Mother Samantha, was present in order to serve as the primary informant throughout the evaluation. Per mother's report child was born full term. Child did not experience any complications at birth, does not have any visual or hearing deficits. Per mother's report Adalynn demonstrates limited attention, focus, concentration and during writing tasks child struggles with letter reversals, letter identification and reading skills as well.

Adalynn also demonstrates deficits in sensory processing and self-regulation, which have impaired their ability to complete activities of daily living and instrumental activities of daily living that compromise their routines and roles at home and in the community. Child demonstrate medical necessity deficits that warrant skilled OT interventions in order to address their established delays and facilitate functional independence and performance. Assessment of Goals/Response to Treatment: Therapist presented a listening activity to work on attention to task and following verbal instructions. Adalynn was able to follow verbal instruction "color the balloon purple and then draw a square around it". She followed with moderate support. Mod cues provided to attend to boundaries when she colored. Therapist then presented a game of Spot It. Adalynn followed game objectives with min verbal prompting. Cues provided to slow down and visually scan her entire card. She transitioned away from therapy session with prompting x 3.

Due to a recent change in Tricare’s process, all patients are now required to be seen by their Primary Care Provider (PCP/PCM) and have an updated referral submitted to Tricare before we can request authorization for therapy. Please note: -The referral end date provided by your PCP/PCM will determine the authorization period for therapy services. -If possible, we recommend requesting a referral period of up to one year, as this will help reduce the number of required follow-up visits. -If you are unsure whether a current referral is on file, please contact your PCP/PCM or Tricare Referral Management directly. This change began earlier this month. While we are actively reviewing all existing referrals, we kindly ask for your assistance in scheduling an appointment with your PCP/PCM and ensuring an updated referral is submitted to help avoid any delays in therapy services. If you are able to view your referral end date, please be aware that your child will need to be seen again before that date to continue services.
