Speech Background History: Marcel Franco is a 2-year 8-month-old male who was referred by his pediatrician for a comprehensive speech and language evaluation due to limited language skills and delayed milestones. Marcel’s mother served as the informant for the evaluation. The evaluation was performed in both English and Spanish however English was established to be primary language. Spanish support is recommended during session. Marcel’s mother reported patient was born full term however mother had preeclampsia and was diagnosed with lupus. Mother reported no NICU stay. Mother reported that at his 2-year checkup Marcel had made no progress in developmental checklist with pediatrician. Mother reported he was diagnosed with Autism (ICD 10 Code: F84.0) by a developmental specialist. Mother reported he is currently receiving ABA and has made progress since starting. Speech Assessment Summary: Marcel, a 3-year, 7-month-old male, was re-evaluated to determine continued eligibility for speech and language services. Marcel has been receiving speech therapy services as well as applied behavior analysis (ABA) since April 2, 2025. Marcel’s dominant language is English. Spanish is spoken at home by his mother; therefore, Spanish support is provided as needed. Parent report, clinical observation, and both formal and informal assessments were used to evaluate his current speech and language abilities. Marcel was administered the Preschool Language Scale–5th Edition (PLS-5) English. On the Auditory Comprehension subtest, he obtained a raw score of 23, a standard score of 55, a percentile rank of 1, and an age equivalent of 1 year, 7 months. On Expressive Communication, he obtained a raw score of 28, a standard score of 70, a percentile rank of 2, and an age equivalent of 2 years. His Total Language score reflected a standard score of 60, a percentile rank of 1, and an age equivalent of 1 year, 9 months. These results indicate that Marcel continues to present with a profound mixed receptive and expressive language disorder. Results should be reviewed with caution, as the PLS-5 is not standardized for children diagnosed with Autism. Marcel has demonstrated significant progress, as evident by consistent increase in scores. However, Marcel requires frequent verbal and visual redirection to remain engaged. He continues to demonstrate growth in his communication skills, particularly in word imitation and spontaneous use of vocabulary, and identifying objects. Caregiver report indicated that they are understanding Marcel more clearly and have noticed a significant increase in his attempts to communicate, however, consistency varies. They describe observing positive changes in his communication since starting services. Marcel has also shown gains in imitating and using words during structured therapy and natural interactions, and he has made progress on all his speech goals. Marcel is now producing 2-word phrases via imitation or knowledge of scripted phrases. Marcel is now identifying some actions, however, requires maximal gestural cues to remain engaged. His current goals continue to remain adequate and appropriate, as they target the fundamental basics of receptive and expressive language development needed for him to build foundational communication skills. Overall, Marcel continues to present with a PROFOUND MIXED RECEPTIVE AND EXPRESSIVE LANGUAGE DISORDER. At his age, children are expected to understand a wide variety of vocabulary, follow everyday instructions, and express themselves using short phrases to engage in early conversation and play. Marcel has made progress, but is still inconsistent. Marcel’s current functioning is considered to be below developmental expectations, impacting his ability to interact with caregivers, participate in age-appropriate play, and communicate his needs effectively. Without targeted intervention, he continues to be at a high risk for persistent delays, frustration, social withdrawal, and behavioral challenges related to communication breakdowns. Skilled speech therapy continues to be medically necessary to support Marcel’s language development. Occupational Background Summary: Child demonstrates significant difficulty with self regulation skills highly likely due to sensory processing difficulties. Child currently demonstrates attention span approximately 20 seconds before attempting to flee. Child struggled to initiate and/or complete simple one step directions with max assistance via visual demonstrations, and verbal prompting for motivation, task initiation and redirection to task. Child exhibits limited attention evident by fleeing/escaping non-preferred activities, and need for constant redirection. Child demos difficulty with in-hand manipulation skills due to delayed finger dexterity, finger isolation and palm translation skills. Fine motor skills involved the small muscles of the body that enable such functions as writing, grasping small objects, and fastening clothing. Weaknesses in fine motor skills can affect a child's ability to eat, write legibly, use a computer, turn pages in a book, and perform personal care tasks such as dressing and grooming.
Occupational Diagnosis/Impressions: Clinical impressions indicate that Marcel, a 3-year, 6-month-old male, presents with significant developmental delays impacting multiple functional domains. Standardized assessment using the Peabody Developmental Motor Scales, Second Edition (PDMS-2) reveals scores falling between -2.0 and -3.0 standard deviations below the mean for his age. His performance yielded age-equivalency scores ranging from 14 to 22 months, which represents a profound delay of 20 to 28 months when compared to his chronological age of 42 months. It should be noted that the reliability of these scores is considered questionable due to behavioral observations during the evaluation, including significant distractibility, a high activity level, and the need for numerous prompts to elicit cooperation; therefore, these scores may represent a conservative measure of his underlying abilities. The observed deficits in fine motor and visual-motor skills, which are well below the 1st percentile, directly and negatively affect his functional participation in age-appropriate activities. He demonstrates a primitive grasp on writing utensils, struggles with basic eye-hand coordination tasks such as stacking blocks or placing puzzle pieces, and lacks the bilateral integration required for tasks like stabilizing paper while scribbling. These motor planning and coordination deficits severely limit his ability to engage in meaningful play and pre-academic readiness activities. Functionally, these impairments manifest as a significant dependence on caregivers for activities of daily living (ADLs), including dressing and self-feeding with utensils, tasks in which a child his age would be demonstrating emerging independence. Furthermore, observational data indicates substantial challenges with sensory processing and regulation, evidenced by his high activity level, poor attention to task, and withdrawn social interactions. His inability to filter environmental stimuli and maintain self-control impedes his capacity to attend to structured tasks, follow simple directions, and engage in reciprocal social exchanges with peers, placing him at risk for safety concerns and social isolation. The combination of these motor, sensory, and regulatory deficits creates a significant barrier to his overall development and participation in home, school, and community settings. Based on the objective findings and clinical observations, skilled occupational therapy services are medically necessary to address Marcel's significant deficits in fine motor coordination, visual-motor integration, sensory processing, and self-regulation. These impairments currently restrict his ability to perform age-appropriate self-care tasks, engage in functional play, and participate in pre-academic learning opportunities. Subjective: Patient was alert and oriented,
Assessment of Goals/Response to Treatment: Therapist presented potato head activity to work on sustained attention, bilateral coordination, and grasping skills. Marcel was initially resistant to engaging in the activity evident by yelling "no" and pushing them away. Therapist then provided a puzzle as an other option to work on skills. Marcel opened/closed locks on puzzle with min assistance. Therapist used animal sounds with increased animation to encourage engagement with therapist, Marcel did provide increased eye contact when strategy was implemented. Therapist then presented color matching and stacking activity. Marcel was able to match colors independently with mod cues to stack boxes in descending order. Therapist modeled making car sounds and rolling cars back and forth. Marcel required mod support to roll car back to therapist. He transitioned away from therapy session with prompting x 4 to clean up the activity. Mod support provided during transition out of therapy session for safety.

