Speech Background History: Lana Archuleta is a 3-year-old female who was referred by her pediatrician for a comprehensive speech and language evaluation due to limited language skills and delayed milestones. Lana’s mother and family member served as the informants for the evaluation. The evaluation was performed in English as this is the primary language in the household. Lana’s mother reported patient was born full term and no complications during birth. Mother reported all milestones were met within normal limits. Mother reported pediatrician was concerned with developmental milestones. Mother also reported Lana only really plays with her older sister and has not seen her interact with other kids at this time as often. Lana demonstrated difficulty with transition to therapy room. During evaluation Lana was observed to participate and require prompts to participate in activities. Lana presented with immediate imitations from clinician. Mother stated she often observes Lana doing this at home as well.

Speech Assessment Summary: Lana is a 3 year 6 month old girl who is due for a review of her speech and language development. Lana was accompanied by her mother who remained in vehicle throughout the re evaluation. Standardized assessment, developmental data, observations, and therapy notes were used to determine progress and growth. Mother stated she is happy with her progress, and see's a significant difference after receiving services. She stated that she is using an exponential amount of language in all settings. She added that her teacher at school does not relay any concerns about Lana's speech or language. She stated that all the children in class ask Lana for help with concepts such as the alphabet. A child Lana's age should . The Preschool Language Scales Fifth Edition was used to assess Lana's Receptive and Expressive language skills. The Preschool Language Scales Fifth Edition (PLS5) was used to reassess Lana's language development and progress. She obtained an Auditory Comprehension raw score of 40, with a standard score of 91, indicating that she is within the AVERAGE rang when compared to children her age. In Expressive communication Lana obtained a raw score of 34 with a standard score was 82 indicating a MILD expressive language disorder. Overall, Lana obtained a Total Language raw score of 174 and a standard score of 86 indicating that her score is right above average when compared to children her age. Lana's receptive and expressive language strengths include: understanding spatial concepts (in, on, out of, off) without gestural cues, understanding quantitative concepts (one, all), making inferences, understanding analogies, identifying colors, identifying shapes (star, circle, square, triangle), pointing to letters, identifying advanced body parts, understanding quantitative concepts (each, every), naming a variety of pictured objects, combining three or four words in spontaneous speech, using a variety of nouns, verbs, modifiers and pronouns in spontaneous speech, using present progressive ing verbs, and answering what and where questions.
