A speech evaluation was requested by her pediatrician based on her chart, as she does not use words. Mother was present in the evaluation and served as the informant. Per parent report, parents actively read to her and try to motivate her to imitate words and sounds. Mother stated Olivia uses about 6-7 words intentionally, but not consistently. Words often used are mom, dad, ball, more, no, help. Per parent report Olivia follows directions accordingly with gestural cues. Per parent report, she sometimes uses signs "more", "milk", "all done". She reported her language of exposure to be 70% English and 30% Spanish. Mother stated Olivia presents with no aversions to textures, however, she sometimes gets overstimulated and tenses up.

Speech Assessment Summary: Olivia is a 2 year, 8 month old who was re-evaluated today to determine continued eligibility of speech and language services. The Preschool Language Scales Fifth Edition (PLS-5) was used to assess overall expressive and receptive language skills. The evaluation was administered in English, as it has been determined to be her dominant language. The following were Olivia's scores: Auditory Comprehension - Standard Score 98 (Raw Score 34); Age-Equivalent 2;8 Expressive Communication - Standard Score 80 (Raw Score 26); Age-Equivalent 1;9 Total Language - Standard Score 88 (Raw Score 60); Age-Equivalent 2;3 Olivia has demonstrated progress as evident by goal progress and increase in standard scores, however, Olivia continues to inconsistently use words and produce approximations. Olivia requires maximal prompts and models to name objects or combine 2 word utterances. Olivia has shown an improvement with sensory regulation, however, she still often becomes shy in front of unfamiliar listeners and avoids contact. Subjective: Patient completed all required tasks,

Assessment of Goals/Response to Treatment: Olivia actively engaged in today’s skilled occupational therapy session, requiring moderate assistance to complete therapeutic activities aimed at improving motor, psychosocial, and cognitive skills. Progress was observed in Activities of Daily Living, particularly with lower body dressing. Initially, Olivia had difficulty sequencing the task of pulling up her pants from the back. After introducing a compensatory strategy—providing moderate verbal cues to pull the back of the pants up before the front—she was able to complete the task more efficiently with only minimal physical assistance. To support psychosocial goals such as social interaction and initiating activities, positive reinforcement using a preferred motivator was incorporated. This approach was effective, as Olivia engaged in a shared, therapist-directed play activity for five minutes with moderate verbal prompting, demonstrating progress toward sustained participation. Fine motor and visual-motor skills were targeted through a sensorimotor activity using play-doh on a baking sheet. Olivia showed good bilateral coordination and in-hand manipulation by rolling and shaping the material. She then used a pencil to draw shapes, reflecting improvement in visual-motor integration and the ability to sustain attention for a 10-minute activity. The tactile component of this task also functioned as a sensory preparation strategy, helping her remain regulated and smoothly transition into the following shared play activity.

We are currently waiting on your pediatrician to sign some papers necessary to submit to the insurance. We sent him these papers on 07/28/2025. We also went in person ourselves on 08/07/2025 to retrieve the signed papers, but the office said the doctor hasn't signed them yet. We wanted to ask if you would be able to go in person to the office as well to try and get these papers signed to try and move the process along so Oliva can start to be treated for Occupational therapy.
