Occupational Background History: DX: H53.8 other visual disturbance / Poor Attention Stella is a 7.6 year old girl who was seen on this date for a scheduled occupational therapy evaluation in order to obtain current level of function. Both parents were present (Jordan and Maxim) in order to serve as the primary informant throughout the evaluation. Per mother's report child was born full term at 38 weeks. Child did not experience any complications at birth, does not have any visual or hearing deficits. Parents report history of limited attention, poor focus, anxious behaviors and difficulty with self regulation skills. Mother reports constant pacing and wondering, difficulty with responding to name or directions at times, difficulty processing emotions and feelings as well as sensory deficits such as (picky eater, poor attention, tactile aversion, auditory aversion and requires constant stimulation).

Occupational Background Summary: Stella demos difficulty with in-hand manipulation skills due to delayed finger dexterity, finger isolation and palm translation skills. Fine motor skills involved the small muscles of the body that enable such functions as writing, grasping small objects, and fastening clothing. Weaknesses in fine motor skills can affect a child's ability to eat, write legibly, use a computer, turn pages in a book, and perform personal care tasks such as dressing and grooming.

Due to age, Stella was assessed via standardized assessment BOT¬2. Per standardized assessment scores Stella demonstrated +1 gain in fine motor integration subtest . Stella falls well below average in the fine motor precision and fine motor integration portions of the BOT2 evident by well below average age equivalency scores. Subjective: Patient was alert and cooperative,

Assessment of Goals/Response to Treatment: Patient presented as alert and cooperative for today's skilled occupational therapy intervention. The session focused on improving psychosocial and cognitive integration skills through participation in a structured, multi-step craft activity. Therapeutic strategies such as a visual schedule, positive reinforcement, and a work-reward sequence were employed to facilitate activity initiation, sequencing, and sustained attention. The patient demonstrated progress toward her goal of improving attention for multi-step tasks, completing the activity with 5 verbal redirections to maintain awareness of the end goal, which is a significant improvement from her baseline of requiring maximum support. Regarding attention to detail, she engaged in the 10-minute structured activity and completed it with 6 minor omissions with the use of a visual checklist, showing positive gains toward her goal of completing similar tasks with no more than 4 errors. Progress was also noted in self-regulation and task initiation; with moderate verbal cues, the patient was able to wait for and attend to full instructions before beginning the activity, indicating improved impulse control. In addressing social skills, the patient required 4 moderate verbal and visual cues to maintain an appropriate arm's-length distance from the therapist during the session, demonstrating an emerging awareness of personal space and improved self-control when compared to her baseline of requiring frequent reminders.

Due to a recent change in Tricare’s process, all patients are now required to be seen by their Primary Care Provider (PCP/PCM) and have an updated referral submitted to Tricare before we can request authorization for therapy. Please note: -The referral end date provided by your PCP/PCM will determine the authorization period for therapy services. -If possible, we recommend requesting a referral period of up to one year, as this will help reduce the number of required follow-up visits. -If you are unsure whether a current referral is on file, please contact your PCP/PCM or Tricare Referral Management directly. This change began earlier this month. While we are actively reviewing all existing referrals, we kindly ask for your assistance in scheduling an appointment with your PCP/PCM and ensuring an updated referral is submitted to help avoid any delays in therapy services. If you are able to view your referral end date, please be aware that your child will need to be seen again before that date to continue services. I apologize for the inconvenience, the patient who was originally has the Wednesday time slot, was suppose to be terminating therapy this week. But they just let me know they would like to finish the two weeks of therapy before getting stationed elsewhere. We would schedule Stella Tuesday at 11, and Friday at 9am for two week. Beginning the week of April 28th we would schedule therapy Wednesdays at 10am and Fridays at 9am.
