Occupational Background History: Santiago is a 2 year old boy who was seen on this date for a scheduled occupational therapy evaluation in order to obtain current level of function. Mother Sayra, was present in order to serve as the primary informant throughout the evaluation. Per mother's report child was born full term. Child did not experience any complications at birth, does not have any visual or hearing deficits. Mother reports Santi demonstrates developmental delays hindering communication, interaction with environment, cognitive skills, fine motor skills and overall delayed milestones.

Subjective: Patient was friendly and cooperative,

Assessment of Goals/Response to Treatment: The patient presented with a friendly and cooperative demeanor, engaging readily in the therapeutic session. Skilled intervention focused on enhancing psychosocial, cognitive, and motor skills through structured play activities. To address the goal of improved self-regulation during transitions, the patient participated in four activity changes. He demonstrated progress by successfully transitioning from a preferred to a non-preferred task on 1 of 4 occasions (25% accuracy) without dropping to the floor, achieved with moderate verbal cueing and light physical guidance at the elbow. This represents emerging progress toward the goal of accepting redirection with 50% accuracy. In addressing social play skills, the patient engaged in a 10-turn block stacking activity with the therapist. He required moderate verbal cues (e.g., "my turn," "your turn") and occasional physical prompting to wait for 4 of his 10 turns, and maximum hand-over-hand assistance for the remaining 6 turns to complete his action, showing a slight decrease in support from his baseline of requiring maximum assistance for all turns. Throughout these tasks, the patient also worked on underlying skills; he sustained attention for up to 3-4 minutes on a matching puzzle, requiring moderate assistance to orient and manipulate the pieces, thereby addressing in-hand manipulation and visual processing. His ability to initiate and terminate tasks is developing, though he continues to require external support to manage frustration and demonstrate self-control when challenged.

Due to a recent change in Tricare’s process, all patients are now required to be seen by their Primary Care Provider (PCP/PCM) and have an updated referral submitted to Tricare before we can request authorization for therapy. Please note: -The referral end date provided by your PCP/PCM will determine the authorization period for therapy services. -If possible, we recommend requesting a referral period of up to one year, as this will help reduce the number of required follow-up visits. -If you are unsure whether a current referral is on file, please contact your PCP/PCM or Tricare Referral Management directly. This change began earlier this month. While we are actively reviewing all existing referrals, we kindly ask for your assistance in scheduling an appointment with your PCP/PCM and ensuring an updated referral is submitted to help avoid any delays in therapy services. If you are able to view your referral end date, please be aware that your child will need to be seen again before that date to continue services.
