Feeding Background History: Jordan is a 1-year 7-month-old female who was referred for a clinical feeding evaluation by her pediatrician due to concerns related to feeding difficulties. Mr. and Mrs. Blake served as the informants for the feeding evaluation. Jordan’s parents expressed concerns with feeding difficulties related to temperature preferences, overstuffing, and difficulties with taking bites from larger pieces of foods (i.e. apples). Mr. Blake, Mrs. Blake, and Jordan all follow a vegan diet. Her parents reported that she prefers cold foods or foods at room temperature, will often overstuff her mouth resulting in anterior spillage, and cannot bite pieces from a larger item (i.e. whole apple) and will often overcompensate by utilizing mandible to "rake" instead of bite. Jordan’s parents reported that Jordan is often willing to try new foods and do not currently have concerns with the number of foods she eats/accepts. Jordan’s diet currently consists of solids and breast milk (breastfeeding). Jordan’s feeding abilities were observed during a clinical feeding evaluation and structured mealtime observation in order to determine the current level of function and identify feeding difficulties.
Feeding Medical History: Jordan’s mother reported that she experienced bleeding during her 1st trimester resulting in bedrest. Jordan was born at 41 weeks via home birth. Her mother reported no complications during or after birth. Jordan’s parents reported that she underwent a labial frenectomy at StarKids dental when she was 2 weeks old due to difficulty with breastfeeding. Jordan’s mother reported immediate improvement following the frenectomy. Jordan’s mother reported that Jordan has received craniosacral therapy previously, however, is not currently receiving it at this time. Jordan’s mother reported that she has been seen by a pediatric dentist due to an “underbite” and has had her airway checked resulting in “no concerns”. Jordan is currently not taking any medications. Jordan’s parents reported no known medical or food allergies. Jordan follows a vegan diet.

Assessment of Goals/Response to Treatment: When counseled by the clinician, parents will incorporate the home management intervention program as a daily and weekly activity to transfer skills learned in therapy into the home environment. Home management training will optimize patients feeding tasks in the home. Mother was present for feeding session and engaged in therapy tasks. The following foods were brought in: mango fruit bar, yogurt, macaroni salad. In 6 months time, Jordan will demonstrate adequate tongue lateralization in 5/10 trials in order to orally prepare solids to promote a safe swallow during meals. Pt demonstrated tongue lateralization in 5/10 trials utilizing z-vibe. Pt was provided with visual/tactile cues. Lateralization was also observed while eating mango fruit bar in 4/7 trials. In 6 months time, Jordan will develop the emergence of appropriate chewing skills (munch/diagonal pattern) when presented with age-appropriate solids in 5/10 trials to promote adequate preparation of solids for safe swallowing during meals. Pt demonstrated an emerging diagonal chew pattern in 3/7 trials while eating a mango fruit bar. Pt was provided with moderate verbal/visual cues.

Mother Karyna, was present in order to serve as the primary informant throughout the evaluation. Per mother's report child was born full term at 41 weeks gestation. Child did not experience any complications at birth, does not have any visual or hearing deficits. Jordan was born with both a tongue tie and lip tie leading to difficulty with oral motor function affecting nursing, speech, eating and overall oral muscle tightness. Jordan also demonstrates sensory deficits such avoiding shoes, socks, tip toeing, avoidant of some textures, and also some sensory seeking behaviors.

My supervisor spoke to me today due to Jordans attendance and canceled therapy visits these past 3 weeks. He asked me to offer you all a 30 day hold if needed. Jordans last therapy session for OT was on 2/25/26. And unfortunately 30 days of no therapy can be an automatic discharge of services. I completely understand things happen and wouldn't want Jordan to lose her therapy and all her progress. Therefore I was informed to offer you all a 30 day hold if needed. Please let me know you would like to move forward.
