Speech Background History: Kolton is a 3-year, 2 month old male who was seen on the above date for an initial evaluation per referral of PCP. This evaluation was administered to assess Kolton 's current level of functioning and progress with therapy for the purpose of authorization for continued speech and language services. Mother was not in the therapy room during the evaluation however provided pertinent verbal and written information after the evaluation. On this day of the evaluation, Kolton appeared to be in good general health. Per parent report, English is the primary language of the home, therefore it is recommended that therapy be conducted in English.
Speech Medical History: Per parent report, medical history is not remarkable. Per mother's report, Kolton was born full term with no complications during pregnancy or delivery. He did not require a stay in the NICU. Mother reported she does not have concerns regarding Kolton's vision or hearing. No known allergies were reported.

Speech Background Summary: During the evaluation, Kolton demonstrated difficulty with flexibility when prompted to clean up a preferred item (i.e., refusing by saying "no", walking away from the working area). Mother stated that Kolton has difficulty with transitions at home and in his other therapies as well.
Speech Diagnosis/Impressions: Kolton is a 3-year, 2 month old male who is presenting with a SEVERE MIXED RECEPTIVE-EXPRESSIVE LANGUAGE DISORDER (ICD-10 CODE: F80.2). Kolton's receptive and expressive language was evaluated using the Developmental Assessment of Young Children Second Edition (DAYC2). Receptive and Expressive Language STRENGTHS consist of: turning his head and looking towards noise, briefly stops activity when name is called, responding with appropriate gestures to “up”, “bye-bye” or other routines, indicating yes or no (or appropriate head movement) in response to questions, points to 15 or more pictures of common objects when they are named, produces three or more consonants, produces string of consonant-vowel sounds, uses word for parent or caregiver discriminately, uses inflection patterns when vocalizing, has a word, sound or sign for “drink”, uses at least five words, says one word that conveys entire thought, can name familiar characters (based on preference), uses 10 to 15 words spontaneously, names eight or more pictures of familiar objects, uses at least 50 different words in spontaneous speech. Speech Assessment Summary: Kolton is a 4 year old male who was seen on the above date for an re-evaluation to determine his current level of functioning and if ST intervention continues to be warranted at this time. Mother reported that Kolton received an Autism diagnosis in July 2025 through his school. He is currently enrolled in a special education classroom. No other changes to his medical history or lifestyle were reported. Kolton appeared to be in good general health on the day of the evaluation. Kolton’s receptive and expressive language skills were assessed using the Preschool Language Scales, English, fifth edition (PLS-5). On the Auditory Comprehension subtest, Kolton received a Standard Score of 60 (raw score 28, percentile rank 1, age equivalent 2-1). On the Expressive Communication subtest, he received a Standard Score of 73 (raw score 33, percentile rank 4, age equivalent 2-7). Kolton received a Total Language Standard score of 64, indicating he is presenting with a SEVERE MIXED EXPRESSIVE LANGUAGE DISORDER. Kolton demonstrated STRENGTHS in receptive language with the following: identifies similar objects from a group of objects, identifies photographs of familiar objects, follows routine familiar directions with gestural cues, identifies basic body parts, identifies things you wear, understands the verbs eat, drink, and sleep in context, engages in pretend play, understands spatial concepts without gestural cues, and identifies colors. He demonstrated DIFFICULTY in receptive language with the following: follows commands with gestural cues, understands pronouns (me, my, your), follows commands without gestural cues, understands the use of objects, understands quantitative concepts, makes inferences, understands analogies, understands negatives in sentences, and understands sentences with post-noun elaboration. A child Kolton’s age should use 4+ words in phrases, have a vocabulary of 1,000+ words, respond to wh- questions across a variety of pragmatic functions, use present progressive –ing, use pronouns, use plurals, and comprehend basic concepts. Limitations in receptive abilities directly impact his ability to understand directions, grasp grammatical structures, and make associations for vocabulary growth. Limitations in expressive abilities directly impact his ability to communicate wants/needs clearly and participate in social exchanges with peers. Speech therapy is indicated and medically necessary to address these deficits and promote developmentally appropriate communication skills.

She stated that the next 6 months to a year will likely be especially challenging as she manages her medical appointments in addition to Kolton’s therapy schedule (Kolton currently attends OT with another provider). Clinician expressed support and reassured mother that scheduling accommodations can be made as needed. Mother was encouraged to keep clinician updated on any changes or adjustments during this time so that Kolton’s therapy can continue to be supported as best as possible.

Tricare is currently experiencing technical difficulties with their technology systems and initial and re-authorizations. As a result, there may be temporary delays in authorizations for your child’s therapy. They have stated a need of 4-8 weeks for their system issues to be resolved as their subcontractor insurance switched from Healthnet Federal Services to Triwest Alliance on 01/01/2025. Due to a recent change in Tricare’s process, all patients are now required to be seen by their Primary Care Provider (PCP/PCM) and have an updated referral submitted to Tricare before we can request authorization for therapy. Please note: -The referral end date provided by your PCP/PCM will determine the authorization period for therapy services. -If possible, we recommend requesting a referral period of up to one year, as this will help reduce the number of required follow-up visits. -If you are unsure whether a current referral is on file, please contact your PCP/PCM or Tricare Referral Management directly. This change began earlier this month. While we are actively reviewing all existing referrals, we kindly ask for your assistance in scheduling an appointment with your PCP/PCM and ensuring an updated referral is submitted to help avoid any delays in therapy services. If you are able to view your referral end date, please be aware that your child will need to be seen again before that date to continue services. We appreciate your patience and cooperation during this transition. If you have any questions, please don’t hesitate to contact our office. Thank you for your time and support.

My office manager mentioned that referrals for re-evaluations don’t expire like they do for initial evaluations, but we do recommend making sure patients have their yearly check-up since we need those wellness notes to submit with the evaluation report to insurance, it helps speed up the approval process. So go ahead and request his referral whenever it’s convenient for you. Thank you! :)
