Feeding Background History: Elianna is a 1 year, 9 month old female who was referred for a feeding evaluation by her pediatrician due to parents having concerns with patient's limited food inventory and food aversions. Elianna's parents, Mr. and Mrs. Estrada served as the informants for this evaluation. Elianna is currently receiving her nutrition from preferred food items and drinks via straw. Parents reported she was progressing well with foods/liquids; however, during transition from bottle to cup, she demonstrated difficulty with breathe-swallow-coordination. Father further reported, that she began to regress about 1 month ago as she is now refusing foods that she previously consumed. He further reported that she is "super picky" with foods. Mr. Estrada reported for Elianna to have no known allergies, no respiratory issues and no gastrointestinal issues. Elianna's feeding/swallowing skills were evaluated using a bedside swallow evaluation and skilled meal observation. Due to Elianna's language background and input/output, the language of intervention will be English.
Feeding Medical History: Parents reported pregnancy was unremarkable. Elianna was delivered at home via vaginal birth. She was born with Fetal Alcohol Syndrome and brain damage. At 18 months of age (July, 2024), an MRI study revealed microcephaly and developmental global delays. On July 2024, Elianna had hip surgery due to left leg being out of socket. She now wears a brace at night.

Based on the bedside swallow evaluation and skilled meal observation Elianna presents with a CHRONIC PEDIATRIC FEEDING DISORDER and ORAL DYSPHAGIA characterized by impaired oral intake that is not age-appropriate and may be attributed to medical diagnosis (Fetal Alcohol Syndrome, brain damage, microcephaly, developmental global delays), nutritional status (food jagging), feeding skill (sensory sensitivities, oral dysphagia), and/or psychosocial dysfunction (problematic mealtime/learned responses). Overt clinical signs/symptoms of aspiration/penetration were observed with trials of thin liquid via straw and solids (crackers) resulting in concerns regarding the pharyngeal phase of swallow. Additionally, parents did not report unexplained fevers or history of aspiration pneumonia. The Pediatric Eating Assessment Tool (PediEAT) was completed by Mrs. Estrada which is a subjective questionnaire used to assess Elianna's feeding behavior. Elianna received a rating approaching with "No Concern" on all subscale scores: selective/restrictive eating, problematic mealtime behaviors, physiologic symptoms, and oral processing. Elianna's ratings on the PediEAT are not consistent with her diagnosis and overall performance in the evaluation. CURRENT FEEDING: Elianna currently eats with supervision in the highchair and is presented with food 5x a day (3x meals, 2x snack). Currently, she is unable to sit up for longer than 1 minute due to poor trunk support. Typically, mealtime is no longer than 30 minutes. Parents are concerned that Elianna had sensory aversions and is food jagging (no longer eating foods that he use to eat). Per parent report, Elianna accepts crunchy, smooth, and lumpy consistencies that are warm, cold, and cool in temperature. Parent further reported for her overall appetite to be fair. When Elianna is presented with non-preferred foods she will spit food out. ACCEPTED FOODS: Elianna eats/chews 30 foods and drinks 5 liquids (juice, lemonade, milk, milkshakes, water). CARBS: crackers, chips, bread, rice, sweet pastries, potato (tater rounds, fried potato, wedges), sweet potato (baked, fries, candied), oatmeal, pop-tarts, pancakes, dry cereal, pizza, max 'n' cheese. PROTEINS: pork/sausage, steak, sausage, lunchmeat, eggs (scrambles, whites), go-gurt. VEGETABLES: squash, zucchini, carrots, corn. FRUITS: watermelon. Mother reported for Andre to have food jagged 16 foods. Children between the ages of 1.5 - 2 years are required to consume 1,000 - Β­1,400 calories per day to achieve adequate nutrition and are expected to consume 35 or more healthy foods across categories to gain the nutrients needed that are important for growth and development. Elianna's caloric daily intake is unknown; however, based on the information provided Elianna's current food inventory has limited protein, vegetable, fruits intake which is vital for growth and nutrition. FEEDING OBSERVATIONS: Elianna was seated in the highchair in a 90-90-90 degree. Non-preferred (beef-jerky) and preferred foods (crackers, flan) items were presented. Elianna took adequate bites from food when given maximum verbal prompts. During consumption of preferred foods, Elianna ate about 90%. When provided with verbal/visual prompts, Elianna interacted with the beef-jerky at the level of her finger pads and palm. When Elianna, was "all done" (ASL) with the food items she threw the food on the floor. Per parental report, feeding performance in the assessment was consistent with her performance at home. MOTOR SKILLS: During opportunity of oral intake of preferred food, Elianna overstuffed oral cavity which resulted in poor management of food. Elianna demonstrated a munching chewing pattern, chewing with mouth open, and no lateralization of tongue resulting in poor bolus formation. Elianna demonstrated overt clinical signs/symptoms of aspiration/penetration when consuming thin liquids via straw and soft solids (crackers). During 3/7 trials of thin liquid via straw and 2/15 trials of flan, Elianna presented with a 5 second delay. Parent reported that she typically doesn't coughs with thin liquids that are in a cup or straw. Based on her age/gender he should be doing the following during feeding: circular rotary chew, chewing with her lips closed, sweeping food from lips with tongue, consuming hard solids, pacing of thin liquids/soft solids with no overt s/s of aspiration/penetration. It should be noted that Elianna consumed soft foods which are easier to manipulate in oral cavity. Patient was not observed to consume regular solids resulting in motor skills not being completely assessed. Treating clinician to monitor motor skills when consuming food of other consistencies. Elianna presents with decreased oral motor skills and pharyngeal dysphagia resulting in poor safe and effective oral intake. RECOMMENDATIONS: Feeding therapy is highly recommended at this time, and should target increasing Elianna's tolerance to non-Β­preferred foods via steps to eating hierarchy and desensitization methods along with increasing oral motor skills. Parent education and mealtime behavior will also be targeted to support feeding development and alleviate the impacts of feeding challenges on the patient and her family. Without feeding therapy, safety risks include but are not limited to: lack of appropriate nutrition due to restricted diet characterized by foods of inappropriate nutritional value, food jagging on limited foods in food repertoire, and poor food management due to oral motor skills. Parents will be responsible for Home Management Intervention Program (HMIP). Due to Elianna's language background and input/output, the language of intervention will be English.

Feeding Assessment Summary: Elianna Estrada is a 2-year, 10-month-old child who was initially referred for feeding therapy due to concerns regarding limited food acceptance, oral motor delays, and feeding inefficiency in the context of cerebral palsy. Elianna is now due for a re-evaluation to determine continued medical necessity for skilled feeding therapy services. A comprehensive reassessment was completed using a bedside feeding evaluation, caregiver interview, and skilled mealtime observation to evaluate current feeding and swallowing skills, oral motor function, safety, and progress since the previous authorization period. Based on re-evaluation findings, Elianna continues to present with a CHRONIC PEDIATRIC FEEDING DISORDER (PFD) and ORAL PHASE DYSPHAGIA, characterized by impaired oral intake that is not developmentally appropriate for age. Contributing factors may include underlying medical diagnoses (e.g., cerebral palsy, brain damage, microcephaly, global developmental delays), nutritional concerns (e.g., food jagging), feeding skill deficits (e.g., sensory sensitivities, oral dysphagia), and psychosocial components (e.g., learned mealtime behaviors, dysregulated feeding responses). At this time, there are no overt clinical signs or symptoms of aspiration or penetration observed during feeding trials. During the re-evaluation, Elianna was observed consuming apple with skin, apple without skin, sausage (with skin), and liquids via straw (Capri Sun). Feeding occurred at the table, and Elianna demonstrated improved tolerance of this environment compared to prior sessions. When presented with apple with skin, Elianna demonstrated a bite-and-pull pattern on three occasions. She required continued visual and verbal modeling to prevent