Mother Maria, was present in order to serve as the primary informant throughout the evaluation. Per mother's report child was born full term. Child did not experience any complications at birth, does not have any visual or hearing deficits. Mother reports difficulty with sustained attention, following simple steps, responding to her name, delayed milestones, sensitivity to sensory input such as noises, textures, clothing, foods and difficulty with self regulation skills.

Child demos difficulty with in-hand manipulation skills due to delayed finger dexterity, finger isolation and palm translation skills. Fine motor skills involved the small muscles of the body that enable such functions as writing, grasping small objects, and fastening clothing. Weaknesses in fine motor skills can affect a child's ability to eat, write legibly, use a computer, turn pages in a book, and perform personal care tasks such as dressing and grooming.

Assessment of Goals/Response to Treatment: Patient was alert, cooperative, and demonstrated full engagement throughout today's skilled occupational therapy session. Progress was noted in psychosocial skills, specifically related to self-control and social reciprocity. When presented with a challenging turn-taking activity with a novel peer, the patient initially demonstrated one instance of adverse behavior (tantrum). With therapeutic intervention, the patient responded to verbal cues to implement a taught coping strategy, which involved selecting a different toy while waiting for their turn. This strategy was effective, allowing the patient to de-escalate and successfully rejoin the activity, indicating emerging skill in frustration tolerance. Furthermore, the patient demonstrated notable gains in cognitive integration and visual processing skills by successfully completing a 12-step constructional task. The patient was able to follow visual instructions to form a magnetic submarine, requiring only minimal verbal cues for sequencing and problem-solving, which showcases improved sustained attention and the ability to match to sample with increased independence.

Please note: -The referral end date provided by your PCP/PCM will determine the authorization period for therapy services. -If possible, we recommend requesting a referral period of up to one year, as this will help reduce the number of required follow-up visits. -If you are unsure whether a current referral is on file, please contact your PCP/PCM or Tricare Referral Management directly. This change began earlier this month. While we are actively reviewing all existing referrals, we kindly ask for your assistance in scheduling an appointment with your PCP/PCM and ensuring an updated referral is submitted to help avoid any delays in therapy services. Her older sister has strep, and her younger sister has tested positive for two other viruses. As of now Lea has their symptoms ( wet cough, runny nose) but has yet to be tested so we don't know if she has Strep, or a virus. Either way she's sick and while she's acting fine I don't want to take her around other children and get them sick

We thought changing things up might give her more motivation. We made sure to stick to her punishment she's had no TV, she's still grounded from all her dresses, Tiaras, and bounce house. We worked alot on taking a breath when we're mad and saying " Azalea Luciana got this" it's worked very well. She wrote all her letters and an apology letter as well. Which she will give to you. I want to say that with the apology letter she was able to trace everything completely by herself at the table while I was in the kitchen.
