Mother Sara, was present in order to serve as the primary informant throughout the evaluation. Per mother's report child was born full term. Child did not experience any complications at birth, does not have any visual or hearing deficits. Child does have history of anxiety hindering socialization and interaction during daily routine and self regulation difficulties.

Occupational Background Summary: Alexis demonstrates severe social anxiety and generalized anxiety throughout her day hindering ADLs and IADLs throughout her day. Child reports feeling out of place, not fitting in, and difficulty to socialize and make any friends. Anxiety also hinders scores and activities due to feeling rushed or that she is not performing well. Mother reports that child's anxious behaviors severely affects relationships at home, having difficulty interacting with siblings and parents leading to prolonged and severe meltdowns.
Occupational Diagnosis/Impressions: Child demonstrates deficits in socialize skills, fine motor precision, fine motor integration, visual motor integration, manual dexterity, manual coordination, sensory processing and self-regulation, which have impaired their ability to complete activities of daily living and instrumental activities of daily living that compromise their routines and roles at home and in the community. Child would greatly benefit from skilled OT interventions in order to address their established delays and facilitate functional independence and performance. Home programming alone is not sufficient to make the needed progress, direct one to one services are required for progress to be achieved. Recent assessment utilizing the Bruininks-Oseretsky Test of Motor Proficiency, Second Edition (BOT-2) indicates that while Alexis has achieved approximately two years of developmental gains, her overall performance remains significantly delayed, falling at least -2 standard deviations below the mean for her chronological age. Her functional skills are commensurate with an age equivalency of 7 years, 6 months to 7 years, 11 months, highlighting a substantial gap in age-appropriate functioning. Significant deficits persist in cognitive domains, particularly impacting executive functioning skills, which contribute to impulsivity and poor safety awareness that necessitates close supervision. These cognitive delays, coupled with impaired sensory processing and self-regulation, continue to limit her independence in activities of daily living (ADLs) and instrumental activities of daily living (IADLs) across home and community settings. Subjective: Patient was alert and cooperative,

Assessment of Goals/Response to Treatment: Patient presented as alert and was cooperative with the therapeutic agenda for today's session. Skilled intervention targeted improvements in self-regulation, attention, and visual-motor integration through participation in a structured sensorimotor activity. During a fine motor task requiring problem-solving and in-hand manipulation, the patient demonstrated emerging progress toward sustained attention goals. The patient was able to maintain focus for approximately 4 minutes with moderate verbal and tactile cues needed for redirection, showing an increase from the baseline of 0%. As the task complexity increased, the patient began to display signs of frustration and emotional dysregulation. With moderate verbal and gestural assistance, the patient was prompted to select a coping strategy and successfully engaged in deep breathing exercises, demonstrating progress toward the goal of independently choosing and completing self-regulation techniques (45% progress). Following the implementation of this strategy, the patient was able to return to a regulated state within approximately 12 minutes with minimal verbal reassurance, indicating progress toward the recovery goal (50% progress). The patient continues to require skilled intervention to improve initiation of coping skills and to decrease the time required to return to a calm state following dysregulation.

08-01-2024	Saul	Alexis was seen on 7/31/24 (by Leslie) as well as sister Katilyn (by Saul). Mother was present entire session due to meltdown that occurred in the home during the weekend. Mother reports Alexis verbally triggered sister by whispering insults to her sister knowing her sister will get triggered and lead to a physical meltdown forcing parents to get involved. The two siblings were seen separate although joined the last 10-12 minutes for mother to discuss consequences to the behaviors. Mom called therapist (Saul) today at 6pm to say that Alexis went to school and told her coach that her sister was allowed to beat her up during the session and was hit several times. Mother reports incident was reported to principal and CPS. Later Alexis admitted to lying about the incident because she was angry at therapist and mother. She did not like to have any consequences and decided to lie in hopes of getting back at her mother and sister.