overstuffing, as she demonstrated a tendency to place additional food into her mouth before adequately chewing the initial bite. Elianna was unable to chew the apple skin effectively and appropriately spit out the skin, demonstrating emerging awareness of texture limits and safety. Based on this observation, apple without skin is recommended at this time. With sausage (with skin), Elianna demonstrated increased mastication time, an open-mouth chewing pattern, and inconsistent tongue lateralization throughout the chewing sequence. She demonstrated improved jaw movement and increased tongue involvement compared to the previous authorization period; however, bolus control remained inconsistent, with the bolus frequently held at midline on the tongue. Elianna required cues to continue chewing and intermittently requested juice to assist with bolus clearance. Overstuffing behaviors persisted, and Elianna became upset when prompted to spit food out. Additional observations included increased saliva production, drooling, and food holding, requiring external cues to resume chewing. When drinking liquids via straw, Elianna demonstrated rapid intake with audible swallows, five consecutive gulps, increased respiratory effort, and two coughing episodes. A pacing strategy of β€œthree sips and stop” was implemented, which improved coordination and reduced signs of incoordination. Oral motor assessment revealed continued deficits in labial closure, oral containment, and endurance, with emerging but inconsistent improvements in tongue lateralization and jaw control. Elianna demonstrated increased facial muscle activation compared to the prior authorization period, reflecting positive response to targeted interventions (e.g., applesauce via straw, lollipops for oral muscle engagement, bite-and-pull strategies). Although mastication efficiency has improved, fatigue during meals continues to impact chewing effectiveness, particularly with more complex textures such as foods with skins. Drooling persists and appears related to reduced labial seal, sensory awareness, and oral motor endurance rather than structural impairment. Since the previous authorization period, Elianna has demonstrated meaningful functional gains, including an expanded food repertoire with the addition of sausage (with and without skin) and other chewier meat textures, improved tolerance for seated mealtimes, and increased independence with utensil use and bringing food to her mouth. Additional improvements include enhanced postural stability, increased use of upper distal extremities during feeding, and improved oral muscle activation and control. Elianna has also demonstrated a reduction in phasic bite patterns and improved responsiveness to pacing strategies, supporting safer and more coordinated oral intake. Despite these gains, ongoing challenges persist, including reduced labial closure with associated drooling, continued overstuffing tendencies, feeding fatigue during dinner meals, and inconsistent bolus management, particularly with more complex textures such as foods with skins. These residual deficits continue to impact the safety, efficiency, and endurance of Elianna's oral intake and warrant continued skilled feeding intervention. Although Elianna has demonstrated measurable progress, she continues to present with a Chronic Pediatric Feeding Disorder (PFD) as defined by the ASPEN Consensus Framework, characterized by impaired oral intake that is not age-appropriate and associated with ongoing feeding skill dysfunction. Persistent deficits include oral motor inefficiency, reduced labial closure, drooling, impaired pacing, and difficulty managing developmentally appropriate textures, which continue to limit the safety, efficiency, and endurance of oral intake. Given Elianna’s underlying neuromotor diagnosis of cerebral palsy, these feeding challenges involve multiple PFD domains, including medical, feeding skill, nutritional, and psychosocial components. Without continued skilled intervention, Elianna remains at risk for feeding fatigue, nutritional imbalance, inefficient intake, and regression of feeding skills. Skilled feeding therapy is medically necessary and habilitative in nature, as ongoing treatment is required to support the development of oral motor strength, coordination, bolus management, pacing, and feeding endurance, and to promote safe, functional, and developmentally appropriate feeding across environments.
