Feeding Background History: Dante is a 9-year 4-month-old male referred by his pediatrician for a clinical feeding evaluation due to parent concerns regarding acceptance of advanced textures and solids. Dante’s mother served as the informant for the evaluation. Dante’s mother reported that feeding concerns began when he was approximately 2-3 years old reporting difficulty transitioning from from breastfeeding to purees and solids. Dante’s mother reported that he currently consumes a wide variety of foods, however, all intake is limited to a pureed consistency. These pureed foods are fed via syringe by his mother as Dante does not utilize a spoon for feeding. Solid food intake is significantly limited consisting of only of snack like foods (e.g.,Goldfish crackers, chips, cookies). No additional solid food textures are reported in the Dante’s diet. Dante’s mother reports instances of constipation, however, he has not been seen by a gastroenterologist. Dante’s mother reported that his pediatrician does not currently have concerns regarding nutrition/growth. Dante’s feeding and swallowing skills were assessed through a clinical feeding evaluation and caregiver interview to determine his current level of function and need for skilled intervention.
Feeding Medical History: Dante’s mother reported that she experienced hyperemesis gravidarum during pregnancy. Dante was delivered at 38 weeks via emergency C-section following prolonged labor. Dante’s mother reported that he was hospitalized when he was 4 years old due to constipation. Dante’s mother reported no known medical or food allergies. Dante was diagnosed with autism when he was approximately 2 years old. Additionally, Dante was diagnosed with ADHD approximately 1 year ago.

Feeding Assessment Summary: Skilled feeding therapy is recommended to address Dante's chronic pediatric feeding disorder and oral phase dysphagia and is deemed medically necessary to support expansion of food variety, improve tolerance of non-preferred textures, and promote the development of age-appropriate oral motor skills. Therapy should utilize a child-led, sensory-based approach, and systematic desensitization strategies to gradually increase Dante’s tolerance for non-preferred foods. Feeding therapy is to target texture progression using the IDDSI framework, with gradual advancement from preferred pureed textures toward minced and moist and mixed consistencies as tolerated and as when oral motor skills are deemed appropriate for these texture specific tasks. This progression will support increased oral sensory input and facilitate development of more efficient chewing patterns. Oral motor skill intervention is recommended to address immature chewing skills, inconsistent tongue lateralization, and lack of a rotary chewing pattern. Food chaining strategies are recommended to bridge from Dante's preferred foods and textures toward more advanced and age-appropriate textures. Without feeding therapy, Dante remains at risk for ongoing selective eating, limited nutritional variety, and delayed oral motor skill development. Additionally, an MBSS is recommended before trialing advanced textures.

Feeding Rehabilitation Potential: Rehabilitation potential is good with consistent feeding therapy and parental support. Dante will be discharged from feeding therapy when long-term and short-term therapy goals are met indicating adequate feeding skills to orally prepare age-appropriate foods for safe swallowing.

Feeding/Swallowing	In 6 months, Dante will transition from accepting preferred purees via syringe to accepting preferred purees via utensil in 3/5 opportunities in order to allow for the appropriate progression to utensil use.
