Speech Assessment Summary: Peyton is a 7 year, 8 month old male who was re-evaluated to determine continued eligibility of speech and language services. Peyton was re-evaluated using the GFTA-3 assessment to test articulation skills and the CELF-5 to test language skills. The evaluation was administered in English, as it is his dominant language. Father reported no changes in medical history. Father brought Peyton to the evaluation, however, he was not present during testing. The following were Peyton's scores in each test: GFTA-3: Total Raw Score - 10, Standard Score - 77, Percentile Rank -6 CELF-5: Core Language Scaled Scores - 26, Standard Score 80, Percentile Rank - 9 Based on standardized scores and clinical observations Peyton presents with a MODERATE articulation disorder and a MODERATE mixed expressive/receptive disorder. Peyton would benefit from speech and language therapy targeting the following areas: f/v substitutions, production of voiceless and voiced /th/ phoneme. 02-04-2026	Avery Fontenot	Mom called front office due to the non compliance message being sent out due to the cancellation of yesterdays visit. Mom was confused and thought the clinic understood she was wanting to be transferred and put on a hold on Monday. Mom then acknowledged all she asked us for is to see if Tori had availability to take the patient. Front office reiterated to mom that we were never made aware of wanting to be on hold while finding an available slot with Tori. Then explaining to mom when she asked to cancel visits with current therapist for this week, we were not told they were wanting to cancel visits these visits on Monday and the front office was only notified on Tuesday the day of the appointment when should would like to cancel those appointments. That is why she received the non-compliance message for yesterdays visits since it fell within the 24 hours of the appointment. Mom also wanted clarification on our 90% attendance policy due to the patients current therapist reaching out and notifying mom of the 8 make-up sessions that they were going to be responsible for. Mom was confused because they only had to cancel two appointments due to Peyton being sick. Front office then made mom aware that in the registration documents she did sign our attendance policy and any sessions missed she will be responsible for. Mom was confused by the number of visits so front office then clarified with mom that due to Christmas break and those make up for those missed appointments were never made that is why this is such a big discrepancy. I then explained to mom due to her switching therapist for her to get with new treating therapist on what is the best plan for Peyton and parents to fix the attendance and schedule the make up sessions. Front office then explained to mom starting next week Peyton will be 1x/week with Tori starting next Monday due to the therapist availability and parents only requesting appointment times after 5:00. Mom acknowledged while front office reiterated that she wont be responsible for the second visit for the rest of the authorization due to this reduction. Mom was made aware again of the 8 visits and to contact the therapist on how to best go about this situation.
02-03-2026	Avery Fontenot	Mom called front office requesting to switch over to Tori. Patient has previously been with Tori but due to leaving and wanting to come back Tori had their previous slot filled already. Front office explained to mom that Tori only has one available slot to see Peyton. Mom was okay with the time and having the visits split between Tori and another Therapist. Front office offered for Peyton to stay with Kayla for one visit and with Tori for the second, but mom declined and stated she would like the visit to be with another therapist. Front office then proceeded to ask if there was any issue with Peyton's current therapist and mom stated there was no issue and they like their current therapist, but at home Peyton asks for Tori frequently so they think it would be best to have tori for one visit and another therapist for the second visit. Front office then told mom if there isn't another therapist to take the second visit she will have to stick to 1x/wk until Tori has availability to see the patient 2x/wk. Mom agreed and asked to cancel all speech appointments for this week. Front office notified mom we would continue with this new schedule Monday of next week 02/09/2026. Mom acknowledged and agreed. Both therapist notified.

I spoke to both parents and let them know that I apologized enough already for what happened, that therapist was having a bad day, and that I was good having them come back because I know we are the best for Peyton. I mentioned that I wanted to have that call to clear the water and let them know that we are starting fresh and that there will be no need for them to feel embarrassed or for us to walk on egg shells. We will treat them professionally and they will treat us the same. They did not start services anywhere else, so our existing auth should be good to continue. I did tell them that they need to get a new DR referral. They will be starting ST again for now and maybe OT in the future.

As always, we at New Beginnings strive to keep our patients, parents, therapists, and staff safe and healthy. 03-04-2026 04:20 PM	Bertha Solis	Hello, this is the exercise program that I believe would be beneficial for Peyton- the reflexes are described and then it gives you examples for exercises, I would do this 1 to 2xs a day for 10-15 reps, select 1 exercise per reflex and for the ones that are holds 15-30 secs with 2-3 reps (: I believe the more gross motor we incorporate, the more our completion of tasks will increase, because his brain will learn increased traces to develop more ideas and planning when he is doing multi-step tasks.
