Speech Assessment Summary: Aziel is a 3-year, 1-month old male who is demonstrating a Severe Mixed Receptive and Expressive Language Disorder (F80.2). These deficits impact Aziel's ability to effectively communicate with family, peers, and community members, contributing to communication breakdowns and frustration. It is recommended Aziel receives services 2x/week for up to 60-minute sessions. Therapy should be conducted in patient’s dominant language. Parent is aware of the diagnosis and prognosis with and without treatment. Parents are also aware of New Beginnings Pediatric Therapy's attendance policy of not falling below 90% monthly attendance. If the patient does not demonstrate consistent, regular attendance in accordance with New Beginnings Pediatric Therapy's policy, they are subject to having speech therapy services reduced or placed on hold. Aziel is a strong candidate for speech therapy intervention. Parent understands the importance of carryover and the need for continuum of care within the home environment. A frequency of 2x/week for 6 months is recommended due to the Aziel's need for intervention in order to prevent regression and/or failure to retain newly learned skills. Telehealth services may also be provided as clinically appropriate and according to board rules. To not warrant services at this frequency will hinder the patient’s progress and development. Discharge date is to be determined based on the patient's progress towards his goals (criteria must be met for all goals) and demonstrating a standard score of 85 or higher on the most current standardized test. The overall goal is to demonstrate age appropriate (average) and/or functional speech and language skills. Medical necessity for skilled speech and language therapy services is indicated as Aziel is not able to clearly or effectively communicate his needs and wants for his personal health and safety. Unclear speech can prevent individuals from having their physical and psychosocial needs met, potentially leading to feelings of isolation, depression, and social difficulties. Therefore, early intervention is crucial to address these issues and reduce the need for more intensive, longterm services in the future. Developmental conditions refer to specific impairments that differ from the normal condition and meet the definition of medical necessity. Development is a natural state but when paired with disorder, disability or delay it indicates an abnormal state. A diagnosis of a severe speech impairment in a patient indicates an abnormal state of function, and speech language treatment services are as medically necessary for the patient as therapy are for an adult who has suffered a stroke and lost speech and language function. Impaired speech and language, loss of hearing, and swallowing difficulties all reflect deficit or loss of body functions and therefore services to treat such impairments meet the definition of medical necessity. Therefore, Aziel will be seen for skilled therapy services at the recommended frequency, however treatment will not begin until the referring physician has signed this plan of care and the insurance authorization is received. Once insurance authorization is approved, the patient will receive therapy services as recommended herein unless the frequency, duration or amount of therapy is changed by the payer or at the parent/patient’s request. All missed or cancelled visits will be made up in the current authorization period. Any changes to this treatment recommendation will be reported to the referring physician in the next progress summary or evaluation report whichever comes due first as per state/federal regulations. The Preschool Language Scales Fifth Edition (PLS5) is an individually administered instrument used to assess a child's receptive and expressive language skills from birth to age 7:11. The assessment identifies receptive and expressive language skills in the areas of attention, gesture, play, vocal development, basic concepts, language structure, integrative language, and literacy development. The PLS5 consists of the Auditory Comprehension standardized scale and the Expressive Communication standardized scale to obtain a Total Language Score and determine if the child is presenting with a language delay or disorder. Mother was already provided with options for other days and times by other treating therapist, which mother also denied. Treating therapist, Victoria, offered either telehealth to mother, however warned that he might not be a good candidate for that. Mother was also offered to do home health services. Waiting for a response.
02-05-2026	Victoria Chacon	Treating therapist was not notified of continuation and approval by insurance, therefore there has been a time lapse of 2 months in between authorizations. It is assumed that since there are two treating therapists, only one was informed. Therapist asked front office for status update on patient, and she was informed that he had auth dating back to December. Therapist quickly messaged mother of patient to inform her of continuation beginning Monday February 9th, however therapist has received no response.
11-26-2025	Avery Fontenot	ST Auth starting 12/15/2025 to 06/12/2026 for 2x/wk = 52 visits. Therapist notified.
11-03-2025	Victoria Chacon	This is being documented in case mother calls upset: Patient and mother saw therapist's previous patient walking out of the clinic with lollipop, and Aziel attempted to take it out of patient's hand. Mother stated "don't worry, you'll get yours later." Lollipops are given based on hardworking and good choices/behavior. Aziel was not given a lollipop due to maladaptive behaviors, and mother indirectly requested his candy after the session saying, "do you want a candy?" 2x. Therapist continued to explain how the session went, and how patient was crying, requires redirection and hand over hand assistance to complete tasks. As mother walked away she stated, "don't worry mommy will get you a candy," which communicated that she might have been upset with Aziel not receiving a candy.
09-24-2025	Avery Fontenot	Called mom regarding new times for her second ST session. Mom agreed to Wednesdays @5:30 with Rachael SLP.
09-23-2025	Lailani Chehedeh	I spoke with Mother regarding Saturday sessions. Mother informed me that she no longer wishes to continue with Saturday appointments. She stated that she prefers weekday sessions anytime after 5:30 PM due to Father’s deployment. Mother was informed that she would be notified if any weekday availability opens up with the speech therapists. She was also made aware that therapists would check with other providers regarding potential availability. Additionally, Mother was advised that weekday evening time slots are limited as they are often reserved for school-aged children.
08-04-2025	Victoria Chacon	Patient demonstrated difficulty throughout the 30 minute session as evidenced by crying, kicking toys and therapist, refusing to participate, taking off his shoes, and becoming increasingly more upset when therapist talks. Therapist followed up with mother about options for Aziel, based on the conversation that other treating therapist had with mother during Saturday session. Core boards and resources were provided to mom. Mother was informed that ABA is provided for children under the diagnosis of Autism, therefore she would be to speak with pediatrician about evaluation prior to requesting ABA referral. Mother stated they do not believe he has Autism, and therapist reminded mother that we cannot diagnose as SLPs, however we will write down the characteristics that are shown consistently throughout the 2 authorization periods of speech therapy. Mother was recommended to think about possibly moving OT to home setting to assist with the initial issues beginning at home.
06-16-2025	Victoria Chacon	Patient did not log into zoom, therefore parent was messaged by therapist, and mother stated "hey we're not home as yet. We might have to reschedule for in person." Mother was informed that this would be a no call no show.
01-28-2025	Teresa	Recommended frequency 2x/week of therapy; however, the mother verbalized she is only able to bring the patient on Mondays until the father is back in February. The new frequency of the patient's sessions will be 1x/week, 30 - 45 minutes.

