Speech Background History: Naya is a 4-year, 4-month old female who was seen on the above date for an initial evaluation per referral of PCP to determine her current level of functioning and if skilled speech intervention is warranted at this time.Naya was accompanied to initial evaluation by her mother who remained in the lobby during the evaluation however she provided written and oral information referenced in this report. Naya has medical diagnoses of unspecified lack of expected normal physiological development in childhood (ICD-10 Code: R62.50) and Developmental disorder of speech and language, unspecified (ICD-10: F80.9). Mother’s main concern is regarding Naya’s communication as she has very limited verbal output. Mother reported Naya frequently engages in immediate echolalia and has difficulty responding to questions. Additionally, mother noted that Naya often uses jargon in her speech. The primary languages of the home are English therefore all testing was conducted in English. If Naya is deemed appropriate for skilled speech and language intervention, it is recommended that therapy should be conducted in English.

Speech Assessment Summary: Naya is a 4- year, 9 month old female who was seen to assess her speech and language skills for continued speech therapy. Naya's mother provided pertinent information prior to the evaluation, which is later referenced in this report. Naya is currently using approximately 2 word phrases, however the majority of her expressive language is made up of immediate echolalia. Treating therapist stated patient is demonstrating difficulty with pragmatics, and is highly unintelligible. She added that she has difficulty with comprehending basic concepts. Treating therapist informed evaluating therapist of concerns with a possible diagnosis of Autism due to observing characteristics throughout the course of therapy. Mother stated the school speech therapist stated she was confident that she did not have Autism. According to ASHA guidelines, by 4 years 9 months, a child typically speaks clearly enough to be understood by both familiar and unfamiliar listeners, uses sentences of four or more words with correct grammar (including pronouns, plurals, and verb tenses), tells stories with a clear beginning, middle, and end, understands and answers a variety of “wh-” questions, follows multi-step directions without visual cues, uses language to share ideas, engage in conversation, and take turns appropriately, and demonstrates age-appropriate play and social communication skills such as cooperating with peers and discussing feelings or experiences. The articulation screener was completed on the PLS5 to which she made 16 errors out of 14 words consisting of CVC and CVCV word combinations. Based on patient's limited verbal output, her articulation and phonology will be monitored throughout the next authorization period. It is recommended that evaluating therapist complete an articulation/phonological processes assessment when patient's vocabulary has increased. The Preschool Language Scales Fifth Edition (PLS5) was used to assess her receptive and expressive language skills. Her Auditory Comprehension Raw Score was 29 with a Standard Score of 58 and a Percentile Rank of 1. Her Expressive Language Raw Score was 29 with a Standard Score of 60 and a Percentile Rank of 1. Her Total Communication Raw Score was 118 with a Standard Score of 56 and a Percentile Rank of 1. These scores indicate that she is more than 2.5 standard deviations below the mean compared to her same aged peers. Her receptive and expressive language strengths are as follows: understanding pronouns (me, my, your), following commands without gestural cues, engaging in symbolic play, recognizing actions in pictures, understanding use of objects, understanding quantitative concepts (one, all), making inferences, naming objects in photographs, using words more often than gestures to communicate, using words for variety of pragmatic functions, and naming a variety of pictured objects. Her receptive and expressive weaknesses are as follows: understanding spatial concepts (in, on, out of, off) without gestural cues, understanding analogies, understanding negatives in sentences, identifying colors, understanding spatial concepts (under, in back of, in front of, next to), understanding pronouns(his, her, he, she, they), using different word combinations, combining three or four words in spontaneous speech, using a variety of nouns, verbs, modifiers and pronouns in spontaneous speech, producing one four or five word sentence, using present progressing ing verbs, using plurals, and answering what and where questions. Based on formal assessment, parent interview, and clinical observation, Ella presents with a SEVERE MIXED RECEPTIVE/EXPRESSIVE Language Delay secondary to Autism Spectrum Disorder. Speech therapy is recommended at this time as Ella does not communicate like other children her age. Without Speech Therapy Ella's safety risk is HIGH, as she cannot communicate pain or danger in emergency situations and cannot effectively communicate her wants and needs.

Weaknesses in fine motor skills can affect a child's ability to eat, write legibly, use a computer, turn pages in a book, and perform personal care tasks such as dressing and grooming.

Baseline 0% Within 3 months, Naya will complete 2/4 memory recall tasks with atleast 50% accuracy provided with 5 or fewer verbal cues in order to sequence steps for ADLs. Baseline 0% Therapist presented play doh activity to work on sustained attention, grasping, and vmi skills. Therapist modeled using clay press to create shapes. Naya required max cues to sequence the task after visual demo. Mod cues provided after 3 trials. Therapist modeled using a knife to cut the play doh. She required tactile cues x 2 to grasp appropriately. Naya transitioned away from the activity with prompting x 2. Therapist presented Lego activity to further work on attention, transition, vmi, and transitioning skills. Naya did recall object of the activity from a previous session. Therapist provided mod cues to follow and attend to diagram. She required mod tactile cues to secure the Legos due to decreased strength. Therapist engaged her in pretend play with Legos figures to work on social, play skills. She reciprocated without prompting.
