Speech Assessment Summary: Aaron is a 5 year 9 month old male who has been receiving speech and language therapy services at New Beginnings Pediatric Therapy Services. Aaron was reevaluated using the PLS-5 English Edition and the Goldman-Fristoe Test of Articulation Third Edition English (GFTA-3). The English edition was utilized in place of the dual language edition due to parent report and patient use of English more than Spanish. Moreover, Aaron primarily uses and understands English in therapy sessions. He received the following scores: Auditory Comprehension standard score: 71 (raw score: 45, percentile rank: 3, age equivalent: 3 years and 11 months). Expressive Communication 62 (raw score: 38, percentile rank: 1, age equivalent: 3 years and 3 months). Total Language standard score 64 (raw score: 81, percentile rank: 1, age equivalent: 3 years 6 months). According to Aaron’s standard scores, he continues to present with a SEVERE MIXED RECEPTIVE-EXPRESSIVE LANGUAGE DISORDER. Aaron requires speech therapy services for the following age-appropriate concepts: post-noun elaboration, identifying advanced body parts, using possessive -s, and describing the function of objects. The GFTA-3 was also administered to determine if Aaron’s speech sounds are appropriate for his age. Aaron received the following standard score on the GFTA-3: Sounds-in-Words 75 (raw score: 31, percentile: 5, age equivalent: 3 years 2 months to 3 years 3 months). 06-03-2025Christine AdameThis supervising clinician spoke with the patient's mother on May 23, 2025 at 4:40 pm. Mother spoke in a calm voice. The clinician heard a lot of background noise, perhaps mother was at the store or driving. The clinician asked mother if she could speak with mother regarding the patient's re-evaluation results. Mother stated that she was available to speak with the clinician. The clinician explained that the patient received the following standard scores: Auditory Comprehension 67 and Expressive Communication 63. The clinician explained that these scores are considered "Severe." The clinician explained the scoring intervals relating to Profound, Severe, Moderate, Mild, and Average. Mother was also informed that the patient has made progress on all his goals, however, he only mastered two of them with an 80% accuracy or more. The clinician explained the new concepts that will be added to the patient's short term goals. Mother stated that she understood. Mother stated that she appreciated the in-depth explanation that was provided because noone had ever explained the scores to her before. The clinician explained that the previous re-evaluation that was performed on the patient was administered by this clinician. This clinician stated that she went into the therapy room to test the patient. The clinician explained that she informs the assistants of the patient's scores so that they can let the parents know if the patient still qualifies or not. Mother apologized that she did not fully understand the scores when the clinician explained them to mother after the re-evaluation. Mother stated that since she had all her children she was not completely focused on the clinician. Mother stated that since the patient is still considered severe, she didn't understand why the treating clinician, Janette, said that he was making progress and "doing good in therapy." The clinician explained that although he is making progress, he is still considered to have standard scores in the severe range. Mother stated that she understood. Mother was not upset at the beginning or completion of the phone call. Phone call was completed. Phone call duration: 11 minutes and 53 seconds.
06-02-2025Janette GomezMom was very concerned with how Supervisor Christine said patient was "very severe". Mom told therapist at the end of his last session that after the re-evaluation, Christine told her that patient was "very severe". Mom told Therapist that due to having all her children there she did not ask anymore questions, but went back to try and talk to her. The office told her she would give her a call back, but didn't. Mom was very upset and concerned as to why she would say "very severe" when she sees patient improve and talk so much more. Mom was very upset with the information and how there was no explanation. Therapist told Supervisor so she could call and talk to patient's mom.

As always, we at New Beginnings strive to keep our patients, parents, therapists, and staff safe and healthy. We have thermometers in our front lobbies and will be checking for high temperatures as we begin the traditional time of year for an increase in flu, colds, and COVID. Any child or parent with a temperature of 100 degrees and above will be asked to reschedule for another day & time.
If you or your child or someone in your home has COVID, we continue to ask that you stay home and quarantine to stop the spread. The CDC urges people sick with symptoms of respiratory viruses to stay home until their fever has disappeared for at least 24 hours without relying on fever-lowering medications.

Please do not come to therapy if you or your child are experiencing the following symptoms: fever, body aches, chills, vomiting, diarrhea, headache. Your treating therapist would be happy to reschedule your visit for you.
