Occupational Background History: Jameson is a 11 year old boy who was seen on this date for a scheduled occupational therapy evaluation in order to obtain current level of function. Mother, was present in order to serve as the primary informant throughout the evaluation. Per mother's report child was born full term. Child did experience a shoulder injury at birth which resolved quickly without extended hospitalization required. Mother reports history of difficulty with attention, executive functioning skills, socialization and anxiety. During the evaluation Jameson demonstrated limited eye contact, flat affect, spun in circles and engaged in self stimming behaviors. Child also required multiple directions to complete task with verbal and visual cues to redirect attention to task.

Occupational Background Summary: Jameson demonstrates difficulty with in-hand manipulation skills due to delayed finger dexterity, finger isolation and palm translation skills. Fine motor skills involved the small muscles of the body that enable such functions as writing, grasping small objects, and fastening clothing. Weaknesses in fine motor skills can affect a child's ability to eat, write legibly, use a computer, turn pages in a book, and perform personal care tasks such as dressing and grooming. During assessment child demonstrated a floating wrist, with very firm pressure limiting accuracy and precision. Child currently fatigues quickly during FM tasks due to forceful usage of hands. Child demonstrates limited hand control during object manipulation and spillage during transfer of objects.

Subjective: Patient completed all required tasks,Patient was alert and cooperative,

Assessment of Goals/Response to Treatment: Patient presented as alert and cooperative, demonstrating full engagement throughout the session and completing all therapeutic activities. Skilled intervention targeted psychosocial and cognitive integration skills through structured, turn-taking tasks. To address the goal of increased social participation, the patient engaged in a therapeutic game with the therapist, successfully maintaining reciprocal interaction and intermittent eye contact for 1.5 minutes with moderate verbal cues to sustain attention. This was achieved on 2 out of 4 opportunities, demonstrating emerging progress toward the goal of 3 minutes of sustained interaction. Furthermore, when an unexpected change was introduced by altering the rules of the game, the patient initially demonstrated frustration and disengaged from the activity. With moderate therapist support utilizing co-regulation strategies and verbal guidance, he was able to recover and re-engage with the modified task within approximately 3 minutes. This shows developing self-regulation skills, although he has not yet met the goal of recovering within 2 minutes. The patient continues to require skilled occupational therapy to advance his ability to initiate social engagement and utilize flexible coping strategies to improve participation in daily routines.

Tricare is currently experiencing technical difficulties with their technology systems and initial and re-authorizations. Due to a recent change in Tricare’s process, all patients are now required to be seen by their Primary Care Provider (PCP/PCM) and have an updated referral submitted to Tricare before we can request authorization for therapy. Please note: -The referral end date provided by your PCP/PCM will determine the authorization period for therapy services. -If possible, we recommend requesting a referral period of up to one year, as this will help reduce the number of required follow-up visits. -If you are unsure whether a current referral is on file, please contact your PCP/PCM or Tricare Referral Management directly. This change began earlier this month. While we are actively reviewing all existing referrals, we kindly ask for your assistance in scheduling an appointment with your PCP/PCM and ensuring an updated referral is submitted to help avoid any delays in therapy services. If you are able to view your referral end date, please be aware that your child will need to be seen again before that date to continue services.
