Speech Background History: Gizelle is a 3 year 8 month old female who was referred by her pediatrician for a comprehensive Speech and Language evaluation due to unintelligible verbal output. The evaluation was completed in both English and Spanish, as Gizelle is exposed to both English 50% and Spanish 50% of the time at home and on TV per parent report. Mother reported that Gizelle was born at term via natural birth without complications. Mother raised concerns that Gizelle met all of her developmental milestones on time and is attempting to express her wants, needs however she is difficult to understand by both familiar and unfamiliar listeners. She is concerned by her lack intelligible speech. Per parent report, she becomes easily frustrated when trying to communicate and will often times tantrum when she wants or needs something and is not being understood. Mother also raised concerns that Gizelle is a picky eater and that she only walks on her tippy toes. Gizelle’s main languages at home are English and Spanish. Although Gizelle is exposed to both languages she demonstrates more understanding in English and the majority of her limited vocabulary is in English. Since Gizelle attempts to communicate and answer questions in English but is also exposed to Spanish, the formal, standardized assessment was conducted in both English and Spanish. She was assessed using the Dual language English/Spanish Edition Preschool Language Scale 5 (PLS-5).

Speech Assessment Summary: Gizelle is an 8 year 7 month old female and is due for a review of her language development. The language of the evaluation was English, and was determined based on parent report of English exposure via media (YouTube videos, games, and TV), school and the primary language spoken at home. Gizelle was accompanied to the evaluation by her father, however, he was not present during testing. The Goldman-Fristoe Test of Articulation (GFTA-3) was used to assess her articulation/phonological skills and The Clinical Evaluation of Fundamentals (CELF-5) was administered to determine Gizelle's weaknesses and strengths in her Expressive and Receptive Language development. Gizelle's scores were the following: Goldman-Fristoe Test of Articulation (GFTA-3) - Sounds-in-Words: Standard Score 75 (Raw Score 7), Percentile Rank 5 Sounds-in-Sentences: Standard Score 91 (Raw Score 2), Percentile Rank 27 The Clinical Evaluation of Fundamentals (CELF-5) - Core Language Score (CLS): Standard Score 64 (Scaled Score 15), Percentile Rank 1 Gizelle is now presenting with a MODERATE articulation/phonological disorder and a SEVERE expressive language disorder. Gizelle has made significant progress through therapy, however, would continue to benefit from speech and language development. She exhibited consistent /th/ distortions in initial and final word position. Such is error is no longer considered typically developing and the occurrence of these phonological processes and articulation errors in her speech may negatively impact her overall intelligibility. Gizelle exhibited inconsistent errors when producing multisyllabic words. Additionally, she experienced communication breakdowns at the conversational level as she verbalizes longer and more complex sentences. The core language subtest looks for errors in the domains of sentence comprehension, word structure, formulated sentences, and recalling sentences. Gizelle's main weaknesses under word structure were in using subjective pronouns, uncontractible copula/auxiliary, and auxiliary + verb ending in -ing. Gizelle was noted to formulate grammatically-correct sentences, however, she had difficulties in producing some as they increased in complexity. Gizelle demonstrated main difficulties in recalling sentences Based on formal assessment, parent interview, and clinical observation, Gizelle presents with a SEVERE expressive language disorder, a MODERATE articulation disorder. Gizelle’s deficits negatively impact her ability to communicate effectively in academic and social settings. Persistent articulation errors reduce speech intelligibility, leading to frequent communication breakdowns during conversation. Language deficits in sentence comprehension, grammatical structure, and sentence recall interfere with her ability to understand classroom instruction, follow directions, and express ideas clearly as linguistic demands increase. Speech-language therapy is medically necessary to improve intelligibility, receptive and expressive language skills, and functional communication to support participation in the educational environment and age-appropriate social interactions.

This is a general message to all parents, do not text this number to reply. Happy New Year from Team New Beginnings! To start the new year right, we are sending some reminders of our policies.



1) Please make sure to inform your front office of any changes to your a) contact phone number, b) your address, c) your insurance, d) your pediatrician. We cannot serve you to the best of our ability if you do not inform us of changes to your personal information in a timely manner. Not informing us of changes may result in delayed authorization.



2) New Beginnings is not a licensed daycare, as such, please be advised that you cannot leave your child in therapy alone. You must remain on premises at all times should your child need assistance with toileting, behavior, or feeding. When you signed our paperwork at the initiation of therapy, you signed an understanding of what our policies and procedures are. Any parent who has a pattern of leaving our premises while their child is in therapy, unfortunately, will be subject to discharge.



3) We have a courtesy HOLD of 30 days and no more whether for change in insurance or any other extenuating circumstance. All 30-day HOLDs must be cleared with the front office and the Clinic Director.



4) We adhere strictly to our 90% attendance policy. If your child is not in therapy, they cannot learn and make progress. Please work with your treating therapist to do all makeups and keep your attendance up. We are here to help your child and cannot do so if they have poor attendance.

5) All cancellations or rescheduling of visits should be made with as much notice as possible. We ask for 24 hours in advance notice when you know you cannot make a visit.

We have thermometers in our front lobbies and will be checking for high temperatures as we begin the traditional time of year for an increase in flu, colds, and COVID. Any child or parent with a temperature of 100 degrees and above will be asked to reschedule for another day & time.
If you or your child or someone in your home has COVID, we continue to ask that you stay home and quarantine to stop the spread. The CDC urges people sick with symptoms of respiratory viruses to stay home until their fever has disappeared for at least 24 hours without relying on fever-lowering medications.

Please do not come to therapy if you or your child are experiencing the following symptoms: fever, body aches, chills, vomiting, diarrhea, headache. Your treating therapist would be happy to reschedule your visit for you.

We wanted to take a moment to keep you informed about an issue that may affect your experience with us. Tricare is currently experiencing technical difficulties with their technology systems and initial and re-authorizations. As a result, there may be temporary delays in authorizations for your child’s therapy.

Please know that we are closely monitoring the situation and working with Tricare to resolve the issue as quickly as possible. They have stated a need of 4-8 weeks for their system issues to be resolved as their subcontractor insurance switched from Healthnet Federal Services to Triwest Alliance on 01/01/2025. We sincerely apologize for any inconvenience this may cause and appreciate your understanding during this time.

We hope this message finds you well. We wanted to inform you that we recently submitted a request for authorization for speech therapy services to El Paso Health on Gizelle’s behalf. Unfortunately, the request was denied.

We inquired with El Paso Health regarding the possibility of submitting an appeal; however, at this time, we are unable to proceed with one. As a result, we are no longer able to provide services for Gizelle under El Paso Health coverage.

That said, we do offer a self-pay option if you wish to continue services. Please let us know if this is something you would be interested in, and we would be happy to provide you with pricing information and next steps.

We sincerely apologize for any inconvenience this may cause. If you have any questions or need further clarification, please don't hesitate to reach out.
