According to clinical observations, Aleyna’s communication is characterized as limited functional words, pointing/reaching, and whining/crying to communicate her wants/needs. Speech Assessment Summary: Aleyna is a 3 year 6 month old female who was due review of her progress and a re evaluation to determine her continued need for speech therapy services. The evaluation was performed in English, as that is the primary language spoken at home. Parents are happy with the progress that Aleyna has made over the course of therapy, stating she used to refuse to use her verbal language, and now she has built confidence and is more intelligible, however they continue to be concerned with increasing her intelligibility. The Clinical Assessment of Articulation and Phonology Second Edition (CAAP2)) was administered to assess Aleyna's articulation/phonological skills. She obtained a Consonant Inventory Score of 48 (number of errors) with a standard score of 64, resulting in a percentile rank of 2, which is more than 2.5 standard deviation below the mean. In the Consonant Inventory Subtest, Aleyna scored a total of 15 (total of errors), in Cluster reduction subtest, she scored a 10 (total number of errors), and on Multisyllabic subtest, she scored a 23 (total number of errors). The errors produced by Aleyna include: Initial word position: /g/ for /d/, /d/ for /g/, /w/ for /r/, /v/ for voiced "th", deletion of /g/, deletion of /s/, /f/ for /s/, /f/ for voiceless "th", /p/ for /g/ Medial word position: /t/ for /k/, deletion of /r/ Final word position: /k/ for /g/, dentalized /t/ for /d/, /t/ for /g/, dentalized /t/ for /g/, /n/ for "ng", /o/ for /l/, schwa for /r/, deletion of /z/, /t/ for /k/, /t/ for /g/, /t/ for /k/, /d/ for /?/, deletion of /t/, deletion of /l/. An oral mechanism examination was completed, and findings revealed the presence of a tongue tie. The patient’s tongue appears notably short, which is likely attributed to a restricted lingual frenulum. Although a previous dentist informed the family that a tongue tie was not present, the restriction is visibly evident during the evaluation. The patient was also previously seen by a pediatrician for concerns regarding jaw popping and a zig-zag mandibular movement during oral opening for specific sounds/words; however, the pediatrician reported this presentation as “normal” and stated she would grow into her jaw. Clinically, jaw popping and atypical mandibular movement patterns are not considered typical and warrant further investigation. Additionally, the patient presents with a wide oral opening, and her tonsils were not fully visible due to the tongue obstructing the view. The tonsillar tissue that was visible appeared red and irritated. Parents reported a history of allergies, and this will continue to be monitored. An ENT referral and a dental referral will be recommended to further assess and support these findings. Based on formal assessment, parent interview, and clinical observation, Aleyna presents with SEVERE ARTICULATION AND PHONOLOGICAL DISORDER. Speech therapy is recommended at this time, as Aleyna does not communicate like other children her age. Her safety and functional health are at risk, as well as her independence in her daily routine. As her intelligibility with unfamiliar listeners prevents her from intelligibly communicating information and details of pain or help she may need.
