Speech Background History: Zaiden is a 7 year, 9 month old child who was referred for a speech and language evaluation. His father served as the informant for this evaluation. Speech Assessment Summary: Zaiden is a 9-year, 7-month old male who was seen on the above date for a re-evaluation to determine current level of functioning and if ST intervention continues to be warranted at this time. Per parent report, there have been no changes to Zaidens medical history or lifestyle since his previous evaluation. Zaiden attends a general education classroom where he receives instruction in English. Zaiden's home language is English and the movies or videos he sees are all in English. Zaiden was administered four tests of the Clinical Evaluation of Language Fundamentals®-Fifth Edition (CELF®-5) from which his Core Language Score was derived. The Core Language Score is a measure of general language ability and provides an easy and reliable way to quantify Zaiden’s overall language performance. The Core Language Score has a mean of 100 and a standard deviation of 15. A score of 100 on this scale represents the performance of the typical student of a given age. Word Classes The Word Classes test is used to evaluate the student's ability to understand relationships between words based on meaning features, function, or place or time of occurrence. The student chooses the two words (i.e., pictures or presented orally) that best represent the desired relationship. Zaiden received a scaled score of 5 (confidence interval = 3 to 7, percentile rank = 5) on the Word Classes test Strengths: semantic class, location, object function Weaknesses: composition, synonyms, word opposites. Formulated Sentences The Formulated Sentences test is used to evaluate the ability to formulate simple, compound, and complex sentences when given grammatical (semantic and syntactic) constraints. The student is asked to formulate a sentence, using target word(s) while using an illustration as a reference. This is a general message to all parents, do not text this number to reply. Happy New Year from Team New Beginnings! To start the new year right, we are sending some reminders of our policies.



1) Please make sure to inform your front office of any changes to your a) contact phone number, b) your address, c) your insurance, d) your pediatrician. We cannot serve you to the best of our ability if you do not inform us of changes to your personal information in a timely manner. Not informing us of changes may result in delayed authorization.



2) New Beginnings is not a licensed daycare, as such, please be advised that you cannot leave your child in therapy alone. You must remain on premises at all times should your child need assistance with toileting, behavior, or feeding. When you signed our paperwork at the initiation of therapy, you signed an understanding of what our policies and procedures are. Any parent who has a pattern of leaving our premises while their child is in therapy, unfortunately, will be subject to discharge.



3) We have a courtesy HOLD of 30 days and no more whether for change in insurance or any other extenuating circumstance. All 30-day HOLDs must be cleared with the front office and the Clinic Director.



4) We adhere strictly to our 90% attendance policy. If your child is not in therapy, they cannot learn and make progress. Please work with your treating therapist to do all makeups and keep your attendance up. We are here to help your child and cannot do so if they have poor attendance.

5) All cancellations or rescheduling of visits should be made with as much notice as possible. We ask for 24 hours in advance notice when you know you cannot make a visit.

As always, we at New Beginnings strive to keep our patients, parents, therapists, and staff safe and healthy. We have thermometers in our front lobbies and will be checking for high temperatures as we begin the traditional time of year for an increase in flu, colds, and COVID. Any child or parent with a temperature of 100 degrees and above will be asked to reschedule for another day & time.
If you or your child or someone in your home has COVID, we continue to ask that you stay home and quarantine to stop the spread. The CDC urges people sick with symptoms of respiratory viruses to stay home until their fever has disappeared for at least 24 hours without relying on fever-lowering medications.

Please do not come to therapy if you or your child are experiencing the following symptoms: fever, body aches, chills, vomiting, diarrhea, headache. Your treating therapist would be happy to reschedule your visit for you.

We wanted to take a moment to keep you informed about an issue that may affect your experience with us. Tricare is currently experiencing technical difficulties with their technology systems and initial and re-authorizations. As a result, there may be temporary delays in authorizations for your child’s therapy.

Please know that we are closely monitoring the situation and working with Tricare to resolve the issue as quickly as possible. They have stated a need of 4-8 weeks for their system issues to be resolved as their subcontractor insurance switched from Healthnet Federal Services to Triwest Alliance on 01/01/2025. We sincerely apologize for any inconvenience this may cause and appreciate your understanding during this time.

Our licensed pediatric physical therapist, with close to 30 years of experience, is here to support your child’s movement, strength, balance, and coordination needs. All patients will need a new referral for PT from their pediatrician, and an initial evaluation by our PT.

As always, we at New Beginnings strive to keep our patients, parents, therapists, and staff safe and healthy. We have thermometers in our front lobbies and will be checking for high temperatures as we begin the traditional time of year for an increase in flu, colds, and COVID. Any child or parent with a temperature of 100 degrees and above will be asked to reschedule for another day & time.
If you or your child or someone in your home has COVID, we continue to ask that you stay home and quarantine to stop the spread. The CDC urges people sick with symptoms of respiratory viruses to stay home until their fever has disappeared for at least 24 hours without relying on fever-lowering medications.

Please do not come to therapy if you or your child are experiencing the following symptoms: fever, body aches, chills, vomiting, diarrhea, headache. Your treating therapist would be happy to reschedule your visit for you.

We would like to inform you of an important update regarding therapy services. Due to a recent change in Tricare’s process, all patients are now required to be seen by their Primary Care Provider (PCP/PCM) and have an updated referral submitted to Tricare before we can request authorization for therapy.

Please note:
-The referral end date provided by your PCP/PCM will determine the authorization period for therapy services.
-If possible, we recommend requesting a referral period of up to one year, as this will help reduce the number of required follow-up visits.
-If you are unsure whether a current referral is on file, please contact your PCP/PCM or Tricare Referral Management directly.

This change began earlier this month. While we are actively reviewing all existing referrals, we kindly ask for your assistance in scheduling an appointment with your PCP/PCM and ensuring an updated referral is submitted to help avoid any delays in therapy services.

If you are able to view your referral end date, please be aware that your child will need to be seen again before that date to continue services.

We appreciate your patience and cooperation during this transition. If you have any questions, please don’t hesitate to contact our office.

He did well, he needed two choices in several questions, I am hoping to fade off the cues, and have him remember that "when" is time and "why" is correlated to the reason, given examples. We also worked on problem-solving when given social scenarios, and he answered correctly when given two choices in some trials, again, I am working on more independent answers from him!
