Speech Background History: Emma is a two year seven month old that came into the offices with mother and referral from doctor. Mother served as informant for this report and discussion regarding Emma's speech and language development. Mother reports that Emma is very talkative but also very difficult to understand. Emma stays at home with mother, as mother works from home and they spend the majority of their time together. Emma has playmates and an older sister that spend time with her, but an unfamiliar communication partner always turns to parent to find out what it is that Emma is trying to say. English is spoken in the home 100% of the time, as Emma does not go to daycare or receive ECI services. Mother explained that the services received from ECI were not helping Emma so she stopped receiving services. Emma talks to mom using English, watches videos in English, and plays with toys using English. It was determined after parent report and building some rapport with Emma; the standard assessment would be done in English. Emma came into the therapy room with little difficulties and began to play with the therapist right away. She answered questions using her words, as well as interacted by labeling, requesting and commenting. Mother reports that Emma is friendly with almost everyone. After a few minutes of play, therapist put items away and Emma was amenable to taking the Assessment.

Speech Background Summary: Emma is a busy toddler that is talkative and knows what she wants. Emma spoke with the therapist in words and phrases. Her speech is difficult to follow and at times, when excited, her speech was unintelligible. As she got excited her speech deteriorated. Play was appropriate and Emma was able to take turns and ask for items needed. She did need a timer in order to finish and transition, but she did so with an animal timer and continued positive feedback for behavior.

Emma presented with BACKING and FRONTING, i.e. dog vs. god, gate vs. tates, and sheep vs. peets. These Processes should be extinguishing at her developmental age, although for both to be happening within words and at Emma's age is considered atypical. Emma also presents with FINAL CONSONANT DELETION, which again, should be extinguished by age 3, although within connected speech FINAL CONSONANT DELETION, CLUSTER REDUCTION, as well as changing of VOWEL AND CONSONANTS within a word structure are considered Atypical. Intervention and remediation are determined to be necessary as Emma DOES NOT present with speech production typical of her same-age peers. The score indicates more than 2 STANDARD DEVIATIONS BELOW that of average for speech development. Speech therapy is warranted at this time to target ATYPICAL PHONOLOGICAL PROCESSES and specific speech phonemes as needed. Speech therapy is recommended at one time per week, as this is what mother can do at this time, at 45-60 minutes a week. Speech Assessment Summary: Emma, a 3-year-old female, was referred for a re-evaluation due to ongoing concerns with articulation. Emma has participated in speech therapy services and has displayed significant progress since beginning intervention. She continues to increase awareness of sounds and placement of articulators, which has contributed to her overall growth in communication. The Goldman-Fristoe Test of Articulation, 3rd Edition (GFTA-3), was administered. On the Sounds-in-Words subtest, Emma obtained a raw score of 71, a standard score of 79, a percentile rank of 8, and an age equivalent of less than 2 years old. These results reflect performance below age expectations, consistent with her continued articulation difficulties. Emma currently presents with a MILD PHONOLOGICAL AND ARTICULATION DISORDER. During the evaluation, Emma demonstrated the ability to verbally express her wants and needs; however, her overall intelligibility remains reduced. She continues to experience difficulty with the production of multisyllabic words, cluster reduction, and final consonant deletion. While these error patterns significantly impact her clarity, Emma has demonstrated noticeable improvement in her speech sound production compared to when she first began therapy. She has met and made progress toward all of her goals and continues to increase her overall intelligibility. Informal language assessment during the evaluation revealed no concerns, and Emma’s mother reports no difficulties in her receptive or expressive language abilities. Parent report and clinical observation indicate that Emma has shown substantial gains in confidence and is more willing to communicate with others. Her mother noted that Emma is speaking more frequently and demonstrates excitement about her progress. At the same time, frustration occasionally occurs when Emma struggles to produce certain sounds, which can interfere with her ability to communicate clearly and effectively. Despite these challenges, Emma’s progress has been steady and significant, and she now presents with a mild phonological and articulation disorder. At her age, children are expected to demonstrate greater speech clarity and intelligibility, with most sounds understood by both familiar and unfamiliar listeners. Emma’s current level of functioning remains below developmental expectations, which impacts her ability to interact fully with peers, communicate effectively with caregivers, and express her needs with clarity. Without targeted intervention, Emma is at risk for persistent articulation delays, frustration, social withdrawal, and behavioral challenges related to communication breakdowns. Skilled speech therapy continues to be medically necessary to support Emma’s articulation development. Intervention will focus on improving her production of targeted sounds, increasing her overall intelligibility, and promoting functional communication through play-based activities, modeling, and structured practice. Parent education will continue to be incorporated to encourage carryover of strategies across environments. Early intervention remains critical to reducing the long-term impact of her articulation disorder and promoting age-appropriate communication skills that will support future academic success and social interaction.

