Subjective: Patient completed all required tasks,

Assessment of Goals/Response to Treatment: Davion initially demonstrated significant behavioral dysregulation when redirected to a non-preferred therapeutic activity, as evidenced by crying, yelling, and hitting. These behaviors reflect ongoing difficulties with coping skills and self-regulation. He required approximately five minutes of skilled co-regulation to de-escalate and reach an appropriate level of alertness for participation. Following the regulation, Davion needed maximal assistance to initiate the structured task and is not yet meeting the goal of beginning activities with five or fewer verbal prompts. Once engaged, however, he showed progress by maintaining attention to a visual processing task for up to three minutes with moderate verbal cues for redirection. During a fine motor activity focused on in-hand manipulation, Davion demonstrated use of a pincer grasp with 4 out of 10 accuracy, requiring moderate physical and verbal support to appropriately handle small fasteners. Difficulties with cognitive sequencing were also observed. He was able to follow two-step directions in 1 out of 4 trials despite maximal verbal and visual cues; as a result, tasks were modified into single-step directions to improve success. Although the session began with notable resistance, the implementation of sensorimotor strategies and positive reinforcement supported Davion in re-engaging and ultimately completing all assigned therapeutic activities.

07-08-2025	Guadalupe Key	Therapist spoke with Davion's parents to discuss his behavior during speech sessions, as he has exhibited aggressive behaviors when asked to engage in non-preferred tasks. The therapist also contacted the speech therapist to share strategies for addressing these behaviors and to suggest incorporating sensory activities to help support attention and engagement.
07-08-2025	Daniella Anchondo	Patient has demonstrated a consistent pattern of screaming/screeching when presented with non-preferred tasks or when not allowed to engage in preferred behaviors. Today’s session focused on seated participation and following directions using the thematic activity “Submarine, Submarine, What Do You See?” During the activity, the patient was prompted to place Velcro pieces on the page. When the clinician redirected him (e.g., by saying “wait” or turning the page) and prevented him from removing the pieces, he responded by screaming. This behavior occurred multiple times but was brief and stopped once the demand was removed or transitioned. Patient exhibited physical behaviors including throwing the chair twice and hitting the clinician on the legs and arms on two occasions. During the session, the patient's father requested to enter the treatment room. The clinician asked front desk staff to redirect him, explaining that in-the-moment interruptions can be disruptive to the session and I would speak to parents after our session. Following the session, the clinician spoke with the parents and the patient’s OT about the need to target emotional regulation and behavioral strategies, with an emphasis on carryover in the home environment. The clinician offered for a parent to observe the next session, but to walk from the beginning, in order to minimize disruption. It was also discussed that while parent presence may be helpful for modeling, long-term goals include the patient’s ability to regulate and participate independently, as parents will not always be present during sessions.

Due to a recent change in Tricare’s process, all patients are now required to be seen by their Primary Care Provider (PCP/PCM) and have an updated referral submitted to Tricare before we can request authorization for therapy. Please note: -The referral end date provided by your PCP/PCM will determine the authorization period for therapy services. -If possible, we recommend requesting a referral period of up to one year, as this will help reduce the number of required follow-up visits. -If you are unsure whether a current referral is on file, please contact your PCP/PCM or Tricare Referral Management directly. This change began earlier this month. While we are actively reviewing all existing referrals, we kindly ask for your assistance in scheduling an appointment with your PCP/PCM and ensuring an updated referral is submitted to help avoid any delays in therapy services. If you are able to view your referral end date, please be aware that your child will need to be seen again before that date to continue services. 07-09-2025 02:45 PM	Guadalupe Key	Hi Mom! He had such a great session today! He worked really well, especially alongside another little girl who is also very sensory-seeking. They did amazing together! He was able to sit and engage in a sensory activity for about six minutes without needing any redirection, which is a big win. We worked on pulling beads out of pink Theraputty, and he completed the task with focus. We also used shaving cream again, this time with added toys, and he had so much fun. He then explored the rice and bean bins and was even able to share materials with his peer during the activity. He didn’t need the swing or the sensory room today at all, and there were no episodes of yelling, dropping to the floor, or attempts to hit. He was happy and engaged throughout the session! Thank you for the opportunity to work with him, he’s making steady progress, and you’ll start to notice the behaviors improving more and more over time 😊
