Assessment of Goals/Response to Treatment: The patient was alert and cooperative throughout the skilled occupational therapy session. Intervention focused on enhancing psychosocial and cognitive integration skills through structured, therapeutic activities. During cooperative play with puzzles, the patient addressed goals related to activity initiation, sequencing, and sustained attention. He demonstrated an improved ability to initiate the task with the support of a visual schedule and sustained his focus for a 10-minute interval with minimal redirection. Progress continues toward the goal of independent toileting hygiene; during a simulated routine, the patient required moderate verbal cues for thoroughness, a notable improvement from the maximum assistance previously required, indicating he is approximately 56% toward meeting this goal. Gains were also observed in the self-care goal for dressing, where he utilized organizational strategies to complete the routine within 18 minutes with minimal prompts for sequencing, showing significant progress at 70% toward the established 15-minute timeframe.
