Speech Background History: Francisco is a 5 year, 6 month old male who was referred by his pediatrician for a speech and language evaluation due to speech and language concerns. Speech Assessment Summary: Francisco is an 8 year, 2 month old male who was re-evaluated today to determine continued eligibility of speech and language services. Francisco arrived to the re-evaluation accompanied by his grandmother, however, she was not present during the evaluation. The Clinical Evaluation of Language Fundamentals (CELF-5) was used to assess his expressive and receptive language skills. Evaluation was administered in English, as it has been observed to be his dominant language and the language he communicates in 100% of the time. The following results were found: Standard Score 68; Sum of Scaled Scores 17; Percentile Rank 2 His core language was assessed, which encompasses the following areas: sentence comprehension, word structure, formulated sentences, and recalling sentences. Francisco presents with a SEVERE expressive/receptive language disorder based on his results. He would continue to benefit from speech and language targeting the following areas: understanding regular and irregular plurals, possessive nouns/pronouns, understanding irregular past tense verbs. Francisco would also benefit from memory recall, as he continues to exhibit difficulties with recalling sentences, and formulating sentences. The Clinical Assessment of Articulation and Phonology Second Edition (CAAP-2) was administered to determine whether Francisco has acquired age-appropriate articulation skills. Francisco was required to produce single monosyllabic and single multisyllabic words and school-age sentences. Under the consonant inventory subtest, he achieved a raw score of 21 and standard score of <55, which is more than 3 standard deviations below the mean. Under school-age sentences, he achieved a raw score of 29 and standard score of <55, which is more than 3 standard deviations below the mean. Results indicate that he continues to present with a PROFOUND articulation disorder. Francisco has been noted to make significant progress in therapy when provided with cues, however, he is not independently producing age-appropriate sounds in words, phrases, or conversation. Assessment of Goals/Response to Treatment: Upon arrival pt completed a 7minute GM heavy work activity c mod A via verbal cuing for problem solving and attention redirection. Pt then transitioned well to a 5 minute FM tabletop activity completing a hand dexterity/ strengthening activity c min A via verbal cuing and no rest breaks on this day. Pt then completed a 15 minute tabletop activity transitioning between 3/3 tabletop task c min A via verbal cuing and visual cuing for problem solving. Pt benefited from visual timer for task initiation and termination on this day. To start the new year right, we are sending some reminders of our policies. 1) Please make sure to inform your front office of any changes to your a) contact phone number, b) your address, c) your insurance, d) your pediatrician. We cannot serve you to the best of our ability if you do not inform us of changes to your personal information in a timely manner. Not informing us of changes may result in delayed authorization. 2) New Beginnings is not a licensed daycare, as such, please be advised that you cannot leave your child in therapy alone. You must remain on premises at all times should your child need assistance with toileting, behavior, or feeding. When you signed our paperwork at the initiation of therapy, you signed an understanding of what our policies and procedures are. Any parent who has a pattern of leaving our premises while their child is in therapy, unfortunately, will be subject to discharge. 3) We have a courtesy HOLD of 30 days and no more whether for change in insurance or any other extenuating circumstance. All 30-day HOLDs must be cleared with the front office and the Clinic Director. 4) We adhere strictly to our 90% attendance policy. If your child is not in therapy, they cannot learn and make progress. Please work with your treating therapist to do all makeups and keep your attendance up. We are here to help your child and cannot do so if they have poor attendance. 5) All cancellations or rescheduling of visits should be made with as much notice as possible. If you or your child or someone in your home has COVID, we continue to ask that you stay home and quarantine to stop the spread. The CDC urges people sick with symptoms of respiratory viruses to stay home until their fever has disappeared for at least 24 hours without relying on fever-lowering medications. The front desk notified me that Mrs. Willie called this morning asking to cancel tomorrow's appointment. However, it was reported to me that she prefers phone calls, as she was unaware I scheduled this appointment. I can see here that the appointment was confirmed by you on 02/01. I also wanted to mention that I will need for you to sign a consent form giving permission for Mrs. Willie to bring Frankie to his appointments, as she is not the primary caregiver. I will not be able to see Frankie until this form is signed. I will have the front office email it to you. Numerous repetitions of each. He did very well, still had inconsistencies, but when I corrected him or prompted he was also to correctly produce. I would recommend you continue to practice these sounds because he still requires maximal cues. Let me know if you have questions!

03-09-2026 12:30 PM	Daniella Anchondo	Yes, they're all going to be in the afternoon at 5pm or 5:30pm. The Friday one is also zoom. All of his appointments are now going to be on zoom. I scheduled him earlier this Friday only because kids are on spring break, so I thought we can do that for this week. We can talk about the schedule a little bit more on Wednesday during his session if you would like... just so everything is clear :)
