Speech Background History: Valentina is a 3 year 6-month-old female who was referred by her pediatrician for a comprehensive speech and language evaluation due to limited language skills. English is the primary language spoken at home. Valentina's mother, Mrs. Arriaga served as the informant for this evaluation. Mrs. Arriaga reports Valentina's communicates her wants and needs using 3-4 word utterances, however unfamiliar listeners struggle to understand her. Valentina's pediatrician is reported to have had concerns of her intelligibility. This break in his communication often has Valentina frustrated having to repeat herself or requiring a caregiver to infer her message. A child Valentina's age should be 50%-75% intelligible.
Speech Medical History: Mrs. Arriaga reported her pregnancy was unremarkable excluding being diagnosed with preeclampsia her 3rd trimester. She had a vaginal delivery at 37 wks with no overt complications. Valentina was reported to have met all developmental milestones excluding speech. As of now, Valentina was diagnosed with astigmatism and is awaiting eye wear.

Speech Assessment Summary: Valentina is a 5 year 11 month old female who was due review of her progress and a re evaluation to determine her continued need for speech therapy services. The evaluation was performed in English, as that is the primary language spoken at home. Parents are happy with the progress that Valentina has made over the course of therapy. The patient has demonstrated significant progress in articulation, and her most recent standardized testing reflected average-range abilities, with Standard Scores of 105 and 109. However, during conversational and spontaneous speech, she continues to present with episodes of reduced intelligibility due to consistent articulation errors, such as substituting /f/ for voiceless “th” (e.g., with) and /v/ for voiced “th” (e.g., that), as well as excessive saliva buildup in the oral cavity. Although strategies to reduce saliva through increased swallowing have been implemented, she demonstrates limited awareness of oral secretions and requires frequent reminders both in the clinic and at home. This ongoing issue continues to negatively impact her overall intelligibility in functional communication. The Goldman Fristoe Test of Articulation Third Edition (GFTA3) was administered to assess Valentina's articulation/phonological skills. She obtained a Total Raw Score for Sounds in Words of 2 (number of errors) with a standard score of 105, resulting in a percentile rank of 63, which is within the normal limits compared to peers her age. Her Total Raw Score for Sounds in Sentences was 1 (number of errors) with a Standard Score of 109 and Percentile Rank of 73, which is within normal limits when compared to her same aged peers. A conversational sample was taken to assess her spontaneous productions of sounds, and she demonstrated the following errors: /f/ for voiceless "th", and /d/ for voiced "th", and /v/ for voiced "th". These are consistent errors and although she is scoring within normal limits in the standardized assessment, her skills have not generalized to conversation. Continued medically necessary speech therapy is recommended due to the patient’s ongoing impairment in functional intelligibility, which directly affects her ability to communicate effectively across environments. Although she has demonstrated progress on standardized assessments (Standard Scores of 105 and 109), she continues to present with consistent articulation errors, including substitutions of /f/ for voiceless “th” and /v/ for voiced “th,” as well as reduced oral-motor awareness resulting in saliva pooling, all of which significantly decrease intelligibility in conversational speech. These deficits are not age-appropriate and continue to interfere with her ability to express needs, participate in academic tasks, and be understood by unfamiliar listeners. Continued therapy will target accurate production of “th” sounds at all levels, generalization of correct productions into spontaneous speech, and improvement of oral-secretion management through structured cues and compensatory strategies. Ongoing skilled intervention remains necessary to ensure measurable functional improvement and to prevent regression in communication abilities. To start the new year right, we are sending some reminders of our policies. 1) Please make sure to inform your front office of any changes to your a) contact phone number, b) your address, c) your insurance, d) your pediatrician. We cannot serve you to the best of our ability if you do not inform us of changes to your personal information in a timely manner. Not informing us of changes may result in delayed authorization. 2) New Beginnings is not a licensed daycare, as such, please be advised that you cannot leave your child in therapy alone. You must remain on premises at all times should your child need assistance with toileting, behavior, or feeding. When you signed our paperwork at the initiation of therapy, you signed an understanding of what our policies and procedures are. Any parent who has a pattern of leaving our premises while their child is in therapy, unfortunately, will be subject to discharge. 3) We have a courtesy HOLD of 30 days and no more whether for change in insurance or any other extenuating circumstance. All 30-day HOLDs must be cleared with the front office and the Clinic Director. 4) We adhere strictly to our 90% attendance policy. If your child is not in therapy, they cannot learn and make progress. Please work with your treating therapist to do all makeups and keep your attendance up. We are here to help your child and cannot do so if they have poor attendance. 5) All cancellations or rescheduling of visits should be made with as much notice as possible. We ask for 24 hours in advance notice when you know you cannot make a visit. Should you have any questions, please feel free to call your corresponding office or TEXT your treating therapist. Due to a recent change in Tricare’s process, all patients are now required to be seen by their Primary Care Provider (PCP/PCM) and have an updated referral submitted to Tricare before we can request authorization for therapy. Please note: -The referral end date provided by your PCP/PCM will determine the authorization period for therapy services. -If possible, we recommend requesting a referral period of up to one year, as this will help reduce the number of required follow-up visits. -If you are unsure whether a current referral is on file, please contact your PCP/PCM or Tricare Referral Management directly. This change began earlier this month. While we are actively reviewing all existing referrals, we kindly ask for your assistance in scheduling an appointment with your PCP/PCM and ensuring an updated referral is submitted to help avoid any delays in therapy services. If you are able to view your referral end date, please be aware that your child will need to be seen again before that date to continue services. 05-02-2024 12:12 PM	Victoria Chacon	Okay so right now the sounds that I'm working on with Valentina are 1) sounds that are similar and produced in the same area of the mouth like p and b, s and z, f and v, k and g, and t and d sounds. Sometimes she will use a sound in place of the other. For example for "zebra" she might say "sebra", so I have to remind her to "turn on her voice" for those sounds (z, b, d, v, g). 2) we're working on multi-syllable words because at times she will delete a portion of the word. More specifically she will not pronounce t or d sounds that are in the middle of the word such as "butter" or "water." 3) we are working on her L sounds. We are working on perfecting the correct placement of that sound, so we practice saying just the sound L makes, then we add a vowel to work on syllables like "la, lo, li, lu, lay" and also very simple words that start with L such as "log, lip, lion." 4) We have been working on words that have consonants next to each other such as "SNake, FLower, SMile, TRack, SLide, SWing". Those are pretty tough for her, but she CAN do it, I'm just working on the consistency of saying them correctly now. 5) The last goal is working on R sounds. This one is similar to how we target the L sounds, so its just the R sound by itself, the R with syllables and then simple R words like "red, rock, rabbit, rainbow."
