Father Zachery, was present in order to serve as the primary informant throughout the evaluation. Per father's report child was born full term. Child did not experience any complications at birth, does not have any visual or hearing deficits. Per father's report Mayah struggles with attention, redirection, memory recall, directions, and scholastic performance.

Occupational Background Summary: Mayah struggles with sustained attention demonstrating fleeing attention after approximately 25-30 seconds, requiring redirection to task, multiple repetitions and decreased accuracy with work. During the fine motor tasks Mayah demonstrates impulsive strokes and reduced accuracy due to rushing thru tasks. Father reports reduced performance at school which has also led to emotional regulation issues at school and home.
Occupational Diagnosis/Impressions: Mayah demonstrates cognitive delays hindering executive functioning skills, functional behavior and safety. Child also demonstrates difficulty with manual control skills, object manipulation, manual dexterity hindering fine motor precision and accuracy. Child will stop task due to difficulty processing what was asked limiting engagement. Child also demonstrates fine motor and grasp delays in BUE evident by premature grasp patterns, spillage and difficulty with manual dexterity tasks. Mayah also demonstrates deficits in sensory processing and self-regulation, which have impaired their ability to complete activities of daily living and instrumental activities of daily living that compromise their routines and roles at home and in the community. Child demonstrate medical necessity deficits that warrant skilled OT interventions in order to address their established delays and facilitate functional independence and performance. Subjective: Patient was friendly and cooperative,

Assessment of Goals/Response to Treatment: Patient presented with a cooperative and pleasant demeanor, demonstrating active engagement in today's session. Skilled occupational therapy intervention focused on enhancing psychosocial, cognitive, and visual processing skills. To address the goal of following multi-step instructions, the patient participated in a structured visual-motor activity requiring a 3-step sequence. The patient successfully completed the designated activity without attempting to alter the task in 2 out of 5 opportunities, requiring moderate verbal prompts to redirect focus and adhere to the established parameters. This demonstrates emerging progress in sustained attention and cognitive integration. Intervention also targeted self-regulation and body awareness within a simulated group context. When engaged in a turn-taking activity, the patient was able to maintain appropriate personal space for up to 3 consecutive minutes before requiring redirection. Moderate visual and verbal cues were provided on 3 of 5 occasions to remind the patient of appropriate body positioning, to which she responded positively. She continues to require moderate assistance to sustain this for the goal of 5 consecutive minutes, but shows improved awareness when provided with external cues. The patient benefits from the continued use of positive reinforcement to encourage on-task behavior and successful task completion.

Tricare is currently experiencing technical difficulties with their technology systems and initial and re-authorizations. As a result, there may be temporary delays in authorizations for your child’s therapy. Due to a recent change in Tricare’s process, all patients are now required to be seen by their Primary Care Provider (PCP/PCM) and have an updated referral submitted to Tricare before we can request authorization for therapy. Please note: -The referral end date provided by your PCP/PCM will determine the authorization period for therapy services. -If possible, we recommend requesting a referral period of up to one year, as this will help reduce the number of required follow-up visits. -If you are unsure whether a current referral is on file, please contact your PCP/PCM or Tricare Referral Management directly. This change began earlier this month. While we are actively reviewing all existing referrals, we kindly ask for your assistance in scheduling an appointment with your PCP/PCM and ensuring an updated referral is submitted to help avoid any delays in therapy services. If you are able to view your referral end date, please be aware that your child will need to be seen again before that date to continue services. Initially, Mayah responded well to a structured schedule and interventions focused on identifying and expressing her emotions. She has since demonstrated meaningful progress in these areas, effectively communicating her feelings through drawings, verbal expression, and writing. Given this progress, we have shifted our focus toward improving her ability to follow directions and instructions, with the goal of enhancing her attention and behavior while reducing impulsivity. From a clinical perspective, it appears that some of Mayah’s behaviors may be maintained by attention. To address this, we have implemented a strategy that minimizes reinforcing negative behaviors by withholding attention when inappropriate choices are made. Rather than responding with visible frustration or immediate consequences, I redirect her using neutral statements (e.g., “I thought we were going to have fun today”) and introduce non-preferred tasks or activities. This approach helps reduce the attention associated with undesired behaviors while encouraging her to pause and think before acting, as the outcome is no longer predictable. During today’s session, Mayah demonstrated the ability to follow instructions, regulate her behavior, and think before speaking or acting. We will continue to implement and refine this approach in therapy. I strongly encourage carrying over these strategies at home to support consistency and generalization of skills. They are requiring a new referral from PCP. Once we receive a new referral from the PCP we'll be able to resume services. It currently is not letting me schedule Mayah for tomorrow:(
