Kenzo's mother provided pertinent information which is later referenced in this report. Kenzo is currently using gestures, some ASL and a few words. Mother stated her concerns are that he is not using 2+ word sentences at his age. He previously received speech therapy in Mississippi, prior to moving to El Paso. A child Kenzo's age should produce grammatically correct sentences, which are longer and more complex, including main character, settings, and words to connect information and ideas to tell stories, use at least one irregular plural form, understand and use location words, use more words for time, following simple directions and rules to play games, locates front and back of book and its title, recognize and name 10 or more letters and can write their own name, imitate reading and writing from left to write, blends word parts, produce most consonants correctly and speech and understandable in conversation. Kenzo was evaluated using the Preschool Language Scales (PLS¬5). Kenzo was evaluated using the Preschool Language Scales (PLS¬5) and the Clinical Assessment of Articulation and Phonology Second Edition (CAAP2) to assess his receptive and language skills, as well as his articulation and phonological skills. The language of the evaluation was English, and was determined based on parent report of English exposure via media (YouTube videos, games, and TV), and the primary language spoken at home. The PLS was completed to assess Kenzo's receptive and expressive language skills. A standard score within the range of typical development is between 85 and 115. Kenzo's Auditory Comprehension Language Raw Score was 39 with a Standard Score of 62 and a Percentile Rank of 1. Due to a recent change in Tricare’s process, all patients are now required to be seen by their Primary Care Provider (PCP/PCM) and have an updated referral submitted to Tricare before we can request authorization for therapy. Please note: -The referral end date provided by your PCP/PCM will determine the authorization period for therapy services. -If possible, we recommend requesting a referral period of up to one year, as this will help reduce the number of required follow-up visits. -If you are unsure whether a current referral is on file, please contact your PCP/PCM or Tricare Referral Management directly. This change began earlier this month. While we are actively reviewing all existing referrals, we kindly ask for your assistance in scheduling an appointment with your PCP/PCM and ensuring an updated referral is submitted to help avoid any delays in therapy services.