Feeding Progress to Date: The following are Elianna short-term goals and progress made during this authorization period: In 6 months, Elianna will accept at least 2 new protein-rich foods in 3 out of 10 opportunities during session or per parent report across 3 consecutive sessions, in order to support increased protein intake and expand dietary variety. (BASELINE: 0/10: CURRENT: 7/10 given maximum verbal/visual models; GOAL MET) In 6 months, Elianna will engage in sensory-based food play (e.g., using hands or utensils to scoop, smear, or manipulate various food textures) with at least 2 non-preferred foods in 3 out of 10 opportunities across 3 consecutive sessions, in order to support sensory regulation, reduce food-related anxiety, and build positive associations with novel foods. (BASELINE: 0/10: CURRENT: 10/10 given maximum verbal/visual models; GOAL MET) In 6 months, Elianna will use tongue lateralization to properly masticate preferred foods in order to promote age appropriate oral motor skills in 3 out of 10 trials across 3 consecutive sessions, in order to promote safe swallow consumption. (BASELINE: 2/10: CURRENT: 5/10 given maximum verbal/visual models; GOAL MET) In 6 months, Elianna will use a diagonal chew with a variety of foods in 3 out of 10 trials across 3 consecutive sessions, in order to promote a safe swallow, facilitate emerging chewing skills, and help expand the variety of foods the child can eat. (BASELINE: 1/10: CURRENT: 5/10 given maximum verbal/visual models; GOAL MET) In 6 months, Elianna will demonstrate functional oral motor sequencing by using a bite-and-pull motion followed by chewing food for at least 15–20 seconds before swallowing, in 3 out of 10 trials across 3 consecutive sessions, in order to reduce overstuffing and support coordination and endurance for safe bolus formation. (BASELINE: 1/10: CURRENT: 3/10 given maximum verbal/visual models; GOAL MET) When counseled by the clinician, parents will incorporate the home management intervention program as a daily and weekly activity to transfer skills learned in therapy into the home environment. The patient demonstrates significant impairments in gross motor development, including decreased strength, impaired balance, reduced coordination, limited mobility, decreased activity tolerance, and diminished kinesthetic awareness. These deficits contribute to an atypical gait pattern and inability to achieve age-appropriate motor milestones. Standardized assessment using the PDMS-2 reveals substantial developmental delays, with age equivalence of 18 months in stationary skills and 9 months in locomotion skills, indicating poor performance in both simple and complex motor tasks. These findings are consistent with severe delays in gross motor development. The patient’s impairments significantly limit her ability to participate in age-appropriate activities of daily living, play, and functional routines, and place her at risk for further developmental delay and long-term functional limitations. Skilled physical therapy services are medically necessary to address the identified deficits and promote optimal developmental progression. I hope you're doing well. I wanted to follow up regarding Elianna's feeding therapy. We are currently in the process of receiving authorization from your insurance company. I’d like to remind you that both speech and feeding therapy fall under the same category when filing insurance claims. Therefore, it is important that Elianna's ECI speech appointments and New Beginnings feeding appointments do not fall on the same day. If they do, the insurance company will only cover one of the services. As long as the appointments are scheduled on separate days, there should be no issues.
We also understand that ECI services are available for children until they turn 3. After Elianna graduates from ECI, we would be happy to continue supporting her therapy journey at New Beginnings if needed.

Any child or parent with a temperature of 100 degrees and above will be asked to reschedule for another day & time.
If you or your child or someone in your home has COVID, we continue to ask that you stay home and quarantine to stop the spread. The CDC urges people sick with symptoms of respiratory viruses to stay home until their fever has disappeared for at least 24 hours without relying on fever-lowering medications.

Please do not come to therapy if you or your child are experiencing the following symptoms: fever, body aches, chills, vomiting, diarrhea, headache. Your treating therapist would be happy to reschedule your visit for you.

I would like to go ahead and get her scheduled for the evaluation. Typically the evaluation is an hour long; however, I do recommend parents to arrive 15 minutes before the planned evaluation as there are quite a few documents that you will need to fill out or I can email them and you can print them out/fill them out and have them ready for the day of the evaluation. These are the days and times that I have available next week: Tuesday at 1:30 or Thursday at 10:30. Please let me know which of these times work best for you. 2) no major illnesses or accidents since last check up, but she did have surgery on her hip at the end of 2024 (left leg was not in hip socket) she is currently wearing a brace only at night, doctors gave the approval for Elianna to return to all forms of therapy, Fetal alcohol syndrome
3)She is delayed on all her developmental milestones
4) no breathing issues, normally sleeps throughout the night and usually takes an hour nap mid day, no enlarged tonsils
5) no medication
6) no preschool nor daycare
7) no vision nor hearing issues
8) Our concern with Elianna's Speech & Language skills is that she isn't really talking yet, strength's include her ability to repeat sounds and her determination
9)Elianna is receiving services from ECI for Physical Therapy, Occupational Therapy, Speech Therapy, & SST (cognitive therapy)

I was reading and a smaller brain doesn’t mean development stops. Development is about connections and learning, and she is clearly doing that. Even if her head size doesn’t change much, her brain can still develop skills. Brain growth and brain learning aren’t the same thing. Therapy works by helping the brain build new pathways and we’re seeing that happen.
