Speech Background History: Leonardo is a 2-year 5-month-old male who was referred by his pediatrician for a comprehensive Speech and Language evaluation. The evaluation was completed in Spanish and English, as those are the primary languages spoken in the home. Currently, Leonard stays with mother and does not attend school or daycare. Mother reported concerns that Leonardo has a bit of a delay and only has a couple of words and has seen a regression in skills. Mother reported Leonardo was recently evaluated and diagnosed with Autism Spectrum Disorder January 2024. He is also pending audiological evaluation.

Speech Assessment Summary: Leo is a 4-year, 4-month-old male with a primary diagnosis of Autism Spectrum Disorder who is being discharged from speech therapy services at this time due to significant progress and achievement of functional communication skills within age-appropriate expectations. Since the initiation of therapy, Leo has demonstrated exceptional growth in his speech and language abilities. At the start of services, Leo presented as a minimally verbal communicator; however, he now communicates effectively across a variety of settings. He is able to answer questions appropriately, express his wants and needs, protest using language, and engage in conversational turn-taking. Leo is now consistently using full sentences and actively participates in interactions with others. Caregiver report indicates that Leo has demonstrated similar progress in the home environment and is increasingly using sentences more consistently in his daily communication. The Preschool Language Scales–Fifth Edition (PLS-5) was administered to assess Leo’s current receptive and expressive language skills. On the Auditory Comprehension subtest, Leo obtained a raw score of 42 and a standard score of 88, which falls within normal limits, indicating age-appropriate receptive language skills. He demonstrates the ability to understand language, follow directions, and comprehend questions appropriately. On the Expressive Communication subtest, Leo obtained a raw score of 37 and a standard score of 80, reflecting functional expressive language skills. He is able to answer questions using full sentences and engage in back-and-forth conversation. Mild areas of difficulty remain with the use of pronouns and responding to “where” questions; however, these skills are expected to continue developing with ongoing exposure in natural environments and school settings. In regard to articulation, Leo presents with a frontal lisp characterized by forward tongue placement during sound production. At this time, Leo demonstrates awareness of correct tongue placement and is stimulable for accurate production of targeted sounds. A home exercise program (HEP) will be provided to support continued practice at home, and caregiver involvement has been strong, with mother actively supporting his communication development. Based on Leo’s progress and current level of functioning, discharge from skilled speech therapy services is recommended at this time. Continued support through daily language exposure, reading activities, and modeling of appropriate grammar (including pronouns and “where” questions) is encouraged. If any concerns regarding Leo’s speech or language development arise in the future, it is recommended that the family contact their pediatrician to obtain a referral for a follow-up speech and language evaluation in approximately six months. Continued monitoring of his communication skills is encouraged as he grows and is exposed to more complex language demands. Early re-evaluation can help ensure that Leo continues to progress and receives support if needed.

Assessment of Goals/Response to Treatment: The patient presented as alert and cooperative, demonstrating excellent engagement in all therapeutic activities. Session focused on articulatory precision and receptive language comprehension. For the objective of producing the /s/ phoneme at the word level, the patient achieved 20% accuracy across 20 trials. Productions were often characterized by a frontalized placement, requiring moderate verbal and tactile cues to facilitate correct tongue positioning behind the teeth. This performance indicates the skill remains in the early acquisition phase, with continued intervention warranted to improve accuracy and reduce cueing dependency toward the long-term goal of 70%. For the receptive language goal of understanding basic analogies, the patient demonstrated comprehension with 30% accuracy given a field of three visual choices. Success was noted on familiar, concrete relationships (e.g., "A bird is to fly as a fish is to swim"), but he required moderate verbal supports, such as sentence stems and explicit explanations of the semantic relationship, for less common pairings. We have thermometers in our front lobbies and will be checking for high temperatures as we begin the traditional time of year for an increase in flu, colds, and COVID. Tricare is currently experiencing technical difficulties with their technology systems and initial and re-authorizations. Dad was telling me he's been having some behaviors at home so I was telling him that its important to set those boundaries with him because he is starting to test to see what he can get away with and what he cant!

For example, when we leave the house it's always a hassle because he want to run and not get in the car, he will kick, yell, hold on to the door etc

I told dad too that from now on we will walk staright to the car and he wont get his lolipop until he is seated in the car seat cause last week he tried to run on dad. Another fun thing you can try to incorporate is red light green light so he knows he needs to stop when you guys say red light and go when its green light! Start at a room in the house and stop and the door way until you guys get to the front and then hit red light okay grab my hand green light run to the car and do red light to stop! Does that make sense?

I wasn't sure on what to do and I just want to set him up for success I think we are going to go ahead and stop the sessions after lots of reflection I think he will continue to improve and be where he needs to be.

I just want to share that Leonardo has made so much progress since starting therapy with us. When he first came in, he was not using words to communicate, and now he is consistently using words and continuing to increase both his receptive and expressive language skills, which is wonderful to see. We are really proud of the progress he has made. Of course, if at any point in the future you begin to have concerns again, you are always welcome to return after the 6-month period, and we would be more than happy to see him again and support him however needed. Also, I wanted to ask if you might still be interested in coming in this Monday so we can celebrate all the hard work and progress Leonardo has made. We would love the opportunity to celebrate him before discharge! 😊