Due to a recent change in Tricare’s process, all patients are now required to be seen by their Primary Care Provider (PCP/PCM) and have an updated referral submitted to Tricare before we can request authorization for therapy. Please note: -The referral end date provided by your PCP/PCM will determine the authorization period for therapy services. -If possible, we recommend requesting a referral period of up to one year, as this will help reduce the number of required follow-up visits. -If you are unsure whether a current referral is on file, please contact your PCP/PCM or Tricare Referral Management directly. This change began earlier this month. While we are actively reviewing all existing referrals, we kindly ask for your assistance in scheduling an appointment with your PCP/PCM and ensuring an updated referral is submitted to help avoid any delays in therapy services. If you are able to view your referral end date, please be aware that your child will need to be seen again before that date to continue services. We put on a little twinkly light with the lights off, which helped him regulate for a bit, and then he start crying again. Then I played Hickory Dickory Dock and it calmed him down and he went directly to look at he clock. But then after about 10 seconds he started crying again, so we took him to the sensory room with the colored lights. Gabriella was playing with a squishy ball with him, and he would push it away, but we would praise him as if he was intentionally rolling it to interact with Gabriella. Then I played some number songs, and that helped calm him down, too, and reduced his crying. He is still adjusting to us, I think, and so right now we are just working on building the rapport, so he is comfortable coming in with us. He didn't really want to play with the toys I brought out for him like bubbles, a ball/hammer toy, some colored counting bears toy, and a little sensory toy, so I'm hoping music or things related to numbers will help him

He engaged with the activities for 1–2 minutes at a time; however, when his attention decreased, he would throw items on the floor and either cry or drop to the ground. Our goals for the session included increasing his ability to request items, improving attention span, and following simple directions. I observed that much of his communication is currently behavior-based. To support him, we used a visual schedule to establish clear expectations and maintain consistency throughout the session. He became upset and cried for approximately five minutes when he saw a phone and was unable to access it. He was successfully redirected using preferred activities such as counting and singing.

He only cried when I didn't let him climb on the table or sink, and again when we had to leave. I used Lailani's Wheels on the Bus picture communication board, but he wasn't too interested in it today. But he was super smiley when I was singing, and thought it was funny when I would put things on my head. I tried to show him how to throw and catch a bouncy ball, but he wasn't interested. He liked jumping around and when I sang. But it was a much better session today :)

He loved both activities very much. We started using card talk with visuals to help with requesting and commenting along with sign language. We read brown bear bear and worked on labeling and identify animals. He played with a coin game were he labeled green, red, purple, and blue with verbal models. He also said clean up when he was ready to go and he said put in when working with the coins. Keep on modeling and talking to him because his words are coming !! :) Awesome work !

It help provided him with visuals and words to reduce frustration. We worked on "Water Open" phrase and him requesting the water with reducing crying and kicking behaviors. Next week we are closed for the long weekend of 4th of July so Saturday Zi will not have a session. However I have some openings this week Tuesday at 9am and Wednesday at 2pm ?

An option could be either telehealth while you work, which I think that would be too difficult for Zi, or I would recommend home health. We don't provide those in home services anymore, but there are other companies who will go to your house. I think that could be really beneficial for Zi, so it's in his comfortable environment.