Assessment of Goals/Response to Treatment: The patient presented as alert and cooperative, demonstrating excellent engagement and completing all therapeutic tasks as directed. Progress was noted across all articulation objectives. The patient has successfully met the goal of imitating oral-motor movements for the production of target word shapes, achieving 80% accuracy (8 out of 10 trials) in producing CVC and CVCV words given minimal cues. This improved motor planning has contributed directly to an increase in overall speech intelligibility. When utilizing traditional articulation strategies, the patient demonstrated intelligible speech in 80% of opportunities during structured tasks, significantly enhancing her ability to be understood by familiar and unfamiliar listeners. Furthermore, she has exceeded the objective related to phonological processes, demonstrating suppression of Final Consonant Deletion in 90% of obligatory contexts during both play-based and structured activities. This marked improvement has a significant functional impact on her capacity to communicate wants and needs effectively within her daily routines.

Has Emma had any major illnesses or accidents? When was her last wellness check? 3. Does Emma get a good 8-10 hours of sleep? Does she wake up during the night? How often? Does she snore or sleep with her mouth open? 4. Is Emma on any medications? If so, please provide name, for what, and how many/much a day. 5. Is Emma in day care or have any other services provided? 6. How is Emma's Vision? How is Emma's hearing? Did she pass the new born hearing screening at the hospital? 7. Any concerns with eating? Does Emma have a good healthy diet? A variety of at least 30 foods? 8. Are there any siblings? or extended family? 9. What is your main concern for Emma? Where would you like to see her a year from now? and in the future? 10. When did you notice that Emma was not producing any language like she should? 11. Any one else in the family, previously or now, had speech therapy? 12. Besides the pediatrician, have there been consults with any other doctors, referrals for something else? Thank you!

Last wellness check was in February. 3. Yes she gets about 8-10 hours of sleep. Sometimes she still wakes up in the middle of the night I would say about 2 times a week. She sleeps with her mouth open but at first then eventually closest it. Doesn't snore. 4. No medications 5. No daycare, I take care of her at home since I work from home. 6. Both hearing and vision exams were taken and passed them both 7. Yes, she loves all food 8. One sibling, Aaliyah 12 year old 9. My main concern is she's not where she's supposed to be at as far as her speech. She talks a lot however some of it is still not clear. I would want her to speak clearly 10. She was receiving at home speech services and they advised me she should be saying a certain amount of words which she wasn't. 11. Yes my other daughter had speech impediment, she wasn't able to hear due to severe ear infections and had to have tubes in her ear. 12. No other referrals, no second opinions.

04-22-2025 09:04 AM	Cathlene Chapman	Good morning, I just wanted to let you know that after reviewing Emma's standardized assessment, it does indicate needed intervention for some phonological processes that need to be extinguished and also considered atypical. Some of those we discussed and a few that we will also focus on will be sounds used for consonants and vowels that are not usually substituted. But I will recommend once a week at 45-60 minutes for a Moderate Phonological Process Delay. I'll turn in report today or tomorrow and hopefully in about 10-14 days the treating therapist will be in touch. Thank you! Let me know if you have any questions.