04-24-2025Patient Note – Marcel Franco Marcel had recently begun services; however, his mother expressed that due to her constantly changing work schedule, she was unable to commit to a consistent day and time. I did manage to schedule a few sessions for her last week, which she attended, although she arrived a few minutes late to some. Unfortunately, this week, in the midst of managing several new patients, I mistakenly overlooked scheduling Marcel. At the end of the week, during my routine review, I realized he had not been seen and immediately called his mother to apologize for the oversight. Upon answering, the mother informed me that she had already decided to withdraw Marcel from New Beginnings and would be transferring services to Angels of Care. I expressed that I was sorry to see them go but understood her decision and reassured her that she is always welcome to choose the provider that best fits her needs.
04-10-2025Delia GomezThe patient’s mother expressed that due to her highly demanding and frequently changing work schedule, she is unable to commit to consistent therapy days and times for her child. As a result, she is requesting that the patient’s schedule be adjusted on a weekly basis according to her availability. I informed the mother that, while I cannot guarantee permanent time slots due to the unpredictability of her schedule, it is actually a requirement of our scheduling policy that patients are assigned consistent, permanent days and times. Despite this, I will make every effort over the next two months (prior to my departure) to accommodate her as best as possible in order to help her stay on track and make up for missed sessions. I also clarified that I cannot make any guarantees—especially regarding weekend sessions, such as Saturday or Sunday afternoons—even if those are the only times her schedule permits. Mother verbalized understanding and stated she is willing to work with us within the limitations discussed. This note is being documented for continuity, as this scheduling concern will need to be considered by the next clinician working with this patient.

As always, we at New Beginnings strive to keep our patients, parents, therapists, and staff safe and healthy. We have thermometers in our front lobbies and will be checking for high temperatures as we begin the traditional time of year for an increase in flu, colds, and COVID. Any child or parent with a temperature of 100 degrees and above will be asked to reschedule for another day & time.
If you or your child or someone in your home has COVID, we continue to ask that you stay home and quarantine to stop the spread. The CDC urges people sick with symptoms of respiratory viruses to stay home until their fever has disappeared for at least 24 hours without relying on fever-lowering medications.

Please do not come to therapy if you or your child are experiencing the following symptoms: fever, body aches, chills, vomiting, diarrhea, headache. Your treating therapist would be happy to reschedule your visit for you.

After speaking with Berenice and Lailani, we all agree that it would be best for Marcel to transition to once-a-week speech therapy sessions, starting this week like we spoke about previously. During our phone conversation, I hadn’t realized Marcel is still on the waitlist for occupational therapy (OT). While we had previously discussed the possibility of starting OT sooner rather than next year, we now believe it would be better to begin in the new year. This decision is based on the upcoming holiday schedule and therapist availability. I completely understand your concern about Marcel having to wait longer. Please rest assured that starting OT next year will not delay his progress, as he will not need to be re-added to our waitlist or start the process over again. We would like to inform all patients with Blue Cross Blue Shield (BCBS) insurance that insurance benefits reset at the beginning of the new year. This means that all copays, coinsurance amounts, deductibles, and out-of-pocket maximums will reset.
As a result, payments for visits will be charged at the full allowed amount until your deductible is met. Depending on your specific BCBS plan, you may or may not have a deductible.

Below are the allowed amounts that will apply until your deductible is met:

Re-evaluation: $234.19

Feeding Therapy (FD):
Initial Evaluation: $67.73

-1 initial evaluation

Visits remaining after this period: 32

2nd ST auth of the year (ending 10/09/2026)
Visits that will be used during this period: 14
-1 re-evaluation
-13 speech therapy sessions

Visits remaining after this period: 18

2nd OT auth of the year (ending 01/16/2026)
Visits that will be used during this period until the end of the year: 13

-1 re-evaluation
-12 occupational therapy sessions

Currently we have a waiting list for new patients and patients starting school in August, which means all of our schedules have shifted and changed this past week. I currently hold 4 spots for Marcel only, two in the morning and two in the afternoon. My clinical director had suggested sharing Marcel with Ms. Lailani which can see him on Saturdays for a weekly visit, and I can see him on Monday and Tuesday the same alternating way we have been. I can also plug in make-up visits on your days off with any available openings I have.
