We would like to inform you of an important update regarding therapy services. Due to a recent change in Tricare’s process, all patients are now required to be seen by their Primary Care Provider (PCP/PCM) and have an updated referral submitted to Tricare before we can request authorization for therapy.

Please note:
-The referral end date provided by your PCP/PCM will determine the authorization period for therapy services.
-If possible, we recommend requesting a referral period of up to one year, as this will help reduce the number of required follow-up visits.
-If you are unsure whether a current referral is on file, please contact your PCP/PCM or Tricare Referral Management directly.

This change began earlier this month. While we are actively reviewing all existing referrals, we kindly ask for your assistance in scheduling an appointment with your PCP/PCM and ensuring an updated referral is submitted to help avoid any delays in therapy services.

If you are able to view your referral end date, please be aware that your child will need to be seen again before that date to continue services.

We appreciate your patience and cooperation during this transition. If you have any questions, please don’t hesitate to contact our office.

I will be conducting Samuel's speech and language evaluation. :) Before we schedule our in person appointment, I just need to gather some background information. So when you have an opportunity can you answer these questions for me: 1.) Any complications with pregnancy or birth? 2.) major illnesses or accident since last check up? Any medical diagnoses (i.e., Autism, ADHD, down syndrome, etc.)? 3.) developmental milestones met within normal limits? or delayed? 4.) sleeping or breathing issues? does patient get 8-10 hrs sleep? wake up several times a night? snoring? sleep with mouth open or closed? enlarged tonsils? 5.) medications? 6.) education? (i.e., day care, preschool) 7.) vision or hearing issues? 8.) In your a few words write your parental concerns with SPEECH and LANGUAGE along with your child’s strengths. 9.) when did your concerns begin? 10) Any history of previous therapy (i.e., speech, occupational therapy, physical therapy, counseling, etc.)

No Birth Complications

Yes he gets 8-10 hours of sleep, sometimes wakes up and Does snore, also sleep with mouth open often.

None, only a multi vitamin a day.

6.) education? (i.e., day care, preschool)

He is homeschooled, very smart.

7.) vision or hearing issues?

None

8.) In your a few words write your parental concerns with SPEECH and LANGUAGE along with your child’s strengths.

Samuel only struggles with pronouncing Rs, Ls, THs and sometime has a small lisp. He communicates great and is high intelligent and social for his age.

He was born at a Birthing center at 9 months and 2 weeks all natural Vaginal birth.

2) At what age he begin to use his first words, and at what age did he begin to use combined words?

I would say around 1 he did single words and 2 he had sentences.


We are going out of town until April 7th. Do you have any availability the 3rd week of April (14th-17th)
