Occupational Background History: Katilyn is a 12.0 year old girl who was seen on this date for a scheduled occupational therapy evaluation in order to obtain current level of function. Mother Sara, was present in order to serve as the primary informant throughout the evaluation. Per mother's report child was born full term. Child did not experience any complications at birth, does not have any visual or hearing deficits. Child does have history of anxiety during daily routine and self regulation difficulties leading to aggressive outbursts.

Occupational Background Summary: Katilyn demonstrates anxious and adverse behaviors highly likely due to AU diagnosis throughout her day hindering ADLs and IADLs throughout her day. Child reports finding difficulty to express herself and control her behaviors. Mother reports that child's anxious behaviors severely affects relationships at home, having difficulty interacting with siblings and parents leading to prolonged and severe meltdowns.
Occupational Diagnosis/Impressions: Katilyn demonstrates deficits in fine motor precision, fine motor integration, visual motor integration, manual dexterity, manual coordination, sensory processing and self-regulation, which have impaired their ability to complete activities of daily living and instrumental activities of daily living that compromise their routines and roles at home and in the community. Child would greatly benefit from skilled OT interventions in order to address their established delays and facilitate functional independence and performance. Home programming alone is not sufficient to make the needed progress, direct one to one services are required for progress to be achieved. Occupational Assessment Summary: Per clinical observations, parental report and functional assessment scores Katilyn demonstrates functional gains from skilled occupational therapy services. Child was assessed via standardized assessment BOT-2, Katilyn demonstrates -2 to -3 standard deviations below her peers and her chronological age of 15 years and 1 month, child age equivalency scores are between 5 years old and 6 months to 5 years and 9 months placing a significant gap between her and her peers. Previously her scores fell under 5 years old, so demonstrates gains of 2+ years. Katilyn demonstrates moderate cognitive delays hindering executive functioning skills, functional behavior and safety. She has great difficulty understanding emotions, feelings and differences between emotions and feelings. She easily becomes overwhelmed and can not explain how she feels, although she has demonstrated great improvement with up to 6 weeks without a tantrum with implementation of self regulation techniques. Occupational Treatment Plan: Katilyn is a 15-year, 1-month-old female who demonstrates functional gains from skilled occupational therapy services, as evidenced by standardized assessment scores on the Bruininks-Oseretsky Test of Motor Proficiency, Second Edition (BOT-2). While she continues to present with significant delays, with scores falling -2 to -3 standard deviations below same-aged peers and yielding an age equivalency between 5 years, 6 months and 5 years, 9 months, this reflects a notable improvement of over two years since her previous assessment. Despite this progress, Katilyn's participation in age-appropriate activities of daily living (ADLs) and instrumental activities of daily living (IADLs) remains significantly impacted by moderate cognitive delays affecting executive functioning, sensory processing deficits, poor safety awareness, and impulsivity, which compromise her functional behavior and require close supervision. The patient has made measurable progress in self-regulation, demonstrating the ability to utilize learned strategies to remain calm for up to six weeks without a tantrum, which directly supports her ability to engage in functional tasks. The patient's intrinsic cognitive and sensory-based deficits present as barriers to a more rapid rate of progress; however, her demonstrated ability to learn and apply new skills, combined with consistent parental support, indicates a good rehabilitation potential for continued benefit from skilled intervention. A frequency of one 45-60 minute session per week is medically necessary to provide the direct, one-to-one skilled intervention required to build upon recent gains, address her significant remaining deficits in motor planning and sensory regulation, and provide caregiver training to ensure carryover of skills, as a home program alone is insufficient to address the complexity of her needs. As functional goals remain unmet, continued skilled occupational therapy is medically necessary to reduce the significant gap between her functional performance and chronological age, prevent regression of skills, and support the achievement of remaining long-term outcomes for improved safety and functional independence at home and in the community.

Assessment of Goals/Response to Treatment: Katilyn completed a self assessment activity x 45 minutes with mod A from therapist for facilitation of questions and responses. Child was able to answer all questions with fair accuracy. Questions allowed for emotional regulation in order to express self as it is difficult for child to communicate when frustrated. Currently working on self expression when child is regulate in order to create the routine. Child transitioned to sensory calming activity as a preferred task with no aversion to task. To start the new year right, we are sending some reminders of our policies. 1) Please make sure to inform your front office of any changes to your a) contact phone number, b) your address, c) your insurance, d) your pediatrician. We cannot serve you to the best of our ability if you do not inform us of changes to your personal information in a timely manner. Not informing us of changes may result in delayed authorization. 2) New Beginnings is not a licensed daycare, as such, please be advised that you cannot leave your child in therapy alone. You must remain on premises at all times should your child need assistance with toileting, behavior, or feeding. When you signed our paperwork at the initiation of therapy, you signed an understanding of what our policies and procedures are. Any parent who has a pattern of leaving our premises while their child is in therapy, unfortunately, will be subject to discharge. 3) We have a courtesy HOLD of 30 days and no more whether for change in insurance or any other extenuating circumstance. All 30-day HOLDs must be cleared with the front office and the Clinic Director. 4) We adhere strictly to our 90% attendance policy. If your child is not in therapy, they cannot learn and make progress. Please work with your treating therapist to do all makeups and keep your attendance up. We are here to help your child and cannot do so if they have poor attendance. 5) All cancellations or rescheduling of visits should be made with as much notice as possible. We have thermometers in our front lobbies and will be checking for high temperatures as we begin the traditional time of year for an increase in flu, colds, and COVID. Any child or parent with a temperature of 100 degrees and above will be asked to reschedule for another day & time. If you or your child or someone in your home has COVID, we continue to ask that you stay home and quarantine to stop the spread. The CDC urges people sick with symptoms of respiratory viruses to stay home until their fever has disappeared for at least 24 hours without relying on fever-lowering medications. Due to a recent change in Tricare’s process, all patients are now required to be seen by their Primary Care Provider (PCP/PCM) and have an updated referral submitted to Tricare before we can request authorization for therapy. Please note: -The referral end date provided by your PCP/PCM will determine the authorization period for therapy services. -If possible, we recommend requesting a referral period of up to one year, as this will help reduce the number of required follow-up visits. -If you are unsure whether a current referral is on file, please contact your PCP/PCM or Tricare Referral Management directly. This change began earlier this month. While we are actively reviewing all existing referrals, we kindly ask for your assistance in scheduling an appointment with your PCP/PCM and ensuring an updated referral is submitted to help avoid any delays in therapy services. If you are able to view your referral end date, please be aware that your child will need to be seen again before that date to continue services.
