Speech Assessment Summary: Jorge is a 2 year, 4 month old male who was seen on the above date for a re-evaluation to determine his current level of functioning and if ST services continue to be warranted at this time. Jorge’s mother continues to report a significant improvement in his overall communication skills since the previous evaluation. No other changes to his medical history or lifestyle were reported. He appeared to be in good general health on the day of the evaluation. Jorge’s receptive and expressive language skills were assessed using the Preschool Language Scales, English, fifth edition (PLS-5). The following were his scores on each subtest: Auditory Comprehension standard score 69 (raw score: 20, percentile: 2, age equivalent: 1 year 4 months) - SEVERE Expressive Communication standard score 85 (raw score: 25, percentile: 16, age equivalent: 1 year 18 months) - MILD Total Language Score 76 (raw score: 45, percentile: 5, age equivalent: 1 year 6 months) - MODERATE Jorge's scores indicate a MODERATE to SEVERE mixed expressive/receptive language disorder. He exhibited main difficulties in the following areas: naming objects, identifying objects, following directions with gestural cues, understanding verbs during play, using words for pragmatic functions, and word combinations. Jorge's strengths were in the following areas: looks at objects or people the caregiver points to and names, responds to an inhibitory word, understands a specific word or phrase without the use of gestural cues, demonstrates functional, relational, and self-directed play, babbles two syllables together, uses representational gesture, uses at least one word, produces syllable strings with inflection similar to adult speech, participates in a play routine with another person for at least one minute while using appropriate eye contact, imitates one word, produces different types of consonant-vowel combinations, and uses gestures and vocalizations to request objects.
