Speech Background History: Dante is a 7-year, 1-month old male who was seen for an initial evaluation per referral of PCP to determine his current level of functioning and if skilled speech intervention is warranted at this time due to concerns regarding speech fluency. Per mother’s report, Dante has demonstrated instances of stuttering since approximately four years of age. She noted that these disfluencies do not occur frequently and do not appear to be associated with stress, as there have been no significant life events or environmental changes that might contribute to their presence. Although the stuttering had not previously been a major concern, Dante shared that it has recently become more bothersome. As he becomes more aware of his speech and interacts with peers in a school setting, he has begun to feel embarrassed when speaking in front of others. He described a sensation of his throat and face feeling “stuck” during moments of disfluency and expressed frustration about not knowing how to make it stop. Both Dante and his mother expressed a strong interest in learning strategies to help him better manage his speech. Mother reported that the primary language in the home is Spanish and Dante's academic language is English. During the evaluation, Dante frequently code-switched between English and Spanish, though he primarily responded in English. Speech Assessment Summary: Dante is a 7 year 6 month old male who was re-evaluated to determine eligibility for speech and language services. He previously presented with a MILD FLUENCY DISORDER and a MILD ARTICULATION DISORDER. The following assessments were used based on previous diagnoses and clinical observations: Stuttering Severity Instrument (SSI-4) to assess fluency, Goldman-Fristoe Test of Articulation 3 (GFTA-3) to assess articulation, and Clinical Evaluation of Language Fundamentals 5 (CELF-5) to assess expressive and receptive language. The following results were found: GFTA-3: Sounds-in-words: Total Raw Score 1, SS 105, Percentile Rank 63 Scores are indicative of WNL articulation skills. SSI-4: Total Score 2.83, Severity WNL CELF-5: Core Language Score: Sum of Scaled Scores 12, SS 59, Percentile Rank 0.3 Language Content Index: Sum of Scaled Scores 12, SS 65, Percentile Rank 1 Dante demonstrated below-average performance within the Core Language and Language Content Index domains, reflecting difficulties across both receptive and expressive language areas. During the Sentence Comprehension subtest, he exhibited challenges understanding sentence modifications, prepositional phrases, and indirect objects, particularly within sequence and temporal conditions. These patterns suggest difficulty processing complex sentence structures and understanding relationships between events and objects over time. Performance on the Linguistic Concepts subtest also indicated reduced comprehension of temporal and multi-step directions, suggesting difficulty integrating order and time-related language. In expressive tasks, Dante had a hard time formulating and recalling sentences, which affected performance on items requiring verbal organization and syntactic planning. On the Word Structure subtest, he showed inconsistent use of irregular plurals, possessive nouns, objective pronouns, comparatives and superlatives, as well as auxiliary and contractible copula forms. In therapy, Dante has been observed to code switch and to be English dominant, as it has also been reported that is the language he feels more comfortable speaking.
