Layla has had one ear infection. She has not experienced any major illnesses or accidents. Mother was provided a referral from Layla's pediatrician to Dr. Suchoff for an Autism/ADHD evaluation. Mother stated that she is concerned since Layla is "not talking enough," she spins, and "she climbs everything."

Speech Assessment Summary: Layla is a 3-year, 1-month-old bilingual (Spanish/English) female who was re-evaluated using the Preschool Language Scale, Fifth Edition – Spanish (PLS-5 Spanish) to determine her current level of functioning and continued need for skilled speech-language therapy. Layla carries a medical diagnosis of Autism Spectrum Disorder (ASD), confirmed in August 2025. She has been receiving speech-language services and has demonstrated noticeable progress, including increased use of three-word utterances and reduced jargon. Despite these gains, her mother reports that Layla continues to have significant difficulty expressing her wants and needs. When not understood, she often repeats the same word multiple times or exhibits frustration by crying, yelling, or throwing herself to the floor. Her mother further reports that Layla primarily communicates in English at home and requests that therapy continue in English, as it is now her dominant language. Layla’s medical history remains unchanged since her previous evaluation. The PLS-5 Spanish was administered to assess Layla’s receptive and expressive language abilities. It is important to note that the PLS-5 Spanish is not standardized on children with Autism Spectrum Disorder; therefore, results should be interpreted with caution and used in conjunction with clinical observation and parental input. The assessment was selected due to Layla’s limited verbal abilities and need for a measure appropriate for young children with emerging language. On the Auditory Comprehension subtest, Layla obtained a raw score of 39, a standard score of 85, and a percentile rank of 16, corresponding to an age equivalency of 3 years, 2 months, indicating receptive language skills within the average range. On the Expressive Communication subtest, she obtained a raw score of 28, a standard score of 78, and a percentile rank of 7, corresponding to an age equivalency of 2 years, 6 months, indicating a mild expressive language disorder. Expressively, Layla was able to use at least five words, engage in turn-taking routines, and extend toys or objects to share attention. However, she had difficulty producing multi-word combinations, using grammatical markers such as present progressive verbs (-ing), plurals, and possessive pronouns, and describing how objects are used. Although tested with Spanish materials, Layla primarily responded in English, consistent with her mother’s report that she now uses English more frequently at home. At three years of age, children are expected to speak in sentences of three to four words, answer simple “who,” “what,” and “where” questions, name familiar objects, and follow two-step directions without visual cues. They should also use grammatical forms such as plurals, -ing verbs, and pronouns, and be understood by familiar listeners most of the time. Layla’s current language skills fall below these expectations. She continues to rely on single words and short phrases, shows limited use of grammatical structures, and requires visual and verbal cues to comprehend directions. These deficits impact her ability to effectively communicate wants and needs, engage with peers and caregivers, and participate fully in her environment. Based on standardized results, clinical observations, and parental report, Layla presents with a MILD MIXED RECEPTIVE AND EXPRESSIVE LANGUAGE DISORDER SECONDARY TO AUTISM SPECTRUM DISORDER. Although she demonstrates emerging strengths, she continues to require skilled intervention to develop functional communication. Without ongoing speech-language therapy, Layla remains at risk for increased communication frustration, behavioral escalation, and further delays in expressive language and social communication development. It is medically necessary for Layla to continue receiving skilled speech-language therapy services provided by a licensed Speech-Language Pathologist (SLP). Therapy will focus on increasing expressive vocabulary and word combinations, improving use and understanding of grammatical structures (e.g., plurals, pronouns, and verb tenses), expanding comprehension of basic concepts, and enhancing functional communication to support emotional regulation and social engagement.
