Robert's Receptive Language raw score was 10 with a Standard Deviation of 73, age equivalent of 10 mo and percentile rank of 4. His Expressive Language Raw Score was 9 with a Standard Score of 73, age equivalent of 8 mo and percentile rank of 4. His Communication Raw Score (sum of raw scores) was 19 with a Standard Score of 74, age equivalent of 9 mo, and a percentile rank of 4. These scores are 2 standard deviation below the mean compared to his same aged peers, and are indicating a MODERATE TO SEVERE Mixed Expressive and Receptive Language Disorder. Robert's expressive and receptive language strengths consisted of: smiling at a person who is talking or gesturing, turning and looking towards noises, briefly stopping activity when his name is called, responding with appropriate gestures such as "high five", moving his body to music, briefly stopping when told "no," producing three or more single vowel sounds (e.g., ah, eh, uh), laughing out loud, producing three or more consonants such as /b/, /m/, /d/, and using word for parent (e.g., ma). Speech Assessment Summary: Robert is a 3 year, 6 month old male who was due or a review of his progress and a re evaluation to determine his continued need for speech therapy services. Robert's mother provided pertinent information during the evaluation, which is later referenced in this report. Mother is content with the progress Robert has made over the course of the authorization period, and stated that Robert is using verbal language to communicate wants and needs. She stated that she has observed an increase in verbal language, however he is difficult to understand and pronouns sounds incorrectly. She added that he will begin school in the Fall of 2026. Robert was evaluated using the Preschool Language Scales Fifth Edition (PLS5). The language of the evaluation was English, and was determined based on parent report of English exposure via media (YouTube videos, games, and TV), and the primary language spoken at home. The Preschool Language Scales Fifth Edition (PLS5) used to assess Robert's Expressive Language skills. He received a Receptive Language Raw Score of 37 with a Standard Score of 84 and a Percentile Rank of 14. His Expressive Language Raw Score of 33 with a Standard Score of 80 and a Percentile Rank of 9. His Total Language Score was 164 with a Standard Score of 81 and a Percentile Rank of 10. These score indicate a MODERATE EXPRESSIVE LANGUAGE DISORDER. His Receptive and Expressive Strengths include: understanding spatial concepts (in, on, out of, off) without gestural cues, understanding quantitative concepts (one, all), making inferences, understanding analogies, understanding sentences with post noun elaboration, understanding spatial concepts (under, in back of, next to), naming objects in photographs, using words more often than gestures to communicate, using words for a variety of pragmatic functions, using different word combinations, naming a variety of pictured objects, using a variety of nouns, verbs, modifiers and pronouns in spontaneous speech, using present progressive verbs, and naming a described objects. Due to a recent change in Tricare’s process, all patients are now required to be seen by their Primary Care Provider (PCP/PCM) and have an updated referral submitted to Tricare before we can request authorization for therapy. Please note: -The referral end date provided by your PCP/PCM will determine the authorization period for therapy services. -If possible, we recommend requesting a referral period of up to one year, as this will help reduce the number of required follow-up visits. -If you are unsure whether a current referral is on file, please contact your PCP/PCM or Tricare Referral Management directly. This change began earlier this month. While we are actively reviewing all existing referrals, we kindly ask for your assistance in scheduling an appointment with your PCP/PCM and ensuring an updated referral is submitted to help avoid any delays in therapy services. If you are able to view your referral end date, please be aware that your child will need to be seen again before that date to continue services. In your a few words write your parental concerns with SPEECH and LANGUAGE along with your child’s strengths. 9.) when did your concerns begin? 10) Any history of previous therapy (i.e., speech, occupational therapy, physical therapy, counseling, etc.)
