This is the front office at New Beginnings. We are currently reviewing Marcelo’s registration and, unfortunately, we are unable to use the referral that was uploaded.
The referral is addressed to a different clinic, and the patient name on the document does not match Marcelo’s. Additionally, the diagnosis code provided is for occupational therapy, while speech therapy was selected during registration.
In order for our clinic to proceed with services, we will need a new referral that includes:
Our clinic listed as the provider
The correct patient name (Marcelo)
Appropriate diagnosis codes for speech therapy (if that is what the PCP is requesting)
