Occupational Background History: Benjamin is a 9 year old boy was seen on this date for a scheduled occupational therapy evaluation in order to obtain current level of function. Mother, was present in order to serve as the primary informant throughout the evaluation. Per mother's report child was born at full term gestation. Child did not experience any complications at birth, does not have any visual or hearing deficits. Child does have a history of seasonal allergies and asthma.

Occupational Background Summary: Child demonstrates significant difficulty with self regulation skills highly likely due to sensory processing difficulties. Child currently demonstrates attention span approximately 2-3 minutes before being distracted by typical environmental stimuli which resulted in gazing around the room and talking about unrelated topics. Parent reports this is typical behavior. Child struggled to initiate directions via visual demonstrations, and verbal prompting for motivation, and redirection to task. Child exhibits limited attention evident by distracting from or protesting non-preferred activities, and need for frequent redirection.
Occupational Diagnosis/Impressions: Benjamin demonstrates deficits in fine motor precision, fine motor integration, visual motor integration, manual dexterity, manual coordination, sensory processing and self-regulation, which have impaired their ability to complete activities of daily living and instrumental activities of daily living that compromise their routines and roles at home and in the community. Child would greatly benefit from skilled OT interventions in order to address their established delays and facilitate functional independence and performance. Home programming alone is not sufficient to make the needed progress, direct one to one services are required for progress to be achieved. Within 6 months, Benjamin will be able to understand and process at least 8/10 emotions in order to communicate with his feelings with family/teachers/therapist reducing adverse behaviors with min support. Goal Met 2. Discriminate between relevant and irrelevant auditory stimuli as evidenced by ability to attend to classroom, gym, auditorium with 75% accuracy, independently to demonstrate improved organization of auditory input. Goal Met 3. Within 6 months, Benjamin will complete selfcare tasks with min A in order to promote independence with ADL's. Goal Met 4. within 6 months, Benjamin will initiate a task with less than 2 prompts 75% of the time to improve attention/focus to task and decrease anxious behaviors. Subjective: Patient completed all required tasks,

Assessment of Goals/Response to Treatment: The patient was cooperative and engaged throughout today's session, participating in all skilled activities designed to improve cognitive integration, visual processing, and fine motor skills for increased independence with activities of daily living. Progress was demonstrated toward the goal of improving memory and sequencing; when presented with a functional task, the patient accurately completed a 3-step directive on 2 out of 3 opportunities, requiring moderate verbal cues. This indicates an improvement from a baseline of requiring maximal support for multi-step instructions. In addressing self-care skills, the patient utilized a visual schedule to complete a simulated hygiene routine, successfully initiating 3 out of 4 steps with moderate verbal cues to reference the aid, showing progress from requiring maximal prompting to begin such tasks. However, significant deficits persist in fine motor skills, specifically with in-hand manipulation. During a sorting task involving small objects, the patient required maximal verbal and tactile cues to facilitate translation and shift, dropping items on 4 out of 10 trials and necessitating hand-over-hand assistance to complete the activity. Continued skilled occupational therapy is medically necessary to address these fine motor deficits and further improve functional performance with self-care and daily routines.

Due to a recent change in Tricare’s process, all patients are now required to be seen by their Primary Care Provider (PCP/PCM) and have an updated referral submitted to Tricare before we can request authorization for therapy. Please note: -The referral end date provided by your PCP/PCM will determine the authorization period for therapy services. -If possible, we recommend requesting a referral period of up to one year, as this will help reduce the number of required follow-up visits. -If you are unsure whether a current referral is on file, please contact your PCP/PCM or Tricare Referral Management directly. This change began earlier this month. While we are actively reviewing all existing referrals, we kindly ask for your assistance in scheduling an appointment with your PCP/PCM and ensuring an updated referral is submitted to help avoid any delays in therapy services. If you are able to view your referral end date, please be aware that your child will need to be seen again before that date to continue services.
