Mother Shannon, was present in order to serve as the primary informant throughout the evaluation. Per mother's report child was born full term. Child did not experience any complications at birth, does not have any visual or hearing deficits. Child does have history of aggression and defiant behavior. Child also demonstrates difficulty with manual control skills, object manipulation, manual dexterity hindering fine motor precision and accuracy. Child will stop task due to difficulty processing what was asked limiting engagement. Child also demonstrates fine motor and grasp delays in BUE evident by premature grasp patterns, spillage and difficulty with manual dexterity tasks. Jaeda also demonstrates deficits in sensory processing and self-regulation, which have impaired their ability to complete activities of daily living and instrumental activities of daily living that compromise their routines and roles at home and in the community. Child demonstrate medical necessity deficits that warrant skilled OT interventions in order to address their established delays and facilitate functional independence and performance. Home programming alone is not sufficient to make the needed progress, direct one to one services are required for progress to be achieved. Assessment of Goals/Response to Treatment: Patient was cooperative and actively engaged in all therapeutic activities during today's skilled occupational therapy session, which focused on advancing psychosocial skills such as coping, self-control, and social reciprocity. To address goals for self-control and sustained attention, the patient participated in structured tasks and successfully maintained focus for a 2-minute interval, requiring only minimal verbal cues for redirection. This represents significant progress from a baseline of less than 1 minute and demonstrates a positive trajectory toward the goal of achieving 3 minutes of sustained attention. Skilled intervention also targeted social reciprocity and coping skills through a sensory-based activity where the patient demonstrated developing emotional awareness by correctly identifying 3 out of 5 presented emotions with moderate verbal and gestural prompts. This is a marked improvement from a baseline requiring maximum assistance and is foundational for improving social understanding and self-regulation. As always, we at New Beginnings strive to keep our patients, parents, therapists, and staff safe and healthy. We have thermometers in our front lobbies and will be checking for high temperatures as we begin the traditional time of year for an increase in flu, colds, and COVID. Any child or parent with a temperature of 100 degrees and above will be asked to reschedule for another day & time. If you or your child or someone in your home has COVID, we continue to ask that you stay home and quarantine to stop the spread. The CDC urges people sick with symptoms of respiratory viruses to stay home until their fever has disappeared for at least 24 hours without relying on fever-lowering medications.
