Occupational Background History: John is a 6.4 year old boy who was seen on this date for a scheduled occupational therapy evaluation in order to obtain current level of function. Mother Victoria and father John, were present in order to serve as the primary informant throughout the evaluation. Per mother's report child was born at 35 weeks gestation. Per mother's report John demonstrated delayed milestones and hypersensitivity to environmental factors. Mother reports John will cover ears or don headphones due to auditory aversion or many sounds. John also demonstrates difficulty with sustained attention, focus, memory recall, and overall executive functioning skills. Mother reports child requires assistance with daily roles and routines due to impulsivity and child forgetting information.

Occupational Background Summary: John was observed to pace room during evaluation, after 5-10 minutes he was able to sit down and engage in assessment. During assessment child was observed with impulsive strokes with limit fine motor accuracy and precision. Errors lead to frustration and limited self regulation. At times child is aware of errors made and would attempt to resolve. At this time child demonstrates sustained attention approximately 45-60 seconds before requiring redirection to task.
Occupational Diagnosis/Impressions: John demonstrates delays hindering executive functioning skills, attention, processing and overall functional behavior and safety. Child also demonstrates difficulty with manual control skills, object manipulation, manual dexterity hindering fine motor precision and accuracy. Child will stop task due to difficulty processing what was asked limiting engagement. Subjective: Patient was friendly and cooperative,

Assessment of Goals/Response to Treatment: The patient presented to the session with a friendly and cooperative disposition, demonstrating good engagement in therapeutic activities. Skilled intervention focused on a multi-step, structured sequencing and assembly task designed to address deficits in cognitive integration, visual processing, and psychosocial skills. The activity required the patient to utilize problem-solving to orient materials, sequencing to follow visual instructions, and sorting to organize components prior to assembly, which functionally targeted improvements in visual-motor integration and eye-hand coordination. Progress was noted toward the goal of improving attention and task completion. During today's session, the patient required moderate verbal and redirection cues (e.g., "Let's finish this step first," "Remember our plan is to complete one before we start the next") to maintain focus and inhibit the impulse to move on prematurely. With this level of support, the patient was able to see 1 of 3 presented activities (33%) through to completion, demonstrating emerging self-control and activity termination skills. This performance indicates progress from baseline but has not yet met the goal of 50% task completion, highlighting the continued need for skilled intervention to address deficits in coping skills, activity initiation/termination, and sustained attention.

Tricare is currently experiencing technical difficulties with their technology systems and initial and re-authorizations. Due to a recent change in Tricare’s process, all patients are now required to be seen by their Primary Care Provider (PCP/PCM) and have an updated referral submitted to Tricare before we can request authorization for therapy. Please note: -The referral end date provided by your PCP/PCM will determine the authorization period for therapy services. -If possible, we recommend requesting a referral period of up to one year, as this will help reduce the number of required follow-up visits. -If you are unsure whether a current referral is on file, please contact your PCP/PCM or Tricare Referral Management directly. This change began earlier this month. While we are actively reviewing all existing referrals, we kindly ask for your assistance in scheduling an appointment with your PCP/PCM and ensuring an updated referral is submitted to help avoid any delays in therapy services. If you are able to view your referral end date, please be aware that your child will need to be seen again before that date to continue services.
