Assessment of Goals/Response to Treatment: The patient actively participated in today's skilled occupational therapy session, completing all structured activities designed to enhance psychosocial, cognitive, and visual processing skills. During a multi-step sequencing task, the patient required moderate assistance to maintain sustained attention and problem-solve when faced with novel steps. Progress was noted in visual processing, as the patient successfully utilized horizontal and vertical scanning to match items to a sample with increased speed and accuracy. In addressing social regulation goals, the patient required moderate support consisting of 4 verbal reminders to maintain appropriate personal space and respect social boundaries during a turn-taking activity. She demonstrated progress in social reciprocity, responding to conversational prompts with moderate verbal cueing for topic relevance and appropriate tone of voice in 3 out of 5 opportunities. When presented with the non-preferred self-care task of organizing materials, the patient required 5 verbal and gestural prompts to initiate the activity using a visual checklist. However, once initiated, she successfully completed the cleanup of her personal area, putting items away in designated locations within 8 minutes with moderate assistance via the visual schedule, showing improvement in task completion and follow-through.
