Speech Background History: Maximus came into facilities with father for an initial evaluation for speech and language. Father was the informant for the initial evaluation and gave pertinent information for sessions to target appropriate goals and objectives. Father reports that approximately two years ago, as the Pandemic had run its course, they felt the need to begin seeking outside help besides school. Maximus had been receiving speech and language therapy since birth. He has a repaired cleft palate that has skeletal anchors and rubber bands that he wears continuously. Father reports that Maximus also has a sensori-neural hearing loss with a hearing aid on the right ear and the use of a BAHA headband for the left ear for use with residual hearing. Dr. Amanda Chiao serves as Maximus' audiologist and New Beginnings will seek to have his audiogram sent for review. Because of various changes with employees through various therapy services for Maximus and the pandemic interfering with receiving services; continuous therapy has been difficult, but with some good gains for Maximus. Father would like Maximus to now continue with therapy as these last two years Maximus' speech has deteriorated and parents feel if the speech is fixed, Maximus can do better. Discussion took place as to mode of communication used at home. Oral and verbal needs in English and if Maximus watches videos or TV, it is done in English. Because father and mother work, and father is a Firefighter, Parents will self pay for services so therapy will take place once-a-week if Maximus qualifies.
Speech Medical History: Per father's report, Maximus was delivered vaginally with no complications, but was also born with cleft lip and palate and failed his new born hearing screening, and report indicating a sensori-neural/conductive hearing loss.

Speech Background Summary: Maximus is a very quiet but observant young boy that interacted with his father and found things to keep busy during interview with parent. Maximus appropriately gave social greetings and farewells. Also answered pertinent questions as to where his hearing aids were and what he did at school. He also said what he liked to do and within the Standardized Language Assessment, Maximus answered basic concept questions that included labeling, detail, and more than one word answers. During the assessment it is noted that parent felt Maximus was getting sick again, as he could not control his cough and had difficulty concentrating and answering due to the cough.

Maximus has a PROFOUND RECEPTIVE LANGUAGE DISORDER. Maximus had difficulty with naming items and using words for details. He also had difficulty concentrating as the cough Maximus had interfered with what he wanted to say. Speech Assessment Summary: Maximus is now 11 years 7 months old and in need of a review to note language and speech progress since beginning speech therapy six months ago at this facility. The Comprehensive Assessment of Spoken Language, Second Edition (CASL-2) was administered to assess Maximus's overall language abilities, specifically targeting receptive vocabulary, expressive vocabulary, and sentence expression. The CASL-2 evaluates a child’s ability to understand and use language across multiple domains, including word knowledge, sentence structure, and the ability to follow directions and answer questions. Max's results indicate significant challenges in expressive language. His receptive vocabulary was in the below-average range, with a standard score of 78 (raw score 44), suggesting difficulty understanding words in context, particularly with “wh” questions and two-step directions. His expressive vocabulary was considerably below average, with a standard score of 40 (raw score 17), and his sentence expression was also below average, with a standard score of 51 (raw score 12), reflecting difficulty generating sentences and formulating ideas verbally. Informal assessment of speech sounds indicated that /l/ and consonant clusters should be targeted, along with bilabials, /t/, /d/, and /s/. Additionally, a tongue tie was observed, which may impact articulation. Findings and recommended areas for intervention were reviewed and discussed with the parent. Max requires maximum redirection during the activities, he becomes easily distracted but can be redirected. Max is engaging in difficult tasks for longer periods of time without multiple melt downs. Melt downs are able to be redirected and we have been able to avoid complete "shut downs". His frustration has decreased which has allowed for longer periods of learning and teaching. Maximus has a SEVERE MIXED RECEPTIVE/EXPRESSIVE LANGUAGE DISORDER, secondary to cleft lip/palate and Hearing Loss in both ears. Maximus had difficulties with expression in grammatically correct sentences, word finding difficulties, and using words when needing assistance, making requests to repeat when he doesn't know, or even using words to advocate for self. It is recommended that Maximus' articulation be addressed through the speech therapy session with listening activities, word play, and language activities targeting specific sounds in words and phrases. At this time, Maximus' speech and language is more than 2 STANDARD DEVIATIONS BELOW that of an average language score on a standardized language test, and he DOES NOT have language and speech like that of his same-age peers. It is recommended that Maximus receive speech therapy at least 60 minutes a week due to SEVERE Receptive/Expressive Language Disorder, but due to the fact the parents are self-pay, it has been decided that Maximus will receive speech therapy 30-45 minutes once-a-week. Maximus' independence and safety risk are considered high, in that he is difficult to understand by an unfamiliar listener if needing to express the need for help or assistance. Maximus may not understand others as they are attempting to help through directions that may be too complex for Maximus. Speech therapy is considered to be habilitative at this time and in need of correction.

