Speech Assessment Summary: Jaethan is a 3 year 6 month old male who was re-evaluated to determine continued need of speech and language intervention. The evaluation was performed in English, as that is the primary language spoken at home. Jaethan's father brought him to the evaluation, however, he was not present during testing. Father reported that Jaethan was inserted PE tubes in November due to frequent ear infections. Jaethan was distractible and required redirection. The Preschool Language Scales (PLS-5) was used to assess Jaethan's expressive and receptive language skills. The following were his scores: Auditory Comprehension - SS 65 (Raw Score 32), Percentile Rank 1 Expressive Communication - SS 89 (Raw Score 32), Percentile Rank 23 Total Language - SS 75 (Raw Score 64), Percentile Rank 5 Scores are indicative of a MODERATE mixed expressive/receptive language disorder. However, Jaethan exhibited greater delays in the area of receptive language. Although Jaethan has made consistent progress in therapy, he would still benefit from language intervention targeting the following areas: understanding object functions, understanding spatial concepts, answering questions, using verbs ending in -ing, using plurals, understanding negatives in sentences. The PLS-5 articulation screener was also assessed due to speech sound errors noted. He exhibited the following errors that are NO LONGER judged to be age-appropriate: final consonant deletion, /l/ and /s/ distortions. Speech sound errors should also de targeted in therapy. Based on formal assessment, parent interview, and clinical observation, Jaethan continues to present with a MODERATE MIXED RECEPTIVE/EXPRESSIVE LANGUAGE DISORDER. Speech therapy continues to be recommended at this time, as Jaethan does not communicate like other children his age. Without Speech Therapy Jaethan's safety risk and functional health are HIGH, as he cannot communicate pain or danger and may have difficulty following directions without multiple gestural prompts and understanding inhibitory words when in unsafe situations.

Jaethan’s mother served as the informant for the evaluation. Jaethan’s mother reported that feeding concerns began when he was approximately 2.5 years old with concerns consisting of limited texture acceptance resulting in developing increased selectivity. Jaethan’s food repertoire consists of proteins and fruits with limited vegetables accepted, however, Jaethan’s mother reported that she has noticed a change in his willingness to try and explore new and non-preferred foods. Additionally, Jaethan’s mother reported that she tends to modify his food by cutting it into small pieces due to safety concerns, however, noted no observed difficulty manipulating the foods he eats. Jaethan has no known food allergies, no gastrointestinal concerns, and no history of respiratory issues. Jaethan’s feeding and swallowing skills were assessed through a clinical feeding evaluation and caregiver interview to determine his current level of function and need for skilled intervention.

Feeding Assessment Summary: Jaethan presents with a moderate pediatric feeding disorder and oral phase dysphagia. Feeding therapy is recommended at this time and is deemed medically necessary to support and improve Jaethan’s tolerance of non-preferred textures and promote the development of age-appropriate oral motor skills (e.g. rotary chew). I wanted to update you regarding the pre-authorization request we attempted to submit to BCBS for Jaethan. Unfortunately, we are unable to move forward with the submission at this time. BCBS is requiring that Jaethan’s pediatrician submit a referral that includes the same diagnosis codes we are submitting for. This is necessary so that BCBS has documentation showing the pediatrician is referring your child to us for these specific services. Once the referral is submitted by the pediatrician and includes the necessary codes, we will be able to proceed with the authorization request to BCBS. We sincerely apologize for the inconvenience and appreciate your patience. They asked if you could please call again and inform them that you no longer have Blue Cross Blue Shield and if they can update Superior as main insurance and if they can provide you with a reference number for us . In addition, if you can also ask if they have an estimated date when it would show as primary insurance so New Beginnings can go back and double check the portail again. Thank you !