To start the new year right, we are sending some reminders of our policies. 1) Please make sure to inform your front office of any changes to your a) contact phone number, b) your address, c) your insurance, d) your pediatrician. We cannot serve you to the best of our ability if you do not inform us of changes to your personal information in a timely manner. Not informing us of changes may result in delayed authorization. 2) New Beginnings is not a licensed daycare, as such, please be advised that you cannot leave your child in therapy alone. You must remain on premises at all times should your child need assistance with toileting, behavior, or feeding. When you signed our paperwork at the initiation of therapy, you signed an understanding of what our policies and procedures are. Any parent who has a pattern of leaving our premises while their child is in therapy, unfortunately, will be subject to discharge. 3) We have a courtesy HOLD of 30 days and no more whether for change in insurance or any other extenuating circumstance. All 30-day HOLDs must be cleared with the front office and the Clinic Director. 4) We adhere strictly to our 90% attendance policy. If your child is not in therapy, they cannot learn and make progress. Please work with your treating therapist to do all makeups and keep your attendance up. We are here to help your child and cannot do so if they have poor attendance. 5) All cancellations or rescheduling of visits should be made with as much notice as possible. As always, we at New Beginnings strive to keep our patients, parents, therapists, and staff safe and healthy. We have thermometers in our front lobbies and will be checking for high temperatures as we begin the traditional time of year for an increase in flu, colds, and COVID. Any child or parent with a temperature of 100 degrees and above will be asked to reschedule for another day & time. 10-23-2024 10:50 AM	Stephen Tullius	Good morning! I want to thank all the Beta users of our app for sharing some great ideas (and some bugs too). Here are some things that we have fixed/added: 1.) When you clicked on the phone notification about a message it was not taking you to the right place. This should be fixed now. 2.) There was an issue when typing in the message response where the letters were going vertically...fixed. 3.) There were some date issues showing up on the calendar....those should also be fixed. 4.) App users were going crazy because they were getting both text messages as well as phone notifications. I can imagine how annoying that was :) This should now be fixed and app users should ONLY be getting phone notifications. Please let me know if you are still getting any text messages. 5.) All notifications that you click on should take you to the correct location in the app. If you get any notification that does not take you where it should, please let me know. 6.) There was an issue where the app would freeze when the phone back arrow was clicked. That should be fixed now. 7.) We added phone notifications for Check In, for Payment links, and for Signing therapy notes instead of getting those by text message. I believe you should not be getting any more messages by text. If you are, please let me know which and I will integrate into the app. 8.) We added Patient Goals to the left side menu to display current short term goals for your child. This will allow you to see what the therapist will be working on in therapy. Every new auth period will display new goals there. It will show for each therapy your child is receiving...Speech, Feeding, OT. 9.) We added a Reports tab on the left side menu to display Evaluations and Plans of Care for your children. These only change once per auth but will allow you to see earlier reports as well so you can review where your child started and where they are now. Over the next couple days, we will be working on our Resources section. The resources have 2 parts....Diagnosis Resources that will be applied at the initial evaluation to help parents understand their child's diagnosis and the types of things we will be working on. Since you all have been here for a while that won't really apply to you. The other type of Resources we are working that does apply to you are Session Resources. The Session Resources will be Homework and also end of session videos from therapists. We are building these modules now. NOTE: You may see some TEST Resources get added to your app and you should get a notification that you got a new resource. This is us building and testing the system. We will always put TEST in the resource name so that you will know it is a TEST. Please feel free to click on the notifications and review the test resources and let me know if something is not working properly. Just please understand that any content in the resource is NOT for your child, it's just some random test materials we copied and pasted or something. Anyway....sorry for the LONG message, but wanted to give you all an update. As always, please send me any issues, or ideas for improvement. I am trying to make this into a very useful tool to help you parents with managing all patient schedules and messaging and homework all in one place. Thank you and have a great day!

Due to a recent change in Tricare’s process, all patients are now required to be seen by their Primary Care Provider (PCP/PCM) and have an updated referral submitted to Tricare before we can request authorization for therapy. Please note: -The referral end date provided by your PCP/PCM will determine the authorization period for therapy services. -If possible, we recommend requesting a referral period of up to one year, as this will help reduce the number of required follow-up visits. -If you are unsure whether a current referral is on file, please contact your PCP/PCM or Tricare Referral Management directly. This change began earlier this month. While we are actively reviewing all existing referrals, we kindly ask for your assistance in scheduling an appointment with your PCP/PCM and ensuring an updated referral is submitted to help avoid any delays in therapy services. If you are able to view your referral end date, please be aware that your child will need to be seen again before that date to continue services. We appreciate your patience and cooperation during this transition. If you have any questions, please don’t hesitate to contact our office. Thank you for your time and support.
