic room throughout the re evaluation. Standardized assessment, developmental data, observations, and therapy notes were used to determine progress and growth. It was previously reported that Amaya received a diagnosis of Autism Spectrum Disorder in October 2024 by Dr. Numsen. She is currently attending ABA therapy Monday through Friday. A child Amaya's age should use clear and grammatically correct sentences, follow multistep directions, understand basic concepts and vocabulary, tell simple stories with a clear beginning, middle, and end, and be understood by others most of the time. The Preschool Language Scales Fifth Edition was used to assess his Receptive and Expressive language skills. The Preschool Language Scales Fifth Edition (PLS?5) was used to reassess Amaya's language development and progress. Amaya obtained an Auditory Comprehension raw score of 37, with a standard score of 60, indicating a SEVERE receptive language disorder. In Expressive communication Amaya obtained a raw score of 33 and a standard score was 56, indicating a SEVERE expressive language disorder. Overall, Amaya obtained a Total Language raw score of 116 and a standard score of 55 indicating a SEVERE MIXED RECEPTIVE/EXPRESSIVE LANGUAGE DISORDER. **IT IS IMPORTANT TO NOTE THAT AMAYA'S ATTENTION WAS FLEETING THROUGHOUT THE EVALUATION, AND REQUIRED MAXIMAL VERBAL REDIRECTION, AS EVIDENCED BY LEAVING HER CHAIR, ATTEMPTING TO THROW FURNITURE TO ELICIT A REACTION FROM THE PARENT AND THERAPIST, AND REFUSING TO PARTICIPATE IN MULTIPLE TASKS IN STANDARDIZED TEST'S TASKS.** Amaya's receptive and expressive language strengths include: understanding spatial concepts (in, on, out of, off) without gestural cues, understanding quantitative concepts (one, rest, all), making inferences, understanding analogies, identifying colors, pointing to letters, naming a variety of pictured objects, combining three or four words in spontaneous speech, using a variety of nouns, verbs, modifiers and pronouns in spontaneous speech, producing one four or five word sentence. When does our current referral expire? Thank you." At 1:22 AM on 03/19/2026, parent sent an additional message stating: "Please cancel appointment." Therapist reviewed and responded at 2:40 AM on 03/19/2026 with the following: "You are still within your authorized visits unless you’ve been told otherwise by the front office." At 7:10 AM, therapist canceled session. The other two will be with co-pay; that is if you'd like to use those two approved visits from Tricare. If you have any questions please give us a call at (915) 855-0601. To stay ahead of this change and help prevent any interruptions in your child’s therapy, we would like to know how you would like to proceed. The options to proceed would be to remain with Dr. Patel and switch to a different insurance plan, or remain with Superior Health and transition to a different pediatrician. If you choose to stay with Dr. Patel and switch insurance plans, we will need a new referral for all services under the new insurance, as well as a copy of your new insurance ID card so we can verify benefits. If you choose to switch pediatricians and remain with Superior Health, we will need a new referral for all services from the new primary care provider (PCP). Please contact the front office as soon as possible so we can help coordinate this change and ensure there are no gaps in your child’s therapy services. Thank you for allowing us to serve your child!

Due to a recent change in Tricare’s process, all patients are now required to be seen by their Primary Care Provider (PCP/PCM) and have an updated referral submitted to Tricare before we can request authorization for therapy. Please note: -The referral end date provided by your PCP/PCM will determine the authorization period for therapy services. -If possible, we recommend requesting a referral period of up to one year, as this will help reduce the number of required follow-up visits. -If you are unsure whether a current referral is on file, please contact your PCP/PCM or Tricare Referral Management directly. This change began earlier this month. While we are actively reviewing all existing referrals, we kindly ask for your assistance in scheduling an appointment with your PCP/PCM and ensuring an updated referral is submitted to help avoid any delays in therapy services. If you are able to view your referral end date, please be aware that your child will need to be seen again before that date to continue services. I would call front office personally as I do not know/understand the new referral process. they will be able to better guide you in the steps needed! :) For next week, Amaya would be seen on both Thursday and Friday. If you need a different day outside of her regular Thursday appointment, I can offer a make-up session with our new hire Lauryn. She is an SLPA. Her opening and availability is much more flexible than mine as she is still building her caseload. Are you interested in that?

And they we would need to get a new referral from a new pediatrician to continue survives with you guys. Can you please confirm. If everything is good we will come in next week, however if we need to get a new referral we will wait until we get that sorted out. You have 2 options: 1) Amaya's current PCP is no longer accepting Superior so you can switch to the other PCP in-office with her current PCP and then all we need is z new referral (this new PCP would have to accept both Superior and Tricare Referred) 2) If you choose to drop Superior and switch to El Paso Health or another insurance company, then all we would need is a new authorization (approval to continue services/coverage) from your new insurer

So after that, New Beginnings will have a ~hard time billing Superior. But I recommend calling front office to double check there since I'm not familiar with authorizations and insurance. They can better help understand.