I will be conducting Maximus' speech and language evaluation. :) Before we schedule our in person appointment, I just need to gather some background information. So when you have an opportunity can you answer these questions for me: 1.) Any complications with pregnancy or birth? 2.) major illnesses or accident since last check up? Any medical diagnoses (i.e., Autism, ADHD, down syndrome, etc.)? 3.) developmental milestones met within normal limits? or delayed? 4.) sleeping or breathing issues? does patient get 8-10 hrs sleep? wake up several times a night? snoring? sleep with mouth open or closed? enlarged tonsils? 5.) medications? 6.) education? (i.e., day care, preschool) 7.) vision or hearing issues? 8.) In your a few words write your parental concerns with SPEECH and LANGUAGE along with your child’s strengths. 9.) when did your concerns begin? 10) Any history of previous therapy (i.e., speech, occupational therapy, physical therapy, counseling, etc.)

Sleeps about 9 hours per night. Does not wake up at night. Snores sometimes. Sleeps with mouth open at times. No enlarged tonsils. 5) No meds. 6) In 5th Grade SPED class. 7) Wears glasses, and wears a hearing aid on his right ear, can't hear from his left ear and uses a cochlear hearing device on his left side. 8) Concerned that his lack of speech is making him fall behind his academic milestones. He is very observant and will figure out how to do things quickly. 9) 3 years ago. 10) Speech therapy since 5 years old, he completed occupational therapy and no longer needs it.

Due to the cleft lip and palate, have you noticed any challenges with the way he pronounces sounds? Also, when was his hearing loss first identified? What kind of hearing loss was he diagnosed with? Does he work with an audiologist? Are there certain environments when when he struggles to hear or communicate effectively? Also, regarding his in person evaluation, would you all be available to come next Tuesday at 3:00 pm? :)

ASL? Verbal? AAC device? One more thing, we have a speech pathologist here who specializes in hearing loss. Would you prefer to be seen by her? I was speaking to her, and she stated that she would love to evaluate him, and if it is okay with you, I can sit in as well! :)

He is seeing an orthodontist right now and has skeletal anchors in place so he wears rubber bands almost 24/7 to help move his upper jaw forward. Yes he has trouble pronouncing his bilabial sounds. His hearing loss was also identified as a baby. Sensorineural hearing loss. His audiologist is Dr. Amanda Chiao. Haven't noticed any environments where he struggles to hear or communicate other than distracting places that catch his attention.

So I went to clarify for me, New Beginnings self-pay status. I can see Max for 30-45 minutes with the 30 minute payment rate that was quoted to you, or if you'd like the 45-60 minute payment rate that was quoted to you. The 15 minute interval is to establish Max into how comfortable he is with moving through the sessions. So if you would like to keep Max at the 30-45 minutes for the quoted amount you were given let's go with that at once-a-week. That way, we can do the 30 and if we need to spend an extra 15 minutes that will not cost you more. Is that alright with you? Also, we will be ready to start January 3rd, at 5PM and the following Thursdays in January, at 5:00PM. Please let me know when you have a moment if you are able to do that. We definitely appreciate you all taking the time to complete this at home, it is key to his progress. A couple of suggestions for and feedback for homework activities at home. Let’s keep it to 10-15 minutes maximum. We want Max to see this as something fun and exciting and not another task he has to compete. Have FUN with it! You guys mentioned he enjoys cooking and being in the kitchen, let’s use this to our advantage and give 1-2 step directions in this setting. For example, “Max get the egg from the fridge and put the pot on the stove.” This is also an opportunity to practice ( as well as for you to observe ) vocabulary he knows or doesn’t know with in his home environment. Examples: making pancakes, hot chocolate, s'mores, or helping out with dinner. I hope this helps. Please let me know if you have questions, don’t doubt in asking, we are here to help. 06-15-2025 04:14 PM	Cathlene Chapman	Good afternoon, I hope you are well this afternoon. I am following up to confirm how well Max did with his re-evaluation. As we had briefly discussed, Max improved on his understanding of vocabulary, and on his understanding of words associated with a picture; almost 20 points on the standardized language assessment. His improvement is still below his same-age peers, but he has shown six months worth of language growth in these six months of therapy. His Expressive Communication improved with a two point increase, with sentence expression emerging with more than two to three-word phrases. Definitely showing improvement when asked to use more words and he will do so; now we will work toward some more spontaneous correctly structured sentences. So his New Plan of Care includes much the same, increasing his knowledge of vocabulary under various components at a higher percentage. Using words and visuals in connection with each other to bring about some spontaneous communication and an even bigger percentage of details. We will also focus on more accurate structures of sentences in grammar and meaning.
