Speech Background History: Xavier is a 9 year, 5 month old male who was referred by his pediatrician for a comprehensive speech and language evaluation due to limited speech skills. Mrs. Hurtado served as the informant for the evaluation. Prior to the evaluation, Mrs. Hurtado reported that at around the age of 4 or 5 Xavier was presenting with autistic characteristics but after further evaluation he was diagnosed with ADHD. Mrs. Hurtado reported that her primary concern is Xavier ability to be understood by others as he still presents difficulty with his speech sounds. He was evaluated using the Clinical Evaluation of Language Fundamentals-­5 Edition (CELF-5) and the Goldman-Fristoe Test of Articulation-3rd Edition (GFTA-3). Due to Xavier language background and input/output, the language of intervention will be English.
Speech Medical History: Mrs. Hurtado reported her pregnancy was unremarkable. She delivered at 41 wks no complications. As of now, Xavier continues to have no medical illnesses nor has had any major accidents.

The evaluation was administered via teletherapy. Mother was present during the evaluation. The Clinical Evaluation of Language Fundamentals-­5 Edition (CELF-5) was administered to gain information and standard scores through a standardized language assessment. Based on the scaled scores the following are his standard scores: Core Language Score 66 (sum of scaled scores: 16 (percentile rank 1). Xavier presents with a MODERATE to SEVERE mixed expressive/receptive language disorder when compared to same-aged peers. He would benefit from language intervention targeting the following weaknesses noted: recalling sentences, particularly those of increased complexity, and semantic relationships. Xavier's progress on articulation skills was assessed using the Goldman-Fristoe Test of Articulation - Third Edition (GFTA-3). Xavier obtained a raw score of 5 (number of errors) and a Standard Score of 79 (percentile 8) in Sounds-In-Words. We wanted to take a moment to keep you informed about an issue that may affect your experience with us. Tricare is currently experiencing technical difficulties with their technology systems and initial and re-authorizations. As a result, there may be temporary delays in authorizations for your child’s therapy.

Please know that we are closely monitoring the situation and working with Tricare to resolve the issue as quickly as possible. They have stated a need of 4-8 weeks for their system issues to be resolved as their subcontractor insurance switched from Healthnet Federal Services to Triwest Alliance on 01/01/2025. We sincerely apologize for any inconvenience this may cause and appreciate your understanding during this time.

Any child or parent with a temperature of 100 degrees and above will be asked to reschedule for another day & time.
If you or your child or someone in your home has COVID, we continue to ask that you stay home and quarantine to stop the spread. The CDC urges people sick with symptoms of respiratory viruses to stay home until their fever has disappeared for at least 24 hours without relying on fever-lowering medications.

Please do not come to therapy if you or your child are experiencing the following symptoms: fever, body aches, chills, vomiting, diarrhea, headache. Your treating therapist would be happy to reschedule your visit for you.

11-06-2024 09:28 PMXavier HinojoXavier (Prim) Replied: Xavier Derek Hinojo DOB: 6/3/15 .no complications he lasted 41 weeks . 3) no illness or accident he was diagnosed with ADHD and he is slightly delayed but when he takes his medications he try's his best. 4) he sleeps fine and through the night he goes to bed at 9 and wakes up at 6:30am but does sleep with his mouth open sometimes. 5 ) he takes focalin 6) he's in 4th grade and attends daycare after school 7) no issues with Vision or hearing 8) he's had a speech delay since he was 3 and developed adhd at the age of 4 or 5 that's when we noticed he needed to get checked. He showed red flags like he was almost autistic but they diagnosed him with ADHD. ... he is enrolled into the ARD program at school and he does speech therapy there he's been in speech since he was small like 5/6 years old. He still has a hard time with his speech but he try's and his strengths is he very determined and brave, outspoken and charming
