A child Milah's age should have a vocabulary of 200 to 300 words, name most common objects, have an MLU of 1.5 to 2.0, use pronouns ME, MY, MINE, refer to self by name, use prepositions ON, IN, UNDER, understand what and where questions, understand actions words, identify family members, sort objects by shape and color. Milah was evaluated using the Preschool Language Scales (PLS¬5) and the Developmental Assessment of Young Children Second Edition (DAYC2). Speech Medical History: Milah was born via Emergency Cesarean Section due to drastic drop in both mother and Milah's heart rates. Mother reported being monitored closely throughout pregnancy due to having a miscarriage prior to this pregnancy. She added that she was in a car accident at 7 months of pregnancy, which caused her to go into pre labor for a couple of months.

Mother stated she was sad because her father is leaving, and continued to explain that he requested a divorce and her father (Milah's grandfather) kicked him out of the house, and mother began to cry. Milah began to show therapist a bump on her forehead, and when asked what happened, mother stated "she hit her head at home" and patient stated "my daddy hit me." Patient was asked a few more times in the clinic room away from mother and patient stated that father hit her, however also stated she fell at home. This is being documented to be monitored and reported.

So when you have an opportunity can you answer these questions for me: 1.) Any complications with pregnancy or birth? 2.) major illnesses or accident since last check up? Any medical diagnoses (i.e., Autism, ADHD, down syndrome, etc.)? 3.) developmental milestones met within normal limits? or delayed? 4.) sleeping or breathing issues? does patient get 8-10 hrs sleep? wake up several times a night? snoring? sleep with mouth open or closed? enlarged tonsils? 5.) medications? 6.) education? (i.e., day care, preschool) 7.) vision or hearing issues? 8.) In your a few words write your parental concerns with SPEECH and LANGUAGE along with your child’s strengths. 9.) when did your concerns begin? 10) Any history of previous therapy (i.e., speech, occupational therapy, physical therapy, counseling, etc.)

I was also in a car accident when I was 7 months pregnant with her and was said to be in pre labor with her for a couple of months. I had to have an emergency c-section after 16 hrs of labor because my heart rate dropped and milahs dropped drastically. 2)none 3)yes all her milestones are on track 4)milah snores a lot and sleeps with her mouth open 5)only Zyrtec for allergies given daily 6) she goes to my sisters for childcare 7)none 8)milah signs really well but I've noticed she gets frustrated with trying to make sentences out of words. She can tell me numbers, letter, and what things are in one word but cannot form full sentences. 9) my concerns began when we moved from Colorado down here to Texas 10)none
