Mother was present in served as the primary informant throughout the evaluation. Per mother's report, Delaylah was born at 40 weeks gestation. Delaylah did not experience any complications at birth, she does not have any hearing deficits. She has been prescribed eyeglasses. Mother reports she is pending scheduling an appt for eye exam. Delaylah does not have history of seasonal allergies.

Delaylah currently demonstrates attention span approximately 2 minutes before losing focus on and requiring prompting to complete instruction provided . Delaylah demonstrated delayed response to initiation and to completion simple one step directions. She completed evaluation tasks with moderate visual demonstrations, and verbal prompting for motivation, task initiation, and redirection to task. Delaylah exhibits limited attention evident by fleeing/escaping non-preferred activities, and need for constant redirection.

Assessment of Goals/Response to Treatment: Delaylah actively participated in today’s session, completing all assigned therapeutic activities. The intervention targeted cognitive, psychosocial, and motor skill development through structured, goal-oriented tasks. Using a sensorimotor approach combined with positive reinforcement, Delaylah engaged in a tabletop activity focused on single-digit addition and subtraction. This task required problem-solving, sequencing, and visual-motor integration. She successfully completed the worksheet, demonstrating effective eye-hand coordination and in-hand manipulation skills during writing. Progress was observed in task initiation. When presented with the math activity, Delaylah began within two minutes after receiving two verbal cues for orientation, showing improvement compared to previous sessions that required moderate assistance and longer initiation time. Additionally, gains were noted in sustained attention following sensory integration strategies. After participating in a preparatory heavy work activity, Delaylah maintained focus on the structured cognitive task for 12 minutes with only one verbal redirection, surpassing her prior baseline of 10 minutes. Psychosocial development was supported through a therapeutic discussion about returning to school.
